Treatment of hepatitis C virus in HIV patients: a review.

BACKGROUND: Survival in HIV infection has improved dramatically in the last decade due to antiretroviral therapy (ART). Due to shared routes of transmission, HCV is found in approximately 30% of HIV infected patients. HCV infection has emerged as a major issue in this population as manifest by a major increase in liver-related mortality. ART-associated hepatotoxicity has been demonstrated to occur more frequently in co-infected individuals and may be severe or fatal in some instances. Thus treatment of HCV has become a priority in HIV infected individuals. OBJECTIVES: The main aims of this re

Visual side effects of pegylated interferon during therapy for chronic hepatitis C infection.

Ocular toxicity, including retinopathy, optic neuropathy and ocular loss, has been infrequently (<1%) reported as a potentially serious adverse event associated with standard interferon therapy. The new pegylated interferons have improved pharmacokinetics which translates to better antiviral efficacy, however, this improved pharmacokinetic profile also has the potential to alter the frequency and extent of their adverse events. We describe a case of chronic hepatitis C infection that developed visual complaints after one month of pegylated interferon, and retinopathy confirmed on ophthalmologi

Essential thrombocythemia (ET): moving from palliation to cure.

Essential thrombocythemia (ET) is a clonal hematopoietic stem cell myeloproliferative disorder characterized by megakaryocytic hyperplasia and persistent thrombocytosis. The clinical presentation and evolution of ET are heterogeneous. This review highlights the current treatment options in the management of ET, including hydroxyurea, anagrelide and both regular and pegylated interferons. Anagrelide, while very effective at controlling counts and symptoms in most patients, may not consistently reduce the bone marrow megakaryocyte mass. Interferon is very effective and not associated with leukem

Oropharyngeal pemphigus in a patient with chronic hepatitis C during interferon alpha-2a therapy.

There are a few reports in the literature concerning pemphigus induced by interferon given for hepatitis C. We present the case of a 28-year-old woman with post-transfusional chronic hepatitis C who developed ulcers and vesicles on her tongue, cheeks, posterior oropharynx and vocal cords 5 months after beginning treatment with recombinant interferon alpha-2a. The direct and indirect immunofluorescence was diagnostic of pemphigus vulgaris. The drug was promptly withdrawn; the patient was medicated with prednisolone and azathioprine and recovered only 3 months later. Although there are several p…

Effects of interferon-alpha-2b and octreotide on healing of esophageal corrosive burns.

OBJECTIVE: The aim of this study was to investigate the effect of interferon-alpha to 2b and octreotide on the treatment of esophagus corrosive burns. METHOD: The experimental study was performed on 63 rabbits. Burn was obtained by treating a segment, isolated in cervical esophagus, with NAOH. The rabbits were allocated into three groups (control, interferon, and octreotide). On the fourth, eighth, and 20th days, 7 rabbits randomly selected from each group were killed. Comparisons among the groups were made by taking histopathologic findings, stenosis index, and hydroxyproline production into

Recombinant alpha-2a interferon treatment in a child with T-cell leukemia and chronic hepatitis B.

We present the case of a 12-year-old boy with T-cell acute lymphoblastic leukemia (ALL) who developed a chronic hepatitis-B virus (HBV) infection during the consolidation phase of chemotherapy and was unable to receive further therapy because of hepatotoxicity. Recombinant alpha-2a interferon (alpha-IFN) treatment (5 million units/m2 per dose, three times a week) was started for chronic HBV infection at the end of the sixth month, and vincristine (1.5 mg/m2) was administered once a month as the only well-tolerated chemotherapeutic agent. During follow-up, the dose of alpha-IFN was increased to

Alpha interferon as initial treatment of essential thrombocythemia. Analysis after two years of follow-up.

AIMS AND BACKGROUND: Recombinant alpha-interferon has been shown to be effective in essential thrombocythemia and in thrombocytosis associated with other myeloproliferative disorders. PATIENTS AND METHODS: Twenty-five untreated patients were enrolled in our study from May 1989 to April 1992. Recombinant alpha interferon-2b was administered at an initial dose of 2 megaunits (MU)/m2 three times a week at escalating doses to 5 MU/m2 or the maximum tolerated dose. The mean follow-up for patients still in treatment at the time of this report was 35.9 months (range, 24-63). RESULTS: Fourteen patie

[Antiviral effect of recombinant interferon-alpha on hepatitis A virus replication in human liver cells].

Two recombinant interferons-alpha (IFNs-alpha) were assayed for their antiviral effect on hepatitis A virus (HAV) replication in the human hepatoma cell line PLC/PRF/5. IFN alpha-2a and IFN alpha-2b resulted in concentration-dependent inhibition of HAV antigen expression and HAV infectivity at non toxic concentrations. Their selectivity indices, calculated as the ratio of the dose that reduced the number of viable cells to 50% (CD50) to the effective dose that inhibited 50% of viral antigen expression (ED50) were > 1000. Recombinant IFN-alpha emerged, from the present study, as a promising can

Essential thrombocythemia--clinical features, therapy and follow-up of 12 cases.

The authors analyzed twelve patients with symptomatic essential throthrombocythemia (E.T.) diagnosed from 1983 to 1991. Haemorrhagic and thrombotic phenomena were the main presenting features. Treatment consisted mostly of alpha-interferon (IFN-alpha 2b) subcutaneously in dosage ranging from 3 to 5 MU/m2 and hydroxyurea (HU) in conventional dosage. A clinical response was seen in seven patients treated with IFN-alpha 2b (4 CR and 2 PR), and in four patients treated with HU (3 CR and 1 PR). No significant side effects were observed. Our findings indicate that IFN-alpha 2b offers a non-leukaemog

alpha Interferon in the management of essential thrombocythaemia.

Thirteen patients (mean age 60.7 years; female:male ratio 10:3) with essential thrombocythaemia were treated with 3 million units (MU)/day interferon alfa-2b subcutaneously (s.c.) for 12 weeks, with all patients requiring a dose reduction after 4 weeks. The mean pretreatment platelet count was 1,400 x 10(9)/L and megakaryocytes were increased in all cases. Splenomegaly was present in six patients and haemorrhagic phenomena were observed in two. Nine patients (69.2%) had objective responses, including two (15.4%) complete responses (platelets less than 450 x 10(9)/L) which were then maintained

[Recombinant interferon alpha-2A in the treatment of HIV-associated Kaposi sarcoma. Long-term results].

47 HIV-positive male patients with cutaneous Kaposi's sarcoma (KS) were treated with recombinant interferon alpha-2A (IFN) in an open, prospective study. 45 patients received 18 million I.U. IFN s.c. daily for the first three months. From the fourth month, the patients received 18 million I.U. IFN s.c. three times weekly. Four of the 45 patients withdrew from therapy because of side effects during the first weeks (weeks 2 to 6) of treatment. The remaining 41 patients were treated for a minimum period of two years or until their death. Two further patients received 36 million I.U. IFN daily for…

Impaired interferon production and natural killer cell activation in patients with the skin cancer-prone disorder, xeroderma pigmentosum.

Xeroderma pigmentosum (XP) is a rare autosomal recessive disorder with sun sensitivity, markedly increased skin cancer susceptibility, and defective DNA repair without consistently identified symptoms of immune deficiency. We examined natural killer (NK) cell activity and interferon production in peripheral blood lymphocytes (PBL) of eight XP patients who had multiple primary skin cancers. The XP patients had normal numbers of T cells and NK cells, as well as normal lymphokine-activated killer cell activity and normal tumor necrosis factor-alpha production. Unstimulated NK cell function was 40

Pediatric conjunctival papilloma managed with staged multimodal therapy: a case report.

BACKGROUND: Conjunctival papilloma is a rare benign ocular surface tumor in children, most commonly associated with low-risk human papillomavirus types 6 and 11. Management can be challenging because of multifocal disease, persistence, recurrence, and the need to preserve healthy conjunctival tissue in young patients. We report a pediatric case of multifocal conjunctival papilloma successfully managed with a staged multimodal approach combining preoperative medical therapy with limited surgical excision. CASE PRESENTATION: A 4-year-old boy presented with multifocal conjunctival papillomatous

Localized palmar mycosis fungoides: A case report.

RATIONALE: Mycosis fungoides (MF) is the most common primary cutaneous T-cell lymphoma, yet palmar MF is extremely rare and easily mistaken for chronic dermatoses such as psoriasis. Delayed recognition can lead to inappropriate treatment and worse outcomes. This case demonstrates how careful histopathology and immunohistochemistry can provide critical diagnostic clarity in atypical presentations. PATIENT CONCERNS: A 63-year-old woman presented with an 8-year history of erythema, dryness, scaling, and fissuring on both palms. The lesions had been misdiagnosed as psoriasis and showed no respons
