Gastroesophageal reflux is associated with an increased risk of gastric cardiac polyps: a case-control study of 140 cases.

BACKGROUND: The pathogenesis of gastric cardiac polyps is not yet clear, and there is little research on their clinical and histopathologic characteristics and correlation with gastroesophageal reflux. Early detection and treatment of premalignant lesions in this area can prevent the development of cancer. We aimed to evaluate the clinical and histopathologic characteristics and risk factors of gastric cardiac polyps to improve the current understanding of the disease. METHODS: Patients diagnosed with gastric cardiac polyps at the Third Affiliated Hospital of Sun Yat-Sen University between Ja

Peutz-Jeghers syndrome: report of 6 cases in a family and management of polyps with intraoperative endoscopy.

BACKGROUND/AIMS: Peutz-Jeghers syndrome is an uncommon, autosomal dominantly inherited disorder characterized by mucocutaneous melanin pigmentation and gastrointestinal hamartomatous polyps. The purpose of this study was to present six cases of Peutz-Jeghers syndrome in a family. METHODS: Enteroclysis, upper and lower gastrointestinal endoscopy, and thyroid, abdominal, and testicular or breast ultrasonography were performed in all subjects. Tumor markers including CEA, alpha-FP, CA 19-9, CA 15-3, and CA 125 were measured. Management of polyps and complications were evaluated. RESULTS: Histor

Colonic Spirochetosis in a 60-Year-Old Immunocompetent Patient: Case Report and Review.

Spirochetes, a genetically and morphologically distinct group of bacteria, are thin, spiral-shaped, and highly motile. They are known causes of several human diseases such as syphilis, Lyme disease, relapsing fever, and leptospirosis. We report a case of colonic spirochetosis in a healthy patient presenting for surveillance colonoscopy. The diagnosis of intestinal spirochetosis was made accidentally during the histological examination of colonic polyps, which were removed during colonoscopy. We also performed an extensive review on intestinal spirochetosis with a focus on clinical presentation

Recurrent small intestine intussusception in a patient with Peutz-Jeghers syndrome.

Peutz-Jeghers syndrome is a rare hereditary autosomal dominant disease caused by a mutation of the tumor suppressor gene serine/threonine kinase 11 located in chromosome 19p13.3. It is characterized by the presence of extensive mucocutaneous pigmentation, especially of the lips and the occurrence of hamartomatous polyps throughout the gastrointestinal tract. Gastrointestinal hamartomas occur predominantly in the small intestine and can become symptomatic leading usually to intestinal obstruction and abdominal pain.We present a case of recurrent intestinal obstruction caused by small bowel intu

[Jejunal intussusception of an inflammatory fibrinoid polyp].

Intussusception of parts of the intestine in adulthood is a rare cause of ileus. In more than 90 % of all cases there is a malignant or benign tumour situated in the parts of the intestine affected. We describe a 55-year-old patient who was admitted to hospital as an emergency because of signs of a proximal intestinal obstruction. Abdominal sonography suggested the diagnosis of a typical jejunal intussusception, whereas gastroscopy yielded no pathological findings. Laparotomy was performed immediately and confirmed the diagnosis. After intraoperative de-invagination 9 cm of jejunum and an infl

Long-term results of polyp clearance by intraoperative enteroscopy in the Peutz-Jeghers syndrome.

PURPOSE: Enteroscopy during laparotomy for Peutz-Jeghers syndrome was introduced in our unit in 1987. Its aim is to achieve more complete polyp clearance and thereby reduce the number of subsequent laparotomies for small intestinal polyps. METHODS: All patients with Peutz-Jeghers syndrome who had undergone intraoperative enteroscopy since its introduction into our unit in 1987 were identified. The numbers of polyps identified by palpation and transillumination and by enteroscopy were recorded. The timing of, indications for, and findings of all subsequent laparotomies were analyzed and compar

Colorectal neoplasms in melanosis coli: a survey in Japan and a worldwide meta-analysis.

PURPOSE: The association between melanosis coli (MC) and colorectal neoplasms remains unclear. Thus, we primarily aimed to clarify the epidemiology of MC in the Japanese population, identify the relationship between the use of anthranoids and MC, and determine the prevalence of detected intestinal lesions in patients with MC. We subsequently conducted a meta-analysis of published data, including our results, to summarize the influence of MC on the prevalence of colonic neoplasms. METHODS: We conducted a retrospective survey in Japan to investigate the effects of MC on intestinal disorders. Th

A Rare Case of Multiple Gastrointestinal Stromal Tumors Coexisting with a Rectal Adenocarcinoma in a Patient with Attenuated Familial Adenomatous Polyposis Syndrome and a Mini Review of the Literature.

BACKGROUND: Multiple gastrointestinal stromal tumors (GISTs) are extremely rare entities that exist either as spontaneous GISTs or as part of various syndromes, such as Carney's triad and type I neurofibromatosis (NF1). Attenuated familial adenomatous polyposis (AFAP) is a variant of familial adenomatous polyposis (FAP) with a milder clinical presentation. Both GISTs and AFAP have been reported to coexist with colorectal cancer, but the coexistence of GISTs and AFAP has never been reported in the literature before. CASE REPORT: A 45-year-old male patient with known AFAP arrived scheduled for

Collagen polyp of the urinary tract: a report of two cases.

Injection of collagen into the urethral or bladder wall has gained popularity as an effective way to control urinary stress incontinence. The same technique has recently been used to improve function of urinary pouches surgically created from intestinal segments. We report the first two cases of a polypoid lesion in these structures, both of which were composed of injected collagen. The first lesion occurred in the ileal urinary pouch of a 41-year-old paraplegic man who had cystoprostatectomy for severe spasm and repeated infection of the bladder. The pouch, removed for repeated infection, sho

[Diagnostic imaging of intestinal invaginations in the adult. Report of 9 cases].

Adult intestinal intussusception affects the distal portions of the small bowel and the colon in 90% of cases. As a rule, its nature is neoplastic, its clinical presentation aspecific and its diagnosis is frequently an occasional finding during routine imaging examinations. We report on 9 adult patients with intestinal intussusception. All patients were examined with more than one of the following imaging modalities: radiologic study of the small bowel, barium enema, ultrasonography (US), and Computed Tomography (CT). The first diagnostic suspicion of intussusception was correctly made at US i

Intestinal obstruction with oral pigmentation: a clue to Peutz-Jeghers syndrome.

Peutz-Jeghers syndrome (PJS) is an autosomal dominant disorder, which manifests as mucocutaneous hyperpigmentation and, intestinal and extraintestinal polyps. The classic triad of abdominal pain, mass and jam-like stools are not found commonly. On clinical examination, a strong suspicion of PJS should be made if patients below 18 years, present with mucocutaneous hyperpigmentation and intestinal obstruction. We report a case of a 16-year-old woman who presented to the surgical emergency with a 1-day history of acute pain in abdomen, in right iliac fossa and features of intestinal obstruction.

Hereditary hemorrhagic telangiectasia associated with multiple pulmonary arteriovenous malformations and juvenile polyposis.

A case with multiple telangiectases of the gastrointestinal tract that caused recurrent bleeding is reported. The hemorrhagic telangiectases were associated with multiple pulmonary arteriovenous malformations leading to significant right-to-left shunting and in combination with an atrial septal defect to progressive right heart failure and finally to the death of the patient. Juvenile polyps and tubulovillous adenomas of the colon were removed. Hemicolectomy had to be performed because of carcinomatous infiltration originating from adenomatous epithelium developing in a juvenile polyp. The pat

Colorectal polyposis as a clue to the diagnosis of Cowden syndrome: Report of two cases and literature review.

Cowden Syndrome (CS) is an autosomal dominant disorder characterized by hamartomatous growth in several organs and by an increased risk of malignancies, which makes its recognition essential to undertake risk reduction measures. Although the involvement of gastrointestinal tract is extremely common, awareness of this entity among gastroenterologists appears limited. We report on two unrelated patients: a 46-year-old male and a 38-year-old woman, who were referred to the Genetic Clinic because of the endoscopic finding of multiple colorectal polyps. Despite both displayed striking clinical (and

Huge Colonic Granuloma of Schistosomiasis Mimicked Cancer in a 10-Years-Old Child: A Case Report.

INTRODUCTION: Schistosomiasis is a trematodes infection more prevalent in tropical and subtropical areas such as Yemen. Giant colonic polyp manifestations of intestinal bilharziasis are uncommon, difficult to differentiate from other colonic polyps, and can mimic cancer. CASE REPORT: A 10-year-old child presented with chronic abdominal pain that started ten months ago. The patient had a family history of lymphoma in his brother. The computed tomography scan showed a sigmoid luminal mass measuring 10×3 cm with significant lumen narrowing and diffuse circumferential wall thickening of the sigmo

Pathological lead points in pediatric secondary intussusception: A single-center retrospective observational study of five common etiologies.

While most pediatric intussusceptions are idiopathic, secondary cases differ significantly in clinical presentation, treatment strategies, and prognosis due to the presence of pathological lead points (PLPs). Accurate PLP identification is crucial for optimizing diagnosis and treatment. This study aims to compare clinical and imaging features across PLP types to improve recognition and intervention efficiency for secondary intussusception. This study investigates common PLPs in pediatric secondary intussusception and compare their epidemiological characteristics, clinical manifestations, imagi

Tubular adenoma with squamous metaplasia of the sigmoid colon: a case report.

A case of tubular adenoma with squamous metaplasia of the sigmoid colon in a 44-year-old female is reported. The tumor was a pedunculated polyp with a size of 1.2 X 1.7 X 1.7 cm. Histologically, the tumor showed tubular adenoma with focal squamous metaplasia. Borderline atypia was noted in a part of adenoma without squamous metaplasia and squamous component. It is supposed that squamous epithelial polyp, squamous cell carcinoma, adenosquamous carcinoma, and mucoepidermoid carcinoma may be derived from colonic adenoma with squamous metaplasia. Only six cases of the adenoma with squamous metapla

Nodular lymphoid hyperplasia of the intestinal tract in infancy and childhood.

Over an 18-year period we have diagnosed nodular lymphoid polyposis of the intestinal tract in 6 patients. The site of the polyposis, which was due to prominent lymphoid hyperplasia, was ileal (3), colonic (2), and rectal (1). The diagnosis was made following complications arising from the polyps, which included recurrent intussusception (2), rectal prolapse (1), intestinal or pseudointestinal obstruction (2), and rectal bleeding (1). Immunoglobulin staining was performed on all the bowel specimens and in every case secretory IgA was present on the mucosal surfaces and IgG and IgA were seen in

Diagnosis of inflammatory and neoplastic colonic disease by sonography.

By instilling fluid into the large intestine, it is possible to demonstrate the entire colon starting at the recto-sigmoidal boundary and ending at the cecum. Aside from demonstrating the intestinal lumen, a high resolving transducer will also allow detailed evaluation of the intestinal wall and its surrounding tissue. Examination of 61 patients with colonoscopic-verified diagnoses showed that polyps as well as colon cancer can be diagnosed by colonic sonography. Aside from such localized changes, colonic sonography can also demonstrate typical changes of Crohn's disease as well as the extent
