We report a patient with intestinal pseudo-obstruction in which both the histopathological findings and the clinical history strongly suggest a visceral myopathy of familial type. Severe intestinal hypomotility in scleroderma is an uncommon but difficult management problem. We present a case of advanced scleroderma bowel complicated by colonic pseudo-obstruction, which was dramatically improved by subtotal colectomy. Congenital short bowel syndrome (CSBS) is an uncommon gastrointestinal disorder in which an unclear aetiology causes considerable intrauterine reduction in small bowel length. As a result of reduced absorptive intestinal length, chronic diarrhoea, vomiting, and consequently, failure to thrive are likely. We report a case of CSBS in a 26-day-old girl who had malrotation and a short bowel with a length of bowel from the pylorus to the ileocecal junction of approximately 40 cm. The patient underwent Ladd's procedure, but she is still dependent on parenteral nutrition.

A 32-year-old male patient with common variable immunodeficiency (CVI), under treatment with human immunoglobulin G, experienced recurrent episodes of intestinal pseudo-obstruction that were medically treated. Imaging tests showed a non-obstructive ileal stenosis and biopsies performed during ileocolonoscopy revealed nod-ular lymphoid hyperplasia.

Chronic intestinal pseudo-obstruction is a life-threatening condition of unknown pathogenic mechanisms. Chronic intestinal pseudo-obstruction can be a feature of mitochondrial disorders, such as mitochondrial neurogastrointestinal encephalomyopathy (MNGIE), a rare autosomal-recessive syndrome, resulting from mutations in the thymidine phosphorylase gene. Patients on ventilators for respiratory failure due to pulmonary disease may develop three major complications requiring general surgical evaluation: gastrointestinal (GI) bleeding, paralytic ileus, and mesenteric vascular insufficiency. GI bleeding from acute hemorrhagic gastritis (stress ulceration) can be avoided by aggressive medical therapy consisting of volume replacement, antacids, nutrition, and treatment of the precipitating causes.
