Narrative review of acute post-craniotomy pain. Concept and strategies for prevention and treatment of pain.

The aim of this narrative review is to confirm that acute pain after craniotomy is frequent and presents with moderate to severe intensity. We also highlight the importance of not only treating post-craniotomy pain, but also of preventing it in order to reduce the incidence of chronic pain. Physicians should be aware that conventional postoperative analgesics (non-steroidal anti-inflammatory, paracetamol, cyclooxygenase inhibitors 2, opioids) are not the only options available. Performing a scalp block prior to surgical incision or after surgery, the use of intraoperative dexmedetomidine, and

Incidental detection of disseminated peritoneal tuberculosis-varied presentation.

BACKGROUND: Incidental peritoneal tuberculosis represents an uncommon variety of peritoneal tuberculosis and surgeons must be aware of this entity particularly in tuberculosis endemic zones. METHODS: We prospectively analysed cases of incidental peritoneal tuberculosis detected during surgery over a period of last six months. RESULTS: We herein describe three such cases of incidental peritoneal TB detected during surgical exploration for other reasons. CONCLUSION: Diagnosis of disseminated peritoneal tuberculosis often remains a challenging task owing to its non specific clinical presentati

Preventing retrograde jejunoduodenogastric intussusception as a complication of a long-term indwelling gastrostomy tube.

Gastrostomy tubes are frequently used to provide enteral access in a variety of patient populations. Long-term complications are usually minor and include ulceration of the surrounding skin, clogging or dislodgment of the feeding tube, and superficial abscess; severe long-term complications are rare. Here we report a case of a life-threatening complication from an indwelling gastrostomy tube, specifically retrograde jejunoduodenogastric intussusception. Computed tomography and intraoperative images, as well as a review of literature, provide a detailed picture for diagnosis and treatment. Alth

Synchronous gallbladder and pancreatic cancer associated with pancreaticobiliary maljunction.

We report the case of a 46-year-old woman who presented with chronic intermittent abdominal pain without jaundice; abdominal ultrasonography showed thickening of the gallbladder wall and dilatation of the bile duct. Endoscopic retrograde cholangiopancreaticography showed pancreatobiliary maljunction with proximal common bile duct dilatation. Pancreatobiliary maljunction was diagnosed. A computed tomography scan of the abdomen showed suspected gallbladder cancer and distal common bile duct obstruction. A pancreatic head mass was incidentally found intraoperative. Radical cholecystectomy with pa

Perioperative pain management.

The clinical anesthesia, general surgery, and gynecology literature addressing the pathophysiology and management strategies for perioperative pain were reviewed. There are few prospective, randomized studies from which to draw meaningful conclusions. Nevertheless, a theoretical construct has been developed which may help the gynecologic surgeon optimizing pain management. The era of managed care and shorter hospital stays has focused physicians and, in particular, surgeons on elements of patient care that can be addressed and improved. Reducing or eliminating postoperative pain without excess

Treacherous Pitfalls in Liver Transplantation for Budd-Chiari Syndrome: When the Devil Hides in the Shadows-A Case Report.

INTRODUCTION: Budd-Chiari syndrome (BCS) is a rare vascular liver disorder characterized by obstruction of the hepatic venous outflow, involving the hepatic veins and/or the inferior vena cava (IVC). Etiology varies geographically, with thrombophilic conditions and myeloproliferative disorders predominating in Western countries, and membranous IVC obstruction being more frequent in Eastern countries. Liver transplantation (LT) remains the definitive treatment for patients with acute liver failure or advanced disease refractory to medical and interventional therapies, but it presents unique sur

Dose control in robot-assisted seed brachytherapy: challenges, current status, and future directions.

Seed brachytherapy treats a range of localized solid tumors, where efficacy and normal-tissue sparing depend on precise dose control. Advancing it from geometry-controlled execution toward autonomous, dose-controlled delivery is obstructed by the irreversibility of implantation: once released, a seed fixes its dosimetric contribution permanently. Errors arising from plan-to-anatomy mismatch, placement imprecision, and limited intraoperative assessment therefore become embedded in the realized dose distribution, beyond postoperative correction. Correction is available only during the procedure,

Breaking surgical barriers: ERAS in action in Romania.

Implementing Enhanced Recovery After Surgery (ERAS) protocols presents challenges for healthcare systems, particularly for patients undergoing complex surgeries. Though ERAS effectively reduces postoperative complications and hospital stays, its implementation varies. Our hospital adopted the ERAS protocol in 2020. This study details specific ERAS components implemented in our clinic, emphasizing surgical and anesthetic strategies. We describe preoperative, intraoperative, and postoperative phases and analyze the evidence for each component's integration. Additionally, we highlight the specifi

Factors Associated With Discordance Between Preoperative Parathyroid 4-Dimensional Computed Tomographic Scans and Intraoperative Findings During Parathyroidectomy.

IMPORTANCE: Parathyroid 4-dimensional computed tomographic scans (4D-CTs) have emerged as an accurate and cost-effective initial localization study for patients with primary hyperparathyroidism. However, potential limitations and factors affecting the accuracy of preoperative 4D-CTs remain poorly defined. OBJECTIVES: To characterize factors associated with missed parathyroid lesions on preoperative 4D-CTs and to investigate patterns of commonly observed errors. DESIGN, SETTING, AND PARTICIPANTS: A prospectively accrued patient database was analyzed from September 1, 2011, through October 31,

Paradoxical refractory hypotension following adrenaline administration in a patient taking clozapine.

Clozapine is a potent antipsychotic commonly used for refractory schizophrenia. Adverse effects are well recognised including constipation, intestinal obstruction, agranulocytosis and cardiomyopathy. We present a case of paradoxical refractory hypotension following epinephrine administration in a patient taking clozapine. A psychiatric inpatient who had been taking clozapine for many years developed paralytic ileus and obstruction requiring surgical intervention. Following initiation of epinephrine administration intraoperatively he developed refractory hypotension which improved only when epi

Laparoscopic management of a rare intrahepatic bile duct variation in a patient with cholelithiasis: a case report and review of the literature.

BACKGROUND: Anatomical variations of the biliary system constitute one of the principal risk factors for bile duct injury (BDI) during laparoscopic cholecystectomy (LC). Preoperative imaging modalities, such as abdominal ultrasonography and magnetic resonance cholangiopancreatography (MRCP), play a pivotal role in identifying potential biliary anatomical variations. In addition, meticulous adherence to standardized surgical techniques during LC serves as a critical safeguard against BDI. Therefore, heightened awareness and comprehensive understanding of biliary anatomical variations are essent

Transverse testicular ectopia presenting as an inguinal hernia in a 2-year-old child: Case report.

INTRODUCTION: Transverse testicular ectopia (TTE) is a rare congenital anomaly where both testes migrate into a single inguinal canal, often mimicking a routine inguinal hernia. This condition is frequently overlooked in low-middle-income countries (LMICs) due to diagnostic limitations, increasing risks of complications such as infertility or malignancy. CASE PRESENTATION: A 2-year-old male from an LMIC presented with a right inguinal swelling and an impalpable left testis. Clinical evaluation suggested a typical inguinal hernia, but suspicion for TTE arose due to contralateral cryptorchidism

Superficial Ulnar Artery Mimicking the Radial Artery During Radial Forearm Free Flap Harvest: A Surgical Pitfall and its Intraoperative Identification - A Case Report.

BACKGROUND: The superficial ulnar artery is a rare anatomical variant that can be misidentified as the radial artery during radial forearm free flap harvest, potentially leading to ischemic complications. Awareness of this anomaly is crucial for reconstructive surgeons to prevent inadvertent vascular injury. We present a case where intraoperative identification techniques helped distinguish this variant, ensuring a successful surgical outcome. CASE PRESENTATION: A 54-year-old male with squamous cell carcinoma involving the upper lip, buccal mucosa, left commissure, and lateral third of the lo

Hypercoagulability in End-stage Liver Disease: Review of Epidemiology, Etiology, and Management.

In this review, we analyze the epidemiology of thromboses related to end-stage liver disease (ESLD), discuss causes of hypercoagulability, describe susceptible populations, and critically evaluate proposed prophylaxis and treatment of thromboses. Classically, ESLD has been regarded as a model for coagulopathy, and patients were deemed to be at high risk for bleeding complications. Patients with ESLD are not auto-anticoagulated, and they do not have a lower risk of portal vein thrombosis, intracardiac thrombus formation, pulmonary embolism or hepatic artery thrombosis. Though the cause of hyper

Concomitant hyperparathyroidism and nonmedullary thyroid cancer, with a review of the literature.

PURPOSE: Primary hyperparathyroidism and concomitant thyroid cancer is a rare and complicated setting for diagnostic imaging. MATERIALS AND METHODS: The authors report the accidental finding of primary hyperparathyroidism in a patient with rapid enlargement of a thyroid nodule and the results of a literature review. RESULTS: Tl-201-Tc-99m subtraction scintigraphy correctly revealed the malignant nature of a large cold thyroid nodule and mediastinal parathyroid hyperplasia. In contrast, high-resolution ultrasound indicated a retrothyroidal hyperplastic parathyroid gland. Surgery followed the

Operative management of penile strangulation caused by a rigid metallic ring: a case report.

Penile strangulation is a time-critical emergency in which delayed intervention risks vascular compromise and tissue injury. Management is heterogenous, varying with device characteristics, duration of incarceration, and severity of effects. We report a man in his seventies with penile strangulation caused by a metallic ring at the distal shaft, immediately proximal to the glans. Bedside removal (lubrication, topical 50% dextrose gel, and standard ring-cutter use) were unsuccessful owing to device rigidity. Given persistent entrapment, urinary difficulty, and evolving ischaemic changes, urgent

Gall Bladder Perforation During Percutaneous Nephrolithotripsy: Lessons Learned.

Percutaneous nephrolithotripsy (PCNL) is the recommended procedure for patients with large and complex renal calculi. Visceral injuries are rare complications of this procedure. A distended gall bladder (GB) that abuts the right kidney is more prone to needle puncture-related injuries. A GB injury leading to peritonitis portends a poorer prognosis. We report a case of GB perforation that happened after an otherwise uneventful PCNL, which was managed with a prompt intraoperative decision of performing laparoscopic cholecystectomy. The initial puncture yielded a straw-colored, gelatinous aspirat

A Five Years Old Child with Failure To Thrive and Vomiting Presenting as a Diagnostic Dilemma: A Case Report.

Vomiting with failure to thrive in older children is a diagnostic challenge due to the diversity in the diagnosis. We report a case of a five-years-old girl with failure to thrive, history of recurrent vomiting and intermittent colicky pain abdomen since 45 days of life. Intestinal malrotation with Ladd's band was diagnosed based on clinical acumen, high- resolution computed tomography, barium follow through and intraoperative findings. Exploratory laparotomy with Ladd's procedure was performed under general anesthesia which showed malrotation at the duodenojejunal junction with a short route
