Iron in Hodgkin's lymphoma.

Anemia is a frequent finding of Hodgkin's lymphoma (HL) diagnosis. It is usually mild, with hemoglobin levels between 10 and 12 g/dl; it is rarely (<10% of cases) a result of bone marrow infiltration; and it displays the characteristics of the anemia of chronic disease due to abnormalities in iron metabolism. The inflammatory cytokine IL-6 is frequently up-regulated in Hodgkin's lymphoma, and IL-6 levels are strongly associated with hepcidin, the main regulator of iron metabolism. Elevated hepcidin levels result in iron restriction and signs of anemia of chronic disease. In addition, the abund…

Iron sufficiency of Canadians.

BACKGROUND: Iron deficiency is the most common nutritional deficiency in the world, but little is known about the iron status of people in Canada, where the last estimates are from 1970-1972. DATA AND METHODS: The data are from cycle 2 (2009 to 2011) of the Canadian Health Measures Survey, which collected blood samples from a nationally representative sample of Canadians aged 3 to 79. Descriptive statistics (percentages, arithmetic means, geometric means) were used to estimate hemoglobin and serum ferritin concentrations, and other markers of iron status. Analyses were performed by age/sex gr…

Iron requirements in infancy.

Iron deficiency anemia is the most common micronutrient deficiency worldwide and infants constitute a risk group due to their high iron requirements. Iron is critical for brain development, and case control studies have shown a consistent association between iron deficiency anemia in infancy and poor neurodevelopment, suggesting that it is important to prevent iron deficiency anemia in infants. However, it is also important to avoid excessive iron intakes which may have adverse effects on growth. Due to redistribution of iron from hemoglobin to iron stores, healthy, term, normal birth weight i…

Role of iron in osteoporosis.

Osteoporosis is a remarkably frequent complication of iron loading conditions such as thalassemia, sicklemia, African siderosis, hemochromatosis, smoking, alcoholism, HIV infection, and cessation of menstruation. The metal suppresses osteoblast formation of bone and may also stimulate osteoclast resorption of bone. Iron also inhibits anterior pituitary synthesis of gonadotrophs. This, in turn, results in depressed formation of gonadal hormones. The tendency of iron-loaded persons to become osteoporotic may be enhanced by gonadal hormone deficiency. Iron binding agents that could specifically w…

Iron and insulin resistance.

BACKGROUND: Preliminary clinical and experimental results suggest that iron can modify hepatocytes' insulin sensitivity by interfering with insulin receptor and intracellular insulin signalling. AIM: To evaluate in vivo the influence of iron on insulin resistance and insulin release in patients with non-alcoholic fatty liver disease and in vitro the interaction between iron and insulin sensitivity by analysing the effect of iron manipulation on insulin receptor expression in hepatoblastoma HepG2 cell line. RESULTS: Insulin resistance evaluated by homeostatis model assessment (HOMA)-insulin r…

Iron deficiency and the intellect.

Children are especially liable to iron-deficiency anaemia in developing countries, and in the inner cities of developed countries. Does the lack of iron cause impaired physical and mental development, and can this in certain circumstances be a permanent effect? One of the reasons that this is such a difficult question to answer is that there can be so many confounding factors, from other nutritional deficiencies, to helminthic infections and malaria in tropical countries. If there is a definite relationship, children in the first 2 years of life will be at particular risk during the mayor spur…

Role of iron in atherosclerosis.

The importance of iron in injury is derived from the ease with which iron is reversibly oxidized and reduced and thus able to participate in the generation of powerful oxidant species, such as hydroxyl radical, and in lipid peroxidation. There is compelling mechanistic evidence for the potential role of iron in atherosclerosis: the role of iron in oxidizing low-density lipoprotein (LDL), iron chelators prevent endothelial cell damage by oxidized LDL, the ability of iron to cause endothelial cell damage, and iron chelators prevent endothelial cell dysfunction and vascular smooth muscle prolifer…

Safety in iron management.

Intravenous (IV) iron therapy has become an integral part of hemodialysis management during the past several decades, and the National Kidney Foundation-Kidney Disease Outcomes Quality Initiative guidelines recognize that most patients undergoing hemodialysis will require IV iron therapy on a regular basis to reach target hemoglobin (Hgb) levels. There now are three IV iron compounds available in the United States: iron dextran, sodium ferric gluconate, and iron sucrose. Although all have been proven effective for increasing Hgb/hematocrit levels, recent data show differences in their relative…

Iron supplementation in pregnancy.

BACKGROUND: Anaemia in pregnancy is a major health problem in many developing countries where nutritional deficiency, malaria and other parasitic infections contribute to increased maternal and perinatal mortality and morbidity. OBJECTIVES: The objective of this review was to assess the effects of iron supplementation on haematological and biochemical parameters, and on pregnancy outcome. SEARCH STRATEGY: The Cochrane Pregnancy and Childbirth Group trials register was searched. Study authors were also contacted. SELECTION CRITERIA: Acceptably controlled trials of iron supplementation for pr…

[Epidemiology of iron deficiency].

During the last years, a large number of surveys have suggested than some fractions of the French population, as in other industrialized countries, presented dietary iron intake below the recommended dietary allowances. Iron deficiency is widespread in children, menstruating women, and particularly pregnant women. Iron deficiency is enough severe to be responsible of anemia in some groups of population. Aside the classical risk of anemia (with its well-known consequences on health), the effects of iron deficiency by itself, taking into consideration the role of iron in numerous metabolic funct…

Iron and liver diseases.

A mild to moderate iron excess is found in patients with liver diseases apparently unrelated to genetic hemochromatosis. Iron appears to affect the natural history of hepatitis C virus-related chronic liver diseases, alcoholic liver disease and nonalcoholic steatohepatitis by leading to a more severe fibrosis and thus aiding the evolution to cirrhosis. A higher frequency of mutations of the HFE gene, the gene responsible for hereditary hemochromatosis, is found in patients with liver diseases and increased liver iron than in normal patients. Patients with excess iron are potentially at a highe…

Iron therapy and cancer.

Anemia in cancer patients has many etiologies. Iron therapy clearly is indicated in patients whose anemias are associated with iron deficiency. However, a frequent cause of anemia in cancer is the "anemia of chronic disorders," in which, although functional iron may be low, tissue iron remains normal or high. Administration of iron with erythropoietin to such patients requires careful and frequent evaluation of hematologic and iron values. Inadvertent iron loading can contribute to deterioration of a variety of organ systems, as well as to increased proliferation of neoplastic cells.

Pathophysiology of iron toxicity.

There are several inherited and acquired disorders that can result in chronic iron overload in humans, and the major clinical consequences are hepatic fibrosis, cirrhosis, hepatocellular cancer, cardiac disease, and diabetes. It is clear that lipid peroxidation occurs in experimental iron overload if sufficiently high levels of iron within hepatocytes are achieved. Lipid peroxidation is associated with hepatic mitochondrial and microsomal dysfunction in experimental iron overload, and lipid peroxidation may underlie the increased lysosomal fragility that has been detected in liver samples from…

Iron status of vegetarians.

An appropriately planned well-balanced vegetarian diet is compatible with an adequate iron status. Although the iron stores of vegetarians may be reduced, the incidence of iron-deficiency anemia in vegetarians is not significantly different from that in omnivores. Restrictive vegetarian diets (eg, macrobiotic) are associated with more widespread iron-deficiency anemia. Western vegetarians who consume a variety of foods have a better iron status than do those in developing countries who consume a limited diet based on unleavened, unrefined cereals. Whereas phytates, polyphenolics, and other pla…

The neuropsychiatry of brain iron.

In the last two decades, many biological functions of iron have been identified, in particular its role in many enzymatic processes, its effect on dopamine D2 receptor function, its interaction with other neurotransmitters (gamma-aminobutyric acid, serotonin, opiate-peptides), and its catalytic role in the nonenzymatic mechanisms for oxidation, hydroxylation, and peroxidation reactions. The role of iron in Parkinson's disease, Alzheimer's disease, brain injury due to exogenous causes, neuroleptic-induced movement disorders, schizophrenia, and other neuropsychiatric disorders is currently being…
