Fluid therapy in the PACU.

The goal of fluid therapy in the PACU setting is the restoration of blood volume and tissue perfusion. Choosing the type of fluid infusion depends on the preoperative, intraoperative, and postoperative condition of the patient. An understanding of the functional fluid compartments, the composition of body fluids and commercially available fluids, and the steps to evaluate fluid depletion allow one to determine the fluid needs of the patient. The orderly and expedient evaluation of fluid status of the postoperative patient involves the assessment of volume status, concentration status, composit

Influences of different resuscitation regimens on acute early weight gain in extensively burned patients.

Dissatisfaction with the massive weight gain that commonly followed crystalloid resuscitation of extensively burned patients dictated the need for a study to determine if acute weight gain could be minimized with an alternative form of resuscitation. Three groups of ten patients each with statistically similar age and burn size (mean BSA 46 per cent) were resuscitated with lactated Ringer's solution (LR), hypertonic saline solution (HPT), or fresh frozen plasma (FFP). The volume of infused fluid and the patient weight gain were measured over the first 48 h of treatment. The mean urine output o

Danger of central pontine myelinolysis in hypotonic dehydration and recommendation for treatment.

Isotonic saline is the fluid most commonly used for treatment of asymptomatic hypotonic dehydration, but as shown in the case presented in this article, rapid increase in serum sodium may follow administration of isotonic saline, leading to the development of central pontine myelinolysis (CPM). Because the necessity of rapid correction is less, whereas the risk of CPM is greater with chronic asymptomatic hyponatremia than with acute hyponatremia, use of a half normal saline might be more appropriate than normal saline for treating certain patients with hypotonic dehydration with asymptomatic h

Effect of crystalloid infusion on hematocrit and intravascular volume in healthy, nonbleeding subjects.

We evaluated the effect of IV crystalloid administration on hematocrit in 28 healthy, nonbleeding volunteers. Normal saline boluses of 10, 20, and 30 mL/kg were delivered at a mean rate of 115 +/- 4 mL/min, followed by a continuous infusion of either 1 or 5 mL/kg/hr. Hematocrit values determined immediately after bolus infusion declined from baseline a mean +/- SEM of 4.5 +/- 0.6 (P less than .001), 6.1 +/- 0.4 (P less than .001) and 6.3 +/- 0.6 points (P less than .001) in the 10, 20, and 30 mL/kg groups, respectively. Twenty minutes into the maintenance infusion, hematocrits had risen 1.5 +/

Incompatibility of remimazolam besylate with Ringer's acetate infusion resulting in total occlusion of an intravenous catheter.

Remimazolam besylate is an ultra-short-acting benzodiazepine derivative recently approved in Japan for general anaesthesia. However, less attention has been paid to the compatibility of remimazolam with infusion solutions, and the mechanism underlying the incompatibility remains unknown. The patient was a 65-year-old man who underwent a high tibial osteotomy. After the induction of general anaesthesia using remimazolam solution (5 mg/mL), we noticed precipitate completely blocking the lumen of the intravenous tube connected to a Ringer's acetate Physio140 drip. The mixture of remimazolam solut

Spontaneous clostridial myonecrosis.

Spontaneous clostridial myonecrosis (SCM) is an uncommon but frequently fatal tissue infection. The case report and review of the literature here provide some insight into the pathophysiology of this entity and clinical settings with which it is associated. It is hoped that this discussion will increase the reader's familiarity with SCM and provide clues that will lead to earlier diagnosis and more effective treatment.

Objective approach to the management of shock.

The objective in managing shock is to prevent anaerobic metabolism in the tissue. In the resuscitation of the acutely hypovolemic patient, volume resuscitation is the key. Drugs are seldom needed to accomplish this goal. Only when shock state results from an increase in the size of the container or from a decrease in the output of the pump, are drugs necessary. The objective management of shock is an understanding of the interrelationship of the pump, the container and the fluid, the mechanisms of monitoring their functions, and the use of the appropriate methods to return them to normal.

[Effects of equiosmolar load of 20% mannitol, 7.5% saline and 0.9% saline on plasma osmolarity, haemodynamics and plasma concentrations of electrolytes].

OBJECTIVES: Mannitol 20% (M) and NaCl 7.5% (H) are hypertonic solutions used in-patient in hypovolaemic shock and in case of increased ICP. To our knowledge no study in humans compared them in an equiosmolar load. The objectives of this study were to compare the pharmacokinetics of both solutions in an equiosmolar load with an isotonic solution (NaCl 0.9% (C)) in terms of osmolarity, electrolytes and haemodynamic parameters over time. STUDY DESIGN: Prospective, randomized, controlled study. PATIENTS: Thirty ASA I and II patients undergoing non haemorrhagic surgery under general anesthesia.

Plasma renin activity following central infusion of angiotensin II and altered CSF sodium concentration in the conscious goat.

To study central influences on the renal release of renin, angiotensin II was infused into the lateral cerebral ventricle of conscious hydrated goats. CSF sodium concentrations was increased or lowered by similar infusions of hypertonic NaCl or of isotonic fructose solution. Infusion of angiotensin II in doses from 0.5 to 1 mug caused a drop in plasma renin activity (PRA) and elicited a rise in blood pressure, antidiuresis, natriuresis, and thirst. Intraventricular infusion of hypertonic NaCl also supressed PRA, induced antidiuresis, natriuresis, and an inconsistent rise in blood pressure. Low

Loop diuretics inhibit detubulation and vacuolation in amphibian muscle fibres exposed to osmotic shock.

The effect of loop diuretics at concentrations known to influence cellular water entry coupled to Na-K-Cl co-transport, upon the vacuolation and detubulation following osmotic shock, was investigated in amphibian skeletal muscles. These were exposed to a glycerol-Ringer solution (18 min), an isotonic Ca2+/Mg2+ Ringer solution and cooling. Adding bumetanide (1.0 and 2.0 microM) to these solutions sharply reduced the incidence of detubulation, assessed by abolition or otherwise of action potential after-depolarisations, from 93.9 +/- 4.7% (n = 6) to 5.0 +/- 1.1% (n = 4: mean +/- SEM: 2.0 microM

Cerebral mechanisms influencing renal sodium excretion in dehydrated sheep.

Reducing the cerebrospinal fluid concentration of NaCl by infusion into a lateral cerebral ventricle of isotonic solutions of 0.3 mol/l mannitol or sucrose at 1 ml/h for 2 h caused a large reduction in renal Na excretion in conscious sheep deprived of water for 24 or 48 h. Infusion of 0.3 mol/l mannitol into the lateral ventricle of water-replete sheep did not alter renal Na excretion but induced a water diuresis. These data indicate that during dehydration, renal Na excretion may be under the control of the brain. The pathway(s) from brain to kidney mediating this antinatriuretic effect are n…

Pulmonary oedema associated with acidosis in patients with cholera.

Five patients with severe acidosis and pulmonary oedema complicating cholera were seen at the Cholera Research Laboratory, Dacca, in a two-year period. All had had inadequate treatment. Their disease resulted in acidosis prior to admission; only the two who subsequently survived received volumes of sodium bicarbonate solutions sufficiently large to repair completely their acidosis. Saline alone worsened pulmonary congestion, while alkali appeared to relieve it despite the accompanying volume expansion. These observations are consistent with the known redistribution of blood to the central circ

Randomized trial of normal saline versus 5% albumin for the treatment of neonatal hypotension.

OBJECTIVES: This study was designed to assess the comparative efficacy of normal saline (NS) and 5% albumin (ALB) for treatment of hypotension in the acutely ill newborn. STUDY DESIGN: Newborn infants who were < 24 hours old and were admitted to the Holden Neonatal Intensive Care Unit at the University of Michigan were randomized to receive one of the two solutions for volume expansion. Hypotension was defined as a sustained (> or =30 minutes) mean arterial pressure (MAP) of < 30 mmHg for infants weighing < or =2500 g, or a MAP of < 40 mmHg for those weighing > 2500 g. The short-term outcome
