Management and long-term comorbidities of patients with necrotizing otitis externa.

PURPOSE: We aimed to present the management of the patients with necrotizing otitis externa (NOE) and its comorbidities in early and long-term follow-up. METHODS: Between 2011 and 2022, 30 patients with the diagnose of NEO, who had cortical bone erosion or trabecular demineralization in temporal bone computed tomography and administered at least 6-week antimicrobial therapy were included in the study. Clinical, laboratory and imaging findings of patients, and comorbidities during follow-up were analysed. NOE extending further from the petro-occipital fissure on magnetic resonance imaging was

Norrie Disease: Cochlear Enhancement and Cerebellar Signal Abnormalities.

Norrie disease is a rare X-linked condition characterized by early childhood blindness and later onset sensorineural hearing loss. We report two male infants with genetically confirmed Norrie disease and characteristic ocular abnormalities consisting of bilateral funnel retinal detachments, anterior segment dysgenesis, and/or buphthalmos and microphthalmia. MRI demonstrated enhancement of the cranial nerves, cochleae, and cerebellum with cerebellar restricted diffusion. Intracranial findings mimicked meningitis, labyrinthitis, and cerebellitis. Neither infant showed clinical signs of infection

Experimental Sendai virus-induced labyrinthitis in guinea pigs. An ultrastructural study of cochlear lesions.

Guinea pigs were inoculated with a Sendai virus into the scala tympani and subsequent pathological changes of the cochleas were investigated by electron microscopy. Replication of the virus was indicated by buddings at the endolymphatic surface and by the intracytoplasmic occurrence of filamentous substances, ie, nucleocapsids. Budding viruses or free virus particles observed in a series of our experiments were identified as Sendai viruses by means of the immunological labeling with ferritin. Viral lesions, which were defined as the pathological changes of the cells associated with virus multi

[Middle ear cholesteatoma].

Cholesteatoma is a serious form of chronic otitis media. The aim of this paper is to present the state of the art of disease management, including recent data from the literature and the authors' derived from their Mentors' teaching, Professor Pierre Roulleau (Paris, France) and Professor Robert Charachon (Grenoble, France). The main recent advances concern the use of cartilage grafts to reconstruct the canal wall and/or tympanic membrane (allowing a significant reduction in residual cholesteatoma) and progress in medical imaging allowing more acute preoperative determination of extension of t

Pathogenesis of labyrinthitis associated with Haemophilus influenzae type b meningitis in infant rats.

The pathogenesis of labyrinthitis associated with bacterial meningitis was studied by histopathologic examination of inner ears of 114 rats with Haemophilus influenzae type b meningitis produced by ip inoculation of one of 13 clinical isolates. Findings consisted of inflammation of the perilymphatic spaces of the cochlea and semicircular canals with sparing of the endolymphatic space, cochlear nerve fibers, and middle ear. The degree of inflammation peaked at 48 hr after inoculation, then declined by 96 to 144 hr. No interstrain differences in type or degree of pathology were observed. Immunof

Harmonic acceleration tests as a measure of vestibular compensation.

The material presented in excerpted from patient evaluations. The study was performed with the use of the Contraves rotary chair. This testing modality was used at each patient visit. All other conventional auditory and ENG tests were also performed. These patients have been selected to illustrate the value of the harmonic acceleration (HA) test as a modality for monitoring patient progress. The HA graphs produced were a result of the presence or absence of central vestibular compensation. The findings on these investigations further confirm that HA test provide an objective monitor for vestib

Vestibular dysfunction associated with benign paroxysmal vertigo.

Benign paroxysmal vertigo (BPV) is a clinical syndrome of vestibular origin although generally no evidence of vestibular dysfunction can be demonstrated with conventional tests. In a review of 1350 consecutive dizzy patients, there were 125 with BPV and of these, 33 underwent a quantitative rotational test of vestibular function. The rotational results showed reduces vestibular system gain for these BPV patients. In addition, they could be subdivided on the basis of a normal or shorter cupular time constant (Tc). Separation of patients into diagnostic categories revealed that those categorized

[Evaluation of cochlear and vestibular functions of patients treated by dibekacin in an E.N.T. unit].

Cochlear and vestibular ototoxicity of dibekacin was evaluated in 27 patients treated with aminoglycoside after surgery on head and neck cancer. The treatment was administered intramuscularly or intravenously during 13 days at the schedule of 3 mg/kg per day. Cochlear and vestibular functions were tested by audiometry and electronystagmography before the administration of the antibiotic and on the 14th, 30th day, and for certain patients on the 4th month after the start of the treatment. No cochlear ototoxicity was found after the dibekacin treatment. On the other hand, the dibekacin treatment

Acute labyrinthitis: a possible association with influenza.

Acute labyrinthitis occurs annually in one in 600 patients, mostly adults in our practice, and lasts for up to two weeks. It may be related to several different viral infections, the commonest of which is influenza A. In an influenza epidemic there was an incidence of the syndrome in one in 100 cases. In the majority of cases there was no demonstrable cause and the aetiology remains speculative. It is suggested that some cases may be associated with viruses as yet unidentified.

Imaging of Meniere's disease.

The role of CT and MR studies in the evaluation of patients with Meniere's disease is to exclude other pathologic conditions that could masquerade as Meniere's disease clinically. This article is divided into three sections corresponding to the three major symptoms of Meniere's disease. Each section presents examples of CT and MR pathologic conditions that could mimic these clinical findings: "sensorineural hearing loss" covers acoustic neuromas and other cerebellopontine angle tumors, inner ear anomalies, otosclerosis, and more; "vertigo" includes examples of labyrinthitis, endolymphatic sac
