Video versus direct laryngoscopy for urgent tracheal intubation in neonates: a systematic review and meta-analysis.

INTRODUCTION: Intubation is most often performed electively by anaesthetists in controlled conditions in operating theatres. In neonates, however, it is most often performed by neonatologists or paediatricians in urgent circumstances in the neonatal intensive care unit (NICU) or delivery room (DR). Neonatal intubation is a difficult skill to learn and maintain, and success rates are suboptimal both in the NICU and DR. Video laryngoscopy (VL) has the potential to increase intubation success and safety as it may offer a better view of the airway, which can be shared by the intubator and other cl…

Office-Based Treatment of Vocal Fold Polyps and Reinke's Edema: A Rational Comparison With Suspension Laryngoscopy.

OBJECTIVE: Benign laryngeal lesions have traditionally been treated through suspension laryngoscopy under general anesthesia (GA). Recently, the development of operative videoendoscopes coupled with photoangiolytic lasers has allowed clinicians to treat these conditions in the outpatient clinic. We report our experience in the office-based (OB) setting for the treatment of patients affected by vocal fold polyps (VFPs) and Reinke's edema (RE), comparing it to patients treated under GA. METHODS: A retrospective analysis was conducted on patients affected by VFP or RE. A 445 nm diode blue laser …

Combined Use of a Gum Elastic Bougie and Video Laryngoscopy for Intubating a Patient With an Unexpected Laryngeal Papilloma.

This is a case report of a 75-year-old man scheduled for apical resection and cystectomy of odontogenic cysts involving both maxillary central incisors who presented with a previously unknown laryngeal mass that was discovered prior to intubation. Following induction and easy mask ventilation, direct laryngoscopy revealed a large mass on the right side of the glottis that impeded passage of a standard oral endotracheal tube. Successful atraumatic intubation was performed with the combination of a video laryngoscope (King Vision, Ambu Inc, Ballerup, Denmark) and a gum elastic bougie (GEB). Alth…

Enhanced visualization of the surgical field in pediatric direct laryngoscopy using a three-dimensional endoscopic system.

BACKGROUND: Direct laryngoscopy and rigid bronchoscopy are currently performed using 2-dimensional endoscopic systems. Our objective was to determine whether a 3-dimensional endoscopic system can enhance visualization of the surgical field in pediatric direct laryngoscopy and rigid bronchoscopy. METHODS: A prospective cohort study was conducted. Thirty three children who underwent direct laryngoscopies in a tertiary referral children's hospital were enrolled. Direct laryngoscopy was performed using both 2- and 3-dimensional endoscopic systems, after which the surgeons scored the quality of th…

Features of lateral cephalograms associated with difficult laryngoscopy in Japanese children undergoing oral and maxillofacial surgery.

BACKGROUND: Difficult laryngoscopy and tracheal intubation are occasionally encountered in children with congenital anomalies or micrognathia. However, no study has elucidated anatomical etiology in relation to craniofacial development. METHODS: Two hundred ten patients aged 8 months-18 years were analyzed. We analyzed the lateral cephalograms of: (i) eight patients in whom laryngoscopy was anticipated as difficult before anesthesia and who were unable to be intubated by direct laryngoscopy and needed fiberoptic bronchoscopy (group A); (ii) 11 patients in whom laryngoscopy was anticipated as …

Dental avulsion due to direct laryngoscopy during the induction of general anaesthesia and avulsed teeth in nasopharynx.

General anaesthesia was induced to a 32-year-old female patient. During direct laryngoscopy, the four upper front incisors were avulsed and fell into the patient's oral cavity without fracture. After endotracheal intubation, her oral cavity was searched laryngoscopically but the teeth were not found. Radiological findings of her chest and abdomen obtained by the C-armed x-ray device and endoscopic findings of the oesophagus were normal. Her head and neck imaging revealed a radiopaque lesion in the nasopharyngeal area. Later, the nasopharyngeal area was examined endoscopically and the teeth wer…

Review article: video-laryngoscopy: another tool for difficult intubation or a new paradigm in airway management?

BACKGROUND: An adequate airway management plan is essential for patient safety. Recently, new tools have been developed as alternatives to direct laryngoscopy and intubation. Among these, video-laryngoscopy has enjoyed a rapid increase in popularity and is now considered by many as the first-line technique in airway management. This paradigm shift may have an impact on patient safety. PRINCIPAL FINDINGS: Studies show that video-laryngoscopes are associated with better glottic visualization, a higher success rate for difficult airways, and a faster learning curve, resulting in a higher success…

Improving bedside airway tests accuracy for predicting difficult laryngoscopy using ultrasound-measured skin-to-epiglottis distance.

BACKGROUND: This study aimed to assess the accuracy of a composite score combining ultrasound-measured skin-to-epiglottis distance (DSE), Upper Lip Bite Test (ULBT), and Modified Mallampati Test (MMT) in predicting difficult airway. METHODS: In this prospective observational study, 250 adults scheduled for elective surgery were assessed preoperatively using ULBT, MMT, and ultrasound measurement of DSE. A composite airway score was calculated by assigning one point for each of the following: DSE > 2.0 cm, ULBT class > I, and MMT grade > II. Difficult laryngoscopy was defined as Cormack-Lehane …

Could mandibular profile angle or inter-pterygoid distance provide a novel method for predicting difficult laryngoscopy?

BACKGROUND: Various tests have been developed to predict difficult airway during preoperative examination, but none has presented an adequate degree of accuracy. This study investigates novel mandibular structures and angles for evaluation as potential predictor of difficult laryngoscopy. METHODS: Following Ethics Committee approval, 1001 patients scheduled for elective surgeries under general anesthesia with oral intubation were included in this prospective study. Difficult laryngoscopy was defined as a Cormack-Lehane grade III or IV view. Standard predictive tests - including neck circumfer…

Assessment of prehospital tracheal intubation technique using initial direct laryngoscopy during videolaryngoscopy: randomized controlled simulated trial.

BACKGROUND: In critically ill patients, tracheal intubation may be required in the prehospital setting, where airway management presents unique technical and logistical challenges. While videolaryngoscopy has emerged as a potential alternative to direct laryngoscopy by providing a better and easier visualization of the glottis, the improved view of anatomical structures does not necessarily correlate with successful tracheal tube placement. Intubation may be harder because novice providers performing videolaryngoscopy may only look at the screen and only obtain a two-dimensional representation…

First-Pass Success of Intubations Using Video Versus Direct Laryngoscopy in Children With Limited Neck Mobility.

OBJECTIVE: It is not clear whether video laryngoscopy (VL) is associated with a higher first-pass success rate in pediatric patients with limited neck mobility when compared with direct laryngoscopy (DL). We sought to determine the association between the laryngoscopy method and first-pass success. METHODS: In this retrospective cohort study, we examined intubation data extracted from 2 prospectively collected, multicenter, airway management safety databases (National Emergency Airway Registry and the National Emergency Airway Registry for children), obtained during the years 2013-2018 in the…

Cervical spine motion during tracheal intubation with manual in-line stabilization: direct laryngoscopy versus GlideScope videolaryngoscopy.

BACKGROUND: The optimal tracheal intubation technique for patients with potential cervical (C) spine injury remains controversial. Using continuous cinefluoroscopy, we conducted a prospective study comparing C-spine movement during intubation using direct laryngoscopy (DL) or GlideScope videolaryngoscopy (GVL), with uninterrupted manual in-line stabilization of the head by an assistant. METHODS: Twenty patients without C-spine pathology were studied. After induction of general anesthesia with neuromuscular blockade, both DL and GVL were performed on every patient in random order. Cinefluorosc…

Effect of cricoid pressure on tracheal intubation time during C-MAC video laryngoscopy: A Randomized Controlled Trial.

BACKGROUND: Cricoid pressure is routinely used during rapid sequence induction; however, its impact on tracheal intubation when using either direct or video laryngoscopy has not been clearly established. Studies suggest that the use of cricoid pressure may prolong intubation time and increase the difficulty of intubation. We conducted a randomized controlled trial to assess the effect of cricoid pressure on tracheal intubation during C-MAC video laryngoscopy in adult patients scheduled for elective surgery. METHODS: A total of 100 adult patients scheduled for elective surgery under general an…

Video analysis of neonatal intubations using video laryngoscopy: a prospective comparison of clinical practice with resuscitation guidelines.

BACKGROUND: The Neonatal Resuscitation Program recommends direct laryngoscopy (DL) as the primary method for neonatal intubation. Video laryngoscopy (VL) is suggested as an option, particularly for training novice operators or for intubating infants with difficult airways. The programme outlines specific steps for intubation, including managing the external environment and techniques for visualising key anatomical landmarks. It is unclear whether the DL method can be effectively applied to VL. OBJECTIVES: To determine the degree of adherence to resuscitation guidelines during intubation using…
