INTRODUCTION: White lentiginosis is characterized by white guttate macules with a typical histological structure: lentiginosis hyperplasia with hypopigmentation.
OBSERVATION: A 23-year-old woman, from Algeria, presented with white macules from 5 years ago. Few women in her family had the same trouble. Cutaneous examination revealed diffused white little macules. Histological examination showed a lentiginous hyperplasia of the epidermis, with elongated club-shaped rete ridges and an unusual loss of pigmentation.
DISCUSSION: Different diagnosis should be discussed with disseminated leucoderma

Several types of maculopapular melanocytic naevi can occur in a multiple form, and be arranged in a nonrandom fashion on the skin. Lentigines, synonymous but not identical to freckles or ephelides, are common skin lesions. In a small number of patients, however, these lesions constitute part of genetic syndromes that are associated with inherited forms of neoplasias or other pathologic processes of the cardiovascular, endocrine, and gastrointestinal systems. Multiple pigmented lesions including benign lentigines, typical nevi, and dysplastic nevi from 3 patients with the dysplastic nevus syndrome contained giant melanosomes (GM). Ultrastructurally the GM were composed of microvesicles, but lacked limiting membrane and filamentous internal structure. As such, they resembled the GM found in 4 other patients with dysplastic nevus syndrome, leopard syndrome and ocular albinism, but differed from those found in the café-au-lait spots of neurofibromatosis, nevus spilus, vitiliginous achromia, and certain animals.

A case-control study of 232 cases of cutaneous malignant melanoma and 232 matched controls was performed to assess the association of the disease with nonmelanotic skin tumors--basal cell carcinomas, squamous cell carcinomas, and solar keratoses.
