Hydroxychloroquine-induced renal phospholipidosis manifesting as proximal tubulopathy in systemic lupus erythematosus.

BACKGROUND: Hydroxychloroquine (HCQ)-induced renal phospholipidosis typically manifests as glomerular "zebra bodies", "myeloid bodies", and "curvilinear bodies" and is generally considered a benign histological mimic of Fabry disease. We report a case of HCQ-induced renal phospholipidosis with proximal tubulopathy presenting as slowly progressive kidney dysfunction and Fanconi syndrome, challenging the notion that renal phospholipidosis is clinically silent. CASE PRESENTATION: A 36-year-old woman with systemic lupus erythematosus (SLE) treated with HCQ for 18 months presented with slowly prog

Mass spectrometric analysis of the amiodarone-induced alteration of phosphatidylcholines in living single cells.

Amiodarone is one of the cationic amphiphilic drugs which cause the drug-induced phospholipidosis (DIPL). With the accumulation of phospholipids, the multiple-membrane structures called lamellar bodies emerge in the cell. Since the pathogenesis mechanism has not been clarified yet, DIPL is still concerned in the clinical use of amiodarone and the new drug development processes. In this study, we have focused on phosphatidylcholines (PCs) which are the representative phospholipids and have developed a single-cell analytical method for the determination of PCs accumulated in the lamellar bodies.

Hydrophilic interaction chromatography with a focus on the drug-phosphate interaction in drug screening to determine the phospholipidosis induction risk.

Cationic amphiphilic drugs (CADs) can induce the hyperaccumulation of phospholipids in cells and tissues. This side effect, which is known as drug-induced phospholipidosis, is sometimes problematic in the development and clinical use of CADs. It is known that CADs generally interact with phospholipids via both hydrophobic and acid-base interactions, and CADs with the larger affinity to phospholipid exhibit the larger induction risk. To develop a chromatographic assay system to predict the phospholipidosis-inducing potential with considering the acid-base interaction between CAD and phosphate g

Drug-induced phospholipidosis caused by combinations of common drugs in vitro.

Drug-induced phospholipidosis (DIPLD), characterized by the accumulation of phospholipids within lysosomes, is suspected to impair lysosomal function and considered an adverse side effect of the administered medication. The increasing use of polypharmacy and the resultant elevated risks of adverse drug reactions raise the need to explore the effects of drug combinations with respect to their influence on side effects, such as DIPLD. In this study, we utilized an in vitro assay to investigate DIPLD that was caused by 24 commonly used drugs applied alone and in binary combinations with each othe

An unusual case of aortic valve obstruction.

During a dissection class for anatomy, a white lipoid mass was found in the ascending aorta, which was partly attached to the wall and filled the sinuses ofValsalva and almost fitting as a cast. This mass prevented full opening of the mobile aortic valve leaflets, thereby causing an obstruction. Microscopic analysis revealed fibres and presence of polymorphonuclear white blood cells. It seems reasonable to assume that this mass has formed in the last weeks or months of the life of this subject, which is much quicker than for calcified aortic valve stenosis. Therefore, signs and symptoms of aor

Phospholipidosis as a function of basicity, lipophilicity, and volume of distribution of compounds.

Drug-induced phospholipidosis (PLD) is an adaptive histologic alteration that is seen with various marketed drugs and often encountered during drug development. Various in silico and in vitro cell-based methods have been developed to predict the PLD-inducing potential of compounds. These methods rely on the inherent physicochemical properties of the molecule and, as such, tend to overpredict compounds as PLD inducers. Recognizing that the distribution of compounds into tissues or tissue accumulation is likely a key factor in PLD induction, in addition to key physicochemical properties, we deve

[Degenerative neurological diseases of the central nervous system related to genetic neurolipidoses].

Genetic neurometabolic diseases in childhood are multisystemic. Surprisingly, these genetic diseases can manifest for the first time during adolescence and adulthood. In this case, the clinical presentation and evolutivity are very different. In childhood, many neurological systems are touched and their evolution is rapidly lethal. In the adult, their presentation may be that of a degenerative disease of the central nervous system and, according to the disease, the syndrome is very particular and very systematized. From our clinical and biological experience, we would like to suggest a decisio

Pulmonary alveolar lipoproteinosis complicating juvenile dermatomyositis.

A 9 year old boy with juvenile dermatomyositis developed pulmonary alveolar lipoproteinosis. This previously unreported association was treated successfully by bronchoalveolar lavage.

Case report: Erdheim-Chester disease: polyostotic sclerosing histiocytosis.

Erdheim-Chester disease is characterised by diffuse metaphyseal and diaphyseal sclerosis of long tubular bones of the appendicular skeleton. In addition, there is an inconstant tendency to visceral involvement by lipo-granulomatous infiltration. Histologically, this condition has marked similarities to Hand-Schuller-Christian disease. A further case is presented which supports the view that Erdheim-Chester disease is part of the spectrum of histiocytoses and not a distinct pathological entity. Polyostotic sclerosing histiocytosis would be an appropriate alternative label for this condition.

Clinico-pathological changes induced in rats treated with amine-curing agent for epoxy resin, bis(4-amino-3-methylcyclohexyl)methane.

Amine-curing agent for epoxy resin, bis(4-amino-3-methyl-cyclohexyl)methane (commercial name; Laromin C) has been suspected to have induced in the workers some toxic signs such as collagen disease like scleroderma or polymyositis. Subacute toxicity of this agent was studied in rats following repeated oral administration. The agent was given orally at 5 dose levels (25 mg/kg to 100 mg/kg per one dose) for periods ranging from 10 days to 4 weeks. After the completion of administration, clinico-biochemical tests and histopathological examinations were carried out. In a few cases, skeletal muscles

Gentamicin or chlorphentermine induction of phospholipidosis in the developing organism: role of tissue and species in manifestation of toxicity.

Daily, s.c. injection of gentamicin (100 mg/kg) for 2 days produced a significant increase in total phospholipid content of newborn rat kidney. Separation of individual phospholipid components revealed a significant rise in renal phosphatidylserine, phosphatidylinositol and phosphatidylcholine, whereas no marked change was noted in sphingomyelin, phosphatidylethanolamine or phosphatidylglycerol. Gentamicin did not significantly alter individual phospholipid classes and total phospholipid content in newborn rat liver and lung. Daily, oral chlorphentermine (60 mg/kg) administration also elevated
