Polypoid endobronchial extension from invasive thymoma.

A case of invasive thymoma which was manifest clinically as bronchial obstruction and metastatic liver tumour is presented. Autopsy revealed a unique polypoid endobronchial extension of the neoplasm. This represents a previously unrecognized pattern of secondary bronchial involvement by invasive thymoma.

Multimodality imaging approach in identifying invasive hepatocellular carcinoma.

Hepatocellular carcinoma (HCC) is the 5th most common neoplasm and the 3rd leading cause of cancer related mortality worldwide. Early stages of the neoplasm may be treated curatively with liver resection or orthotopic liver transplant. However, HCC has a high propensity for vascular and locoregional invasion, which can preclude these treatment options. The portal vein is the most invaded structure, while other regional structures affected include the hepatic vein, inferior vena cava, gallbladder, peritoneum, diaphragm, and the gastrointestinal tract. Management of invasive and advanced stages

Obesity and risk for liver disease: a two-sample Mendelian randomisation study.

The associations between obesity and liver diseases are complex and diverse. To explore the causal relationships between obesity and liver diseases, we applied two-sample Mendelian randomisation (MR) and multivariable MR analysis. The data of exposures (BMI and WHRadjBMI) and outcomes (liver diseases and liver function biomarker) were obtained from the open genome-wide association study database. A two-sample MR study revealed that the genetically predicted BMI and WHRadjBMI were associated with non-alcoholic fatty liver disease, liver fibrosis and autoimmune hepatitis. Obesity was not associa

Novel insights into the development of portal vein thrombosis in cirrhosis patients.

The prognostic impact of portal vein thrombosis (PVT) in liver cirrhosis remains controversial among studies, primarily because the risk stratification of PVT is often lacking. A definition of clinically significant PVT should be proposed and actively improved. Moreover, the risk factors for the development of PVT in liver cirrhosis should be fully recognized to screen and identify high-risk patients. Currently, well-recognized risk factors include a reduced portal vein flow velocity, a worse liver function, splenectomy, liver transplantation, and factor V Leiden and prothrombin G20210A mutati

Carcinofetal alterations in glycogen phosphorylase isozymes in rat hepatomas.

As part of a continuing study on the loss or retention of enzymes involved in specific hepatic functions in a series of rat liver neoplasms ranging widely in growth rate and degree of differentiation, isozyme patterns of glycogen phosphorylase in rat hepatomas were compared with those in rat tissues during development. A third phosphorylase isozyme, distinguishable kinetically, immunologically, electrophoretically, and by isoelectric focusing from liver and muscle types is commonly present in various rat hepatomas including Novikoff, Morris, and Yoshida hepatomas with various degrees of retent…

Mature teratoma of liver in an adult male patient.

Teratomas are germ cell tumors occurring usually in gonadal organs. They are neoplasms composed of one or more of the three germ layers. Extragonadal teratomas are uncommonly reported in the literature. The liver is an extremely rare site for teratoma, constituting < 1% of all teratomas. The majority of the liver teratomas are found in the pediatric population with only a dozen cases reported in adults to the best of our knowledge. We present a case of a 27-year-old male with a history of abdominal pain of 5 months duration. CT scan revealed liver mass suggesting teratoma. The patient underwen

Liver-Directed Therapy for Neuroendocrine Metastases: From Interventional Radiology to Nuclear Medicine Procedures.

Neuroendocrine neoplasms (NENs) are rare and heterogeneous epithelial tumors most commonly arising from the gastroenteropancreatic (GEP) system. GEP-NENs account for approximately 60% of all NENs, and the small intestine and pancreas represent two most common sites of primary tumor development. Approximately 80% of metastatic patients have secondary liver lesions, and in approximately 50% of patients, the liver is the only metastatic site. The therapeutic strategy depends on the degree of hepatic metastatic invasion, ranging from liver surgery or percutaneous ablation to palliative treatments

Donor origin of neuroendocrine carcinoma in 2 transplant patients determined by molecular cytogenetics.

Organ transplant recipients have an increased tumor incidence owing to their immunocompromised state. The origin of such tumors, whether donor or recipient, will have a clinical impact on decision-making concerning immunosuppressive therapy, retransplantation, and for recipients of other organs from the same donors. We report molecular cytogenetic determination of donor origin in 2 cases of small-cell neuroendocrine carcinoma developing in sex-mismatched transplant recipients (kidney and liver). Fluorescence in situ hybridization (FISH) analysis was performed on liver core needle biopsy materi

Hypervascular hepatic metastases: CT evaluation.

Recent reports have indicated that non-contrast material-enhanced scans are of less value than urographic contrast material-enhanced studies in the computed tomographic (CT) evaluation of hepatic metastases. The authors retrospectively reviewed the CT scans of 28 patients with hypervascular liver metastases to determine whether these metastases were more likely to become isodense with the liver after contrast material enhancement, thus necessitating the performance of non-contrast-enhanced scanning. Non-contrast-enhanced and contrast-enhanced incremental dynamic scanning was performed in patie

Renal tubular dysfunction and cancer mortality in the Japanese general population living in cadmium-non-contaminated areas.

The relationship between cadmium exposure, exposure-related renal tubular dysfunction, and mortality have been reported, mainly in the residents of Cd-contaminated areas in Japan. The aim of this study was to establish the cause-effect relationship between renal tubular dysfunction and cancer mortality in the general population in non-contaminated areas. A 19-year cohort study was conducted in 1110 men and 1703 women in 1993 or 1994, who lived in three cadmium-non-contaminated areas. Mortality risk ratios of urinary β2-microglobulin (β2MG) and N-acetyl-β-glucosaminidase (NAG) for all malignant

Adenomatoid Tumor of the Liver with Prominent Admixed Cavernous Hemangioma-Like Vessels: An Exceptional Occurrence.

Adenomatoid tumors are benign neoplasms of mesothelial phenotype, commonly found in the genital tracts. Hepatic adenomatoid tumors are extremely uncommon and can present diagnostic challenges, particularly when mixed with other histologic elements. Here we report an adenomatoid tumor with prominent admixed cavernous hemangioma-like vessels in the liver of a 58-year-old woman. Grossly, the tumor appeared as a pedunculated lesion on the hepatic surface with cystic spaces filled with blood. Microscopic examination revealed numerous blood vessels of varying sizes intermingled with anastomosing tub

Splanchnic vein thrombosis in myeloproliferative neoplasms: treatment algorithm 2018.

Myeloproliferative neoplasms (MPNs) are a leading cause of splanchnic vein thrombosis (SVT). SVT is observed in all MPNs and frequently affects young patients. Therapy should be addressed to three main goals: preventing thrombosis recurrence, managing the underlying MPN, and supporting liver dysfunction. Life-long oral anticoagulation with vitamin K antagonists is the cornerstone of the antithrombotic treatment. However, recurrences of SVT or other thrombosis may occur in 15-20% of patients. Direct oral anticoagulants can represent an alternative and preliminary data encourage comparative stud

Liver transplantation for hepatoblastoma in a child with congenital absence of the portal vein.

Congenital absence of the portal vein with systemic visceral venous return in a 8-year-old girl with oculoauriculovertebral dysplasia (Goldenhar Syndrome) had been previously reported following its discovery during preoperative evaluation of a liver mass which was diagnosed as focal nodular hyperplasia after open biopsy. Subsequently, an enlarging diffuse hepatic neoplasm developed with associated elevated alpha feto-protein levels. Repeat biopsy and imaging showed the tumor to be a hepatoblastoma involving both lobes of the liver. The patient was treated by hepatic resection and orthotopic li

Towards the optimization of management of hepatocellular carcinoma.

Hepatocellular carcinoma (HCC) is the fifth most common neoplasm in the world, closely correlated with viral hepatitis and liver cirrhosis. The vast majority of HCC patients present at a late stage and are unsuitable for surgery due to limited liver functional reserve. Tumors can involve major vessels or hilar structures, necessitating major liver resection and/or rendering liver resection unfeasible. A series of new technologies have been developed to optimise HCC management. Stem cell therapy improves impaired liver functional reserve prior to liver resection. Intravascular radiofrequency ab

Imaging diagnosis-cholangiocellular adenoma: contrast-enhanced ultrasonographic findings of a benign tumor mimicking malignant neoplasia in a dog.

A 9-year-old Giant Schnauzer was referred for polyuria and polydipsia. On abdominal ultrasound, a hyperechoic mass with low color Doppler signal was detected in the medial right hepatic lobe. Contrast-enhanced ultrasound (CEUS) demonstrated increased enhancement of the mass during the arterial phase, and contrast washout during portal and late phases with decreased enhancement relative to the liver. These findings were consistent with primary liver malignancy or liver metastasis. A final diagnosis of cholangiocellular adenoma was made based on histopathology. To our knowledge, this is the firs

Post-transplant lymphoproliferative disorder presented as small bowel intussusception in adult liver transplant patient.

Intestinal obstruction after liver transplant is a rare complication, with diverse clinical manifestations. Intestinal adhesion is the most common cause. However, internal hernia, abdominal wall hernia, and neoplasm are also reported. Intussusception is another rare cause of intestinal obstruction, which has been reported primarily in pediatric patients. Herein, we report a case of intestinal obstruction from intussusception in an adult liver transplant patient associated with post-transplant lymphoproliferative disorder.

Desmoid fibromatosis of the liver: a case report and rare entity with diagnostic resolution by histopathology and immunohistochemistry.

BACKGROUND: Desmoid tumors (aggressive fibromatosis) are rare, locally invasive fibroblastic neoplasms with no metastatic potential but a propensity for local recurrence. Primary involvement of the liver is exceptionally uncommon and poses significant diagnostic challenges due to overlapping radiologic features with primary hepatic malignancies and other spindle cell neoplasms. Large mesenchymal tumors may rarely be associated with nonislet cell tumor hypoglycemia. CASE PRESENTATION: A 71-year-old woman presented with a five-year history of progressive abdominal swelling and recurrent hypogly
