Recycling and Reinforcing Intimomedial Flap of the Infrarenal Aorta Using Anterior Longitudinal Ligament in Patients With Acute Trauma With Bowel Injuries.

We present a patient with blunt abdominal trauma with severe acute right limb ischemia and clinical signs of diffuse peritonitis. Computed tomography angiography showed circumferential dissection of the infrarenal aorta with occlusion of the right common iliac artery. We opted for simultaneous abdomen exploration and open repair of injured aorta. Critical weakening of the aortic wall with imminent rupture was identified intraoperatively. Aortotomy cranially from bifurcation showed circumferential intimomedial dissection. The fixation of fragile intimomedial flap of aortic dissection was achiev

Whole exome sequencing implicates PTCH1 and COL17A1 genes in ossification of the posterior longitudinal ligament of the cervical spine in Chinese patients.

Ossification of the posterior longitudinal ligament (OPLL) of the cervical spine is a complex multifactorial disease. Patients with OPLL commonly present with symptoms in their 40s or 50s. The genetic basis of OPLL remains poorly understood. Exome capture combined with massively parallel DNA sequencing has been proposed as an efficient strategy to search for disease-causing genes of both monogenic and multigenic disorders. To identify candidate pathogenic genes associated with OPLL, we performed whole exome sequencing (WES) on two unrelated southern Chinese OPLL patients. The entire DNA coding

Inhibitory effect of YQHYRJ recipe on osteoblast differentiation induced by BMP-2 in fibroblasts from posterior longitudinal ligament of mice.

Ossification of posterior longitudinal ligament (OPLL) is a common disease in Asian countries. Osteoblast differentiation in posterior longitudinal ligamentous fibroblast is a pathologic basis of OPLL. Nowadays, an effective pharmacotherapy for OPLL is still hunted for. YQHYRJ Recipe (YQHYRJ) is designed based on traditional Chinese medicine (TCM) theories, and previous clinic trials reported its effect on relieving syndromes of cervical spondylopathy. To clarify the YQHYRJ effect of OPLL on a cellular level, we induced mice fibroblasts from posterior longitudinal ligaments to differentiate in…

Multicenter study investigating the postoperative progression of ossification of the posterior longitudinal ligament in the cervical spine: a new computer-assisted measurement.

OBJECT: Ossification of the posterior longitudinal ligament (OPLL) often progresses after surgery, and this may cause late-onset neurological deterioration. There have been few studies, however, to clarify any correlation between progression and clinical outcome, partly because of the lack of studies involving reliable and reproducible methods by which detection of progression is made possible. The authors conducted a multicenter study to investigate the occurrence of postoperative progression and to elucidate the possible risk factors in a large-scale patient population, and a novel computer-

Radicular compression by lumbar intraspinal epidural gas pseudocyst in association with lateral disc herniation. Role of the posterior longitudinal ligament.

Among unusual abnormalities of the lumbar spine reported since the introduction of Computed Tomography (CT), the presence of gas lucency in the spinal canal, known as vacuum phenomenon, is often demonstrated. On the contrary, epidural gas pseudocyst compressing a nerve root in patients with a lateral disc herniation has rarely been reported. We report a case of a 44-year-old man who experienced violent low back pain and monolateral sciatica, exacerbated by orthostatic position, one week before admission. A lumbosacral spine CT showed the presence of vacuum phenomenon associated with a degenera

[Roentgenological measurement of the cervical vertebral bodies in ossification of the posterior longitudinal ligament (OPLL) and cervical spondylosis (CS) (author's transl)].

Many studies of the pathogenesis of ossification of posterior longitudinal ligaments (OPLL) have been reported, and both general and local factors have been pointed out. In case a longitudinal ligament is affected by both the general and local OPLL factors for many years, the shape and size of the cervical vertebral bodies and the arrangement of the cervical spinal column might be affected. The relation between OPLL and disc degeneration, the morphology of the facet joint and the change of longitudinal ligament tension has been reported by many authors. But the relation between OPLL and the sh…

Early somatosensory evoked potential monitoring warnings during positioning for cervical laminoplasty in ossification of the posterior longitudinal ligament patients.

BACKGROUND: Neurological deterioration during cervical laminoplasty for ossification of the posterior longitudinal ligament (OPLL) remains a significant clinical concern. This study evaluated the utility of somatosensory evoked potential (SEP) monitoring initiated immediately after anesthetic induction as an early warning system during surgical positioning. METHODS: We retrospectively reviewed 63 consecutive patients with cervical OPLL who underwent laminoplasty (2023-2026). SEP monitoring of all four limbs was initiated in the supine position after induction to establish the intraoperative b

Effect of continued prenatal Pilates until childbirth on postpartum ligamentous recovery: a longitudinal follow-up of a randomized controlled trial.

BACKGROUND: Pregnancy-related hormonal and biomechanical adaptations increase ligamentous laxity and joint mobility, which may persist into the postpartum period and contribute to pain and functional limitations. Although prenatal Pilates attenuates ligamentous laxity during pregnancy, its effects on early postpartum recovery remain unclear. This study aimed to investigate whether continuation of a structured prenatal Pilates program until childbirth facilitates ligamentous recovery and improves functional outcomes at six weeks postpartum. METHODS: This dual-center, parallel-group randomized

Machine learning and neural network algorithms for prediction of C5 palsy after posterior surgery for ossification of posterior longitudinal ligament.

PURPOSE: C5 palsy (C5P) is one of the main postoperative complications of ossification of the posterior longitudinal ligament (OPLL). However, an accurate predictive tool for C5P remains to be developed. Forecasting this potentially avoidable consequence preoperatively can improve patient management, shared decision-making, and counseling. METHODS: Electronic medical records and radiographic measurements of patients with OPLL who underwent posterior cervical spine surgery between January 1, 2012 and May 1, 2022 were retrospectively collected and analyzed. The data were split into training (80
