Barriers and facilitators to implementing prevent-teach-reinforce for young children in community-based early intervention services for autism.

The present study examined barriers and facilitators to the implementation of evidence-based practices to manage challenging behavior under real-world conditions within community-based autism services. Parents, early intervention practitioners, and administrators at a public agency who participated in Prevent-Teach-Reinforce for Young Children (PTR-YC), an intervention based on positive behavior support principles, shared their perspectives on the factors that helped or hindered the implementation of the program. Barriers and facilitators were identified at the macrosystemic (community), servi

Zero-Cost-Based Management Theory: A Conceptual Framework for Organizational Transformation.

Escalating costs, particularly in healthcare, have intensified the need for approaches that improve performance while preserving financial sustainability. Existing models such as zero-based budgeting (ZBB), zero-based cost management (ZBCM), and value-based management emphasize expenditure control and efficiency but often assume the availability of resources for implementation. This paper introduces Zero-Cost-Based Management Theory (ZCBMT), a conceptual framework explaining how organizations can achieve meaningful organizational change with minimal additional financial investment. The theory

Migraine and Headache Care in the Republic of Ireland: History and a Vision for the Future Influenced by the COVID-19 Pandemic.

BACKGROUND: The care and management of migraine/headache patients in the Republic of Ireland over the last 25 years are summarized in this article. METHODS: Collaboration between voluntary patient organizations (the Migraine Association of Ireland or MAI), primary care services, and hospital/community shared healthcare professionals (most notably the headache specialist nurse), is highlighted as one of the key features of this management strategy in an underfunded and under-resourced public healthcare system. CONCLUSION: The migraine/headache community in Ireland is small, but they have been

A review of the incidence and transmission of Listeria monocytogenes in ready-to-eat products in retail and food service environments.

Contamination of ready-to-eat products with Listeria monocytogenes may occur at several stages before consumption. Accessibility to the public and relatively limited control interventions at retail and food service establishments (compared with the processing sector of the food industry) and the lack of a specific regulatory framework increase the likelihood of introduction of this pathogen into some foods in these establishments. This review is a compilation of available information on the incidence and transmission of L. monocytogenes through ready-to-eat products at the retail and food serv

A rapid assessment of take-home naloxone provision during COVID-19 in Europe.

BACKGROUND: In March 2020, the World Health Organization declared COVID-19 a global pandemic. In the following weeks, most European countries implemented national lockdowns to mitigate viral spread. Services for people who use drugs had to quickly revise their operating procedures to rearrange service provision while adhering to lockdown requirements. Given the scarcity of literature published on overdose prevention during COVID-19 in Europe, we aimed to examine how these changes to service provision affected take-home naloxone (THN) programmes and naloxone availability across Europe. METHODS

Rectal Adenocarcinoma Presenting as a Cervical Mass: A Case Report.

BACKGROUND Invasive cervical tumors are often seen in clinical practice. However, there are multiple structures within the pelvis, and invasion of the cervix from another site must be included in the differential diagnosis. In such cases, a multidisciplinary approach is needed to define the organ of tumor origin. Ensuring proper staging and histologic analysis are critical for optimal management. CASE REPORT We present a case of a 68-year-old woman who presented to her gynecologist with painless post-menopausal vaginal bleeding. She was diagnosed with a locally aggressive cervical adenocarcino

Dental care for aging populations in Denmark, Sweden, Norway, United kingdom, and Germany.

This article reviews access to and financing of dental care for aging populations in selected nations in Europe. Old age per se does not seem to be a major factor in determining the use of dental services. Dentition status, on the other hand, is a major determinant of dental attendance. In addition to perceived need, a variety of social and behavioral factors as well as general health factors have been identified as determinants of dental service use. Frail and functionally dependent elderly have special difficulties in accessing dental care; private dental practitioners are hesitant to provid

[Vertebral hydatidosis: case report].

Hydatidosis caused by echinococcus granulosus may affect any organ in the body, with the lungs and the liver as the most commonly affected organs. Vertebral compromise resulting from echinococcus granulosus has a low prevalence and accounts for less than 1% of bone compromise. We report the case of a 50 year-old female who presented at the Trauma Service with progressive low back pain with 5 months of duration that irradiated to the right lower limb, and led to neurologic compromise of the limb. Imaging studies showed spondylodiscitis at T12-L1, confirmed by a biopsy. Treatment of this conditi

Management of portal hypertension-related bleeding.

Although endoscopic sclerotherapy and TIPS remain the primary therapeutic tools in management of acute variceal bleeding, surgical shunts must be considered for low-risk patients with bleeding. OLTx is the only definitive treatment for patients with end-stage liver disease and vascular decompensation. Furthermore, the current prospective multicenter randomized study, funded by the National Institutes of Health and Human Services, will help determine the role of DSRS versus TIPS in cirrhotic patients with good hepatic reserve. This is a necessity in a time in which organ shortages are ever-incr

[Rethinking aging, autonomy and disability in France: A University-driven, territory-based, and prevention-oriented transformation of the health system - Position of the French Conference of Deans of Medical Faculties].

Population ageing, the increasing burden of chronic diseases, and the growing prevalence of disability are major challenges for the French health system. These trends expose the limitations of a model that remains largely organized around disease and dependency, with fragmented coordination between healthcare, social care, and community services. This fragmentation results in disrupted care pathways, delayed identification of functional decline, territorial inequalities, and an increasing burden on patients, caregivers, and health professionals. This position paper presents the vision of the F

Covid-19 pandemic: from carnival masks to surgical masks.

Given the high transmissibility of SARS-CoV-2, COVID-19 pandemic has a huge impact on our health system. Even in developed countries, strategic resources soon become insufficient. Although people over 60 and with comorbidities are at greater risk of developing severe forms, younger people may also require precious and scarce care. Hence, the World Health Organization recommend tests - PCR and serological tests - for detecting infected people on a large scale. The most common symptoms are fever, fatigue, dry cough, anorexia, myalgia, and dyspnea, with tomographic pulmonary findings being freque
