Analysis of two different surgical approaches for fractures of the mandibular condyle.

BACKGROUND: Fractures of the condyle account for one third of all the mandibular fractures. Different surgical approaches to the condyle described hitherto testify to the advantages and disadvantages of the different surgical techniques used for approaching the condyle in such cases of fractures. We have described and compared two of such surgical techniques in this study. AIM: The aim of this study is to compare the outcome of dealing with condylar fractures by two different surgical techniques: the mini retromandibular approach, and the preauricular approach. MATERIALS AND METHODS: A prosp…

Surgical treatment on displaced and dislocated sagittal fractures of the mandibular condyle.

PURPOSE: The purpose of this study was to evaluate the effect of surgical treatment on displaced and dislocated sagittal fractures of the mandibular condyle (SFMC). PATIENTS AND METHODS: Twenty-four patients with 28 displaced and dislocated SFMCs were distinguished into type M, type C, and type L fractures according the location of the fracture line. The fractured fragment was reduced and fixated with two 0.6-mm 4-hole micro-plates via a preauricular temporal incision. The fragment was extirpated when it was too small to be fixated. The postoperative position and profile of the fragment was e

Treatment of mandibular angle fractures with a matrix miniplate: a preliminary report.

Mandibular angle fractures are technically challenging, and a spectrum of techniques for treatment of these fractures has been proposed in the literature. Currently, fixation with one or two miniplates has become a widely accepted method of providing internal fixation and eliminating the need for postoperative maxillomandibular fixation. In this study, the utility of a single 2.0-mm matrix miniplate for mandibular angle fracture management was examined. In a laboratory biomechanical analysis, the overall stability of the single 2.0-mm matrix miniplate compared favorably with two 2.0-mm minipla

[The use of vibrotherapy to prevent suppurative-inflammatory complications in mandibular fractures].

Patients with uni- and bilateral traumatic fractures of the mandible at risk for pyoinflammatory complications were subjected to vibration massage at frequencies identical to those of cardiovascular system. Rheographic evaluation of the blood flow showed vibration-related normalization of the regional circulation at the lesion which went faster and reached the values on the healthy side on day 14. In controls similar improvement took place only in 58% of cases. The results suggest that vibration massage at cardiovascular frequencies can effectively prevent pyoinflammatory complications in the

Midface deviation due to mandibular fractures. (An experimental study with clinical comparison).

An experimental study was undertaken into the effect of fractures produced in the body of the mandible, the base of the condylar process and the mandibular condyles, on the bones of the middle third of the face using rabbit sucklings. The rabbits were sacrificed when fully grown, when it was found that in all cases the bones of the middle face had deviated to the traumatized side in comparison with the neurocranium. The greatest asymmetry occurred after injuries to the condyles and least following injuries to the body of the mandible. A comparative study on patients past growing age who had ha

Outcomes of Open, Percutaneous, and Closed Treatment of Isolated Mandibular Condylar Fractures.

Objectives Mandibular condylar fractures are common facial fractures, and the optimal balance between open and less invasive treatment remains debated. Readmission provides a reproducible hospital-utilization outcome that can be examined in large administrative datasets. This study aimed to compare 30-day readmission following open, percutaneous, and closed treatment of mandibular condylar fractures (MCF) using national inpatient databases. Methods We performed a retrospective database study using the 2016 Kids' Inpatient Database (KID) and the 2019-2020 National Readmissions Database (NRD). K

Maxillomandibular Fixation Using Orthodontic Brackets as a Treatment Option for Mandibular Fractures.

Maxillomandibular fixation (MMF) is a treatment option for mandibular fractures. Various techniques can be used for MMF which include arch bars, eyelets, and intermaxillary fixation (IMF) screws. When a patient undergoing orthodontic treatment experiences a mandible fracture, conventional MMF wiring techniques cannot be employed without removal of the existing orthodontic brackets. In such cases, the existing orthodontic brackets can be utilized for MMF. To the best of the author's knowledge, this is the first documented case in the literature where a mandible fracture in a patient undergoing …

Does closed treatment of paediatric mandibular condyle fractures result in restitutional remodelling?

The treatment of paediatric mandibular condylar fracture (PMCF) is typically non-operative. The purpose of this study was to determine if non-operative management of PMCF results in a new condylar process of normal morphology to regenerate after closed treatment (restitutional remodelling). The specific aim of the study was to observe restitutional remodelling (RM) in PMCF and review the literature. The investigators designed and implemented a retrospective study on paediatric patients (age<12) with unilateral or bilateral condyle fractures treated with non-operative treatment between January

Surgical Management of Mandibular Subcondylar Fractures Under Local Anesthesia: A Proposed Protocol.

PURPOSE: The optimal treatment for adult mandibular condylar fractures (MCFs) has largely shifted in favor of open reduction and rigid internal fixation. However, a sizeable portion of these injuries are still being treated in a closed manner based on old literature, convenience, results deemed acceptable, and lesser associated morbidity. Most MCFs with multiple associated facial traumas are appropriately treated under general anesthesia (GA). However, some selected isolated MCFs or those with minimal associated mandibular or low midfacial fractures can be treated optimally with patients under
