PURPOSE: In orthognathic surgery for skeletal mandibular prognathism, maxillary clockwise rotation with posterior impaction and mandibular setback is often performed. Change in airway volume after orthognathic surgery is one of the concerns. The present study was performed to evaluate the effects of maxillary clockwise rotation with posterior impaction and the influence of different mandibular techniques, including sagittal splitting ramus osteotomy (SSRO) and intraoral vertical ramus osteotomy (IVRO), on upper airway volume changes before and after bimaxillary surgery for mandibular prognathi

We report a case of oblique osteotomy and coronoidectomy for correction of extreme prognathism in Apert syndrome.A 16-year-old girl presented with a prognathic mandible. A prototype model was formed using computed tomography. After a mock bilateral oblique osteotomy of the mandible, the distal segment did not overlap the proximal segment, and it caused bony impingement between the coronoid process and the subcondyle of the proximal segment. An oblique osteotomy of the mandible was done through a submandibular approach, and bilateral coronoidectomy followed. After surgery, mandibular parameters

Mandibular surgery encompasses a wide range of procedures, including the management of traumatic fractures and elective osteotomies, where optimal bone healing and soft-tissue regeneration are essential for successful clinical outcomes. Platelet-rich fibrin (PRF), a second-generation autologous platelet concentrate, has gained increasing attention because of its ability to provide a fibrin scaffold enriched with platelets, leukocytes, cytokines, and growth factors that promote angiogenesis, osteogenesis, and soft-tissue regeneration. This narrative review summarizes the current evidence regard

OBJECTIVES: This study aimed to investigate the mandibular canal of ramus and design a suitable osteotomy line for intraoral vertical ramus osteotomy (IVRO) using cone-beam computed tomography (CBCT).
MATERIALS AND METHODS: Ninety patients were classified into class I, II, and III skeletal pattern groups. When extended from the horizontal base plane (0 mm, mandibular foramen [MF]), with a 2-mm section interval, to 10 mm above and 10 mm below the MF, the following landmarks were identified: external oblique ridge (EOR), posterior border of the ramus (PBR), and posterior lateral cortex of ramus

Mandibular osteotomy carries with it a risk of damaging blood vessels and nerves when using traditional surgical techniques. Piezosurgery®, is a new technique that uses ultrasonic vibration to enable bone-selective sectioning without damage to the surrounding soft tissues. However, paralysis may not be completely eliminated using Piezosurgery® for osteotomy. We investigated how piezoelectric surgery in bilateral sagittal splitting ramus osteotomy (BSSRO) affected the surrounding soft tissue. Forty-four patients with skeletal mandibular prognathism underwent mandibular setback with BSSRO. Patie…

An invitation to report for a follow-up was accepted by 74 patients (51 females and 23 males) who had undergone various types of osteotomy (50 mandibular and 30 maxillary osteotomies; 6 patients had undergone both mandibular and maxillary osteotomy). Their age at the time of operation ranged from 15 to 44 years. The follow-up period ranged from 3 months to 7 years. Maxillary osteotomy was never followed by a relapse, but 12 of the 50 patients treated by mandibular osteotomy showed some relapse: hyperplasia of the mandibular condylar process caused some relapse in 2 cases, and in 10 other cases…

The treatment of mandibular deformities with an anterior open bite is challenging. In this study, skeletal stability after mandibular osteotomies was evaluated to determine the best treatment for mandibular prognathism with an anterior open bite in three procedures: intraoral vertical ramus osteotomy (IVRO), conventional sagittal split ramus osteotomy (conv-SSRO), and SSRO without bone fixation (nonfix-SSRO). Patients who underwent mandibular osteotomy to correct skeletal mandibular protrusion were included. Changes in skeletal and soft tissues were assessed using lateral cephalograms taken be…

PURPOSE: Fracture lines in unfavorable locations are referred to as "bad splits" in a mandibular sagittal split osteotomy (SSO). Several modifications of the technique by Obwegeser have been introduced to minimize this risk. This in vitro study was performed to determine whether the shape of the osteotomy cut affects the torque and the fracture pattern of an SSO in pig mandibles.
MATERIALS AND METHODS: In a split-mouth model, 16 mandibles were split according to the Hunsuck-Dal Pont modification of the Obwegeser technique. Using an oscillating saw, sharp-edged osteotomies were created on one

This study was performed to investigate the impact of the split line on postoperative stability and temporomandibular joint (TMJ) disorders (TMD) after sagittal split ramus osteotomy. Skeletal Class III malocclusion patients who underwent mandibular osteotomy with/without plate fixation between August 2008 and March 2023 were enrolled retrospectively. The osteotomy lines were divided into original sagittal split ramus (SSRO), short lingual (SLO), and short buccal (SBO) osteotomies. Skeletal stability was evaluated, and TMDs were assessed preoperatively. Overall, 135 patients (270 TMJs) (87 fem…

A technique of harvesting bone grafts from the medial surface of the angle of the mandible during a bilateral sagittal split osteotomy procedure is described. In 20 patients who underwent mandibular setback for the correction of class III dentofacial deformities, bone grafts were harvested from the medial mandibular angle and used for simultaneous augmentation of the midface or for interpositioning and stabilization of the maxilla after LeFort I maxillary anterior or inferior repositioning. The mean postoperative follow-up was 6 months (range, 3-12 months). No complications occurred, and posto

BACKGROUND: Mandibular hypoplasia may result from congenital deformities or trauma or infection during the early stage of facial skeleton development. Deficiencies in the growth of the mandible can not only cause various degrees of facial deformity but also affect breathing and occlusal function. Here, we report our experiences with mandibular distraction combined with orthognathic surgical techniques for the treatment of severe adult mandibular hypoplasia.
METHODS: Cephalometric analysis was conducted in all patients for quantitative evaluation. A computer-assisted surgical simulation was pr

INTRODUCTION: Mandibular resection and reconstruction are common but complex procedures in head and neck surgery. Resection with adequate margins is critical to the success of the procedure but technical training is restricted to real case experience. Here we describe our experience in the development and evaluation of a mandibular resection and reconstruction simulation module.
METHODS: 3D printed (3DP) models of a mandible with a pathologic lesion were developed from imaging data from a patient with an ameloblastoma. During an educational conference, otolaryngology trainees participated in

PURPOSE: Today, the most common orthognathic procedure for correction of mandibular deformities is the bilateral sagittal split osteotomy, also called sagittal ramus osteotomy. Permanent injury to the mandibular nerve (V3) is one of the main complications, with a reported incidence between 5% and 30%. Orthognathic surgery using sagittal ramus osteotomy of the mandible as the procedure of choice should be re-evaluated because of the complexity and the relatively high risk of damage to the inferior alveolar nerve. Surgical techniques that allow for accurate condylar positioning with a lower risk
