BACKGROUND: Despite increases in musculoskeletal disorders (MSD) in Canada, evidence suggests utilization of chiropractic services has remained relatively stable over time. Understanding the extent to which chiropractors are consulted and factors associated with their utilization may suggest factors related to accessing care. We assessed the change in prevalence and characteristics of Canadians seeking chiropractic care across two time periods 2001-2010 and 2015-2018.
METHODS: We used national cross-sectional data from seven cycles of the Canadian Community Health Survey between 2001 and 2018

OBJECTIVES: The comparative effect of chiropractic vs medical care on health, as used in everyday practice settings by older adults, is not well understood. The purpose of this study is to examine how chiropractic compares to medical treatment in episodes of care for uncomplicated back conditions. Episodes of care patterns between treatment groups are described, and effects on health outcomes among an older group of Medicare beneficiaries over a 2-year period are estimated.
METHODS: Survey data from the nationally representative Survey on Assets and Health Dynamics among the Oldest Old were l

OBJECTIVE: To describe a less common initial symptom of nephrolithiasis, its diagnostic pitfalls, risk factors, and mimicry of other conditions. Intervention and long-term management of nephrolithiasis is also discussed.
CLINICAL FEATURES: A Caucasian man aged 25 years had sudden bilateral inguinal and occasional periumbilical pain. The initial symptom suggested an abdominal pathologic condition; however, costovertebral angle pain followed 1 hour later with no radiation between the 2 anatomic sites. The initial urine dipstick result was negative for hematuria, but a kidney, ureter, and bladde

BACKGROUND: Coflex interlaminar stabilization (CIS) is a second-generation interspinous implant that promotes intersegmental flexion following decompression for moderate to severe lumbar spinal stenosis. We describe the management of a patient with persistent spinal pain syndrome-2 (PSPS-2) status-post CIS implant presenting to a chiropractor's office.
CASE PRESENTATION: A 77-year-old Hispanic male Army veteran presented with PSPS-2 status-post L3-L5 laminectomy and CIS for lumbar spinal stenosis. A 4-visit trial care plan ensued with flexion-distraction manipulation, drop-assist spinal manip

OBJECTIVES: The purpose of this study was to use previously validated methods to quantify and relate 2 phenomena associated with chiropractic spinal manipulative therapy (SMT): (1) cavitation and (2) the simultaneous gapping (separation) of the lumbar zygapophyseal (Z) joint spaces.
METHODS: This was a randomized, controlled, mechanistic clinical trial with blinding. Forty healthy participants (18-30 years old) without a history of low-back pain participated. Seven accelerometers were affixed to the skin overlying the spinous processes of L1 to L5 and the S1 and S2 sacral tubercles. Two addit

Scoliosis in patients with rheumatoid arthritis (RA) can cause significant pain and disability. RA has been extensively studied in relation to the cervical spine, yet the pathology of the thoracic and lumbar spine in RA patients has been largely overlooked. A 66-year-old woman, with longstanding RA and severe scoliosis, presented to the chiropractic clinic with a five-month history of exacerbated low back pain radiating to the right lower limb. The patient was treated with a combination of full-spine mechanical spinal distraction, spinal manipulative therapy, mechanical distraction of the cerv

BackgroundChanges in pain sensitivity are a commonly suggested mechanism for the clinical effect of spinal manipulative therapy (SMT). Most research has examined pressure pain thresholds (PPT) and has primarily been conducted in controlled experimental setups and on asymptomatic populations. Many important factors are likely to differ between research and clinical settings, which may affect PPT changes following SMT. Therefore, we planned to investigate PPT before and after clinical chiropractic care and investigate relationships with various potentially clinically-relevant factors.MethodsWe r

BACKGROUND: Patient reported outcome measure (PROM) use is a marker of high-quality chiropractic care. Limited information exists regarding use of PROMs by United States chiropractors, especially within integrated health systems like the Veterans Health Administration (VHA) that prioritize high-quality, data-driven performance monitoring. This study aimed to understand VHA chiropractor perspectives on PROM use, including specific measures, administration characteristics (initial and follow-up timing), workflow, and perceived utility. METHODS: A national electronic survey of VHA chiropractors w

Acute traumatic stroke of the cerebellum is rarely seen in children. In adults, chiropractical manipulation, yoga exercises, bow hunting and cervical trauma have all been associated with vertebrobasillar damage and subsequent stroke due to cerebellar infarction. We present a case of bow hunter's stroke in a child. An 11-year-old boy developed deep coma one day after minor occipital head injury due to an infarct in the left cerebellum and ipsilateral medulla oblongata. Magnetic resonance angiography (MRA) showed hypoperfusion of the left vertebral artery and occlusion of the posterior and anter

Abdominal migraine (AM) is a pediatric functional gastrointestinal disorder characterized by recurrent episodes of abdominal pain and associated symptoms, for which existing pharmacological therapies may be limited by suboptimal efficacy and adverse effects, underscoring the need for alternative treatment approaches. This case report describes a 10-year-old girl diagnosed with mild thoracic scoliosis (20° Cobb angle) who complained of recurrent periumbilical pain for the past 16 months that lasts between two and 24 hours, accompanied by vomiting and pallor. These episodes were triggered by str

BACKGROUND: Randomized clinical trials (RCTs) are the gold standard for assessing treatment effectiveness; however, they have been criticized for generalizability issues such as how well trial participants represent those who receive the treatments in clinical practice. We assessed the representativeness of participants from eight RCTs for chronic spine pain in the U.S., which were used for an individual participant data meta-analysis on the cost-effectiveness of spinal manipulation for spine pain. In these clinical trials, spinal manipulation was performed by chiropractors.
METHODS: We condu
