Negotiating genetic risk in Indonesia: Development and psychometric evaluation of the knowledge and attitude toward consanguineous marriage instrument in a mixed-methods cross-sectional study.

BACKGROUND: Consanguineous marriage can increase recessive genetic risks in offspring, yet instruments for examining genetic-risk understanding and attitudes remain limited in Indonesia. This study developed and evaluated a questionnaire assessing knowledge responses and attitudes toward consanguineous marriage in Kalimantan. METHODS: In a convergent mixed-methods cross-sectional survey, 272 respondents completed ten Yes/No knowledge items with written justifications and 15 Likert-type attitude statements. Knowledge responses were examined using Rasch modeling, attitude items using explorator…

Mapping the evidence on interventions that mitigate the health, educational, social and economic impacts of child marriage and address the needs of child brides: a systematic scoping review.

More than 650 million women alive today were married as children. Relative to efforts to prevent child marriage, efforts to support child brides have received much less attention. This review set out to map and describe interventions that support child brides. We performed a scoping review using seven electronic databases coupled with a grey literature search in January 2022. Data were extracted using a piloted extraction tool and findings were reported in narrative synthesis. A total of 34 projects were included in our review. Most projects focused on improving sexual and reproductive health …

Molecular investigation of interspousal transmission of hepatitis C virus in two Japanese patients who acquired acute hepatitis C after 40 or 42 years of marriage.

A 65-year-old woman (C1I) and a 65-year-old man (C2I) contracted acute hepatitis C 40 or 42 years after marriage, respectively, in Japan. They had no discernible risk factors for acquiring hepatitis C virus (HCV) infection, except that they had monogamous sexual relationships with their spouses (C1S [66-year-old] with hepatocellular carcinoma and C2S [64-year-old] with liver cirrhosis, respectively) who were infected with HCV of the same genotype (1b) and had a high-titer HCV RNA in the serum (bDNA probe assay, 17 Meq/ml [C1S] and 15 Meq/ml [C2S]). The HCV isolates from Patients C1I and C1S an…

Transitions Into and Out of Perceived Foreign-Origin Discrimination Among Marriage-Migrant Women in South Korea: Asymmetric Effects on Acculturative Stress and Heterogeneity by Country of Origin.

In recent decades, South Korea has transitioned from an ethnically homogeneous nation to a more multicultural society, with foreign-born women marrying Korean men accounting for most new international marriages. Despite government efforts to support integration, marriage-migrant women continue to experience foreign-origin discrimination. This study examines the association between perceived foreign-origin discrimination and acculturative stress, with a focus on distinguishing between the effects of transition into and out of discrimination. This study also examines whether these effects vary b…

Age at first marriage, age at first sex, family size preferences, contraception and change in fertility among women in Uganda: analysis of the 2006-2016 period.

BACKGROUND: Uganda's fertility was almost unchanging until the year 2006 when some reductions became visible. Compared to age at first marriage and contraceptive use, age at sexual debut and family size preferences are rarely examined in studies of fertility decline. In this study, we analyzed the contribution of age at first marriage, age at first sex, family size preferences and contraceptive use to change in fertility in Uganda between 2006 and 2016. METHODS: Using data from the 2006 and 2016 Uganda Demographic and Health Survey (UDHS), we applied a nonlinear multivariate decomposition tec…

Molecular analysis of the interspousal transmission of hepatitis B virus in two Japanese patients who acquired fulminant hepatitis B after 50 and 49 years of marriage.

A 71-year-old (C1I) and 69-year-old (C2I) Japanese female contracted fulminant hepatitis B after 50 and 49 years of marriage, respectively. Both index cases exhibited high levels of anti-HBc IgM antibodies (24.2 and 31.5 S/CO, respectively), suggestive of acute hepatitis B virus (HBV) infection, although they had no discernible risk factors for HBV infection, except for chronically HBV-infected spouses with detectable HBV DNA (3.3 log copies/ml [C1S: 72-year-old] and 7.2 log copies/ml [C2S: 71-year-old]). The HBV genotype/subgenotype was identical in each couple (B/B1 or C/C2). The HBV isolate…

Examining perceived effects of same-sex marriage legalization among sexual minority women: Identifying demographic differences and factors related to alcohol use disorder, depression, and self-perceived health.

BACKGROUND: Reductions in structural stigma, such as gaining access to legalized same-sex marriage, is associated with positive psychological and physical health outcomes among sexual minorities. However, these positive outcomes may be less robust among sexual minority women (SMW). METHODS: This study examined how perceptions of the impact of legalized same-sex marriage among SMW may 1) differ by demographic characteristics and 2) predict alcohol use disorder, depression, and self-perceived health. A diverse sample of SMW (N=446) completed an online survey in 2020 assessing the perceived impa…

Innovative approaches to promoting cervical health and raising cervical cancer awareness by use of existing cultural structures in resource-limited countries: experiences with traditional marriage counseling in Zambia.

The Cervical Cancer Prevention Program in Zambia (CCPPZ) has increasingly used community-level structures to increase the uptake and ensure the sustainability of the program. Traditional marriage counselors, the alangizi, who have existed in the Zambian society for many years, are one of the structures used by the program to impart cervical cancer knowledge and increase access to screening and care using an existing community structure. Several steps were followed in developing this intervention: (a) ensuring the alangizi understood the process of screening by encouraging them to go through th…

U.S. marital disruptions and their economic and social correlates, 1860-1948.

A new estimate of U.S. marital disruptions shows an increase in desertions relative to divorces after 1900. Desertions were the more volatile component of marital disruptions because of their greater responsiveness to general economic conditions. Large marriage cohorts, formed in the years of economic expansion, disrupted in greater numbers: an increase in the marriage rate by 10 per 1,000 unmarried women raised the proportion of disrupted marriage by 7.3 percentage points. Conversely, during years of recession, many poorer couples were discouraged from marriage; smaller marriage cohorts with …

Exploring married girls' subjective experiences of well-being and challenges.

INTRODUCTION: The well-being of married girls is often significantly affected by their early transitioning into the institution of marriage. This is accompanied by a normative shift in their focus from education and personal development to family life and motherhood. However, it is important we understand married girls' perspectives of what their marriage mean to them and how that affects their well-being. Therefore, this study explored the subjective experiences of well-being and challenges among married girls in the Northern region of Ghana. METHODS: A phenomenological approach was used to …

Marital status, immigration, and reproductive health among adolescent mothers in Canada, 1990-2018: A population-based, observational study.

Immigrants to Canada increasingly come from regions where child marriage (<18 years) is prevalent. We described the prevalence, demographic characteristics, and reproductive health correlates of marriage among births to Canadian-born and foreign-born adolescent mothers. Using Canadian birth registrations from 1990 to 2018, marriage prevalence, parental birth region, and parental age gap were examined by maternal birthplace (Canada and 12 world regions) among births to mothers <18 years. Adjusted odds ratios (AORs) of preterm birth (PTB), small for gestational age (SGA), and repeat birth were e…

Cohabitation, post-conception unions, and the rise in nonmarital fertility.

The majority of U.S. nonmarital births today are to cohabiting couples. This study focuses on transitions to cohabitation or marriage among pregnant unmarried women during the period between conception and birth. Results using the newly-released 2006-2010 National Survey of Family Growth show that nonmarital pregnancy is a significant precursor to cohabitation before childbirth (18%), exceeding transitions to marriage (5%) by factor of over three. For pregnant women, the boundaries between singlehood, cohabitation, and marriage are highly fluid. The results also reveal substantial variation in…
