Partial breast irradiation versus whole breast radiotherapy for early breast cancer.

BACKGROUND: Breast-conserving therapy for women with breast cancer consists of local excision of the tumour (achieving clear margins) followed by radiotherapy (RT). Most true recurrences occur in the same quadrant as the original tumour. Whole breast radiotherapy (WBRT) may not protect against the development of a new primary cancer developing in other quadrants of the breast. In this Cochrane Review, we investigated the delivery of radiation to a limited volume of the breast around the tumour bed (partial breast irradiation (PBI)) sometimes with a shortened treatment duration (accelerated par

Pattern of distribution of intraductal and infiltrating ductal carcinoma: a three-dimensional study using serial coronal giant sections of the breast.

The purpose of this study was to establish the 3-dimensional (3D) structure of the breast tissue and to study the distribution and relationship between the intraductal and infiltrating components of ductal carcinoma and other proliferative epithelial lesions of the breast. Thirty mastectomy specimens with infiltrating carcinoma less than 3.0 cm in diameter were serially cut in the coronal plane. Each giant section was divided into small sections for routine processing. Using Photoshop (Adobe) and PowerPoint (Microsoft) software programs, the routinely stained sections were scanned and assemble

Breast carcinoma in situ in 167 women--incidence, mode of presentation, therapy and follow-up.

In the city of Malmö, in southern Sweden, 1693 women were diagnosed as having breast carcinoma during 1976 through 1984. Of these, 167 women had pure in situ breast carcinoma (9.9%). One hundred and thirty-two had ductal carcinoma in situ (DCIS) alone or in combination with lobular carcinoma in situ (LCIS), intracystic carcinoma and/or Paget's disease of the nipple. Thirty-three had pure LCIS and two had pure intracystic carcinomas. The incidence of breast carcinoma in situ (CIS) in women 20 years of age or older was 18.7 per 10(5) woman years with high rates of DCIS for all ages above 40, whe

Pushing the Envelope: Skin-Only Mastopexy in Single-Stage Nipple-Sparing Mastectomy with Direct-to-Implant Breast Reconstruction.

BACKGROUND: Despite advances in skin envelope reduction techniques and experienced nipple-sparing mastectomy flap procedures, the rate of nipple malposition and secondary revision in these patients remains high and eligible candidates are limited. In this article, the authors present a novel technique combining skin reduction nipple-sparing mastectomy surgery with single-stage skin-only mastopexy and direct-to-implant reconstruction. METHODS: A retrospective review was performed at a single institution from 2015 to 2018. All patients were operated on using this technique consecutively, by a b

[Vaginal hysterectomy and bilateral adnexectomy for female to male transsexuals in an interdisciplinary concept].

BACKGROUND: FtM reassignment surgeries are rare and require interdisciplinary cooperation of plastic surgeons and gynaecologists. There are only few homogeneous data and standardised recommendations about the operative access to hysterectomy and bilateral adnexectomy of FtM transsexual patients. PATIENTS AND METHODS: Between 2006 and 2009 106 FtM transsexuals were hysterectomised in an interdisciplinary concept of plastic surgeons and gynaecologists in the Frauenklinik des Rotkreuzklinikums München. Firstly plastic surgeons performed a complete colpectomy, after that a vaginal hysterectomy wi

[Surgical treatment of gynecomastia].

BACKGROUND: Gynecomastia describes a benign increase of the mammary gland in men. When medical treatment fails, the surgical procedure is the treatment of choice. There are two main surgical procedures: subdermal mastectomy and liposuction. AIM: To evaluate the results after surgical treatment (mastectomy) performed in a general surgery unit. METHOD: We performed a retrospective study; all the patients operated for gynecomastia were reviewed. The clinical, imaging, biological, intraoperative and histological data were included into a MS Access database and statistical analyzed. RESULTS: Fro

Comparing the Use of Transverse Abdominis Plane Block and Splash Block for Postoperative Pain Control in Dogs Undergoing Mastectomy-A Blinded Randomized Prospective Clinical Study.

To compare the use of postoperative analgesia for mastectomy, 44 dogs were randomly allocated to either the Splash treatment group (group A) or the Transverse Abdominis Plane block treatment group (TAP, group B). Following intramuscular (IM) premedication with pethidine (4 mg kg-1) and acepromazine (0.01 mg kg-1), anesthesia was induced with intravenous (IV) propofol and maintained with isoflurane by an anesthetist (DC) who was unaware of the treatment. In group A, ropivacaine 0.5% (2 mg kg-1) was administered prior to surgical wound closure. In group B, ropivacaine 0.5% (0.8-1 mg kg-1 per poi

Poland syndrome combined with breast cancer: a case report.

BACKGROUND: Poland syndrome is an occasional congenital malformation characterized by unilateral chest wall dysplasia and ipsilateral upper limb abnormalities. An association between Poland syndrome and breast cancer has been reported, but no clear etiological link between Poland syndrome and breast tumors has been established. We report a case of Poland syndrome combined with breast cancer and analyzed the clinical features of breast cancer in this case and its influence on the choice of treatment for breast cancer. CASE DESCRIPTION: In February 2022, we admitted a 47-year-old woman with Pol

[Laparoscopic Abdominoperineal Resection of the Rectum in a Case with Pagetoid Spread of Anal Canal Carcinoma].

The patient, a man in his 60s, first noticed an elevated lesion around the anus 3 years ago. The lesion failed to subside with the topical drug preparations prescribed at a local clinic, and the patient was referred to the Department of Dermatology of our hospital for further workup and treatment. The findings of biopsy from the lesion suggested skin infiltration of anal canal carcinoma, and the patient was referred to the Department of Surgery. Examination here revealed only induration of the anal canal, with no abnormality of the rectal mucosa even when the endoscope was reversed to visualiz

Prophylactic Mastectomy: Postoperative Skin Flap Thickness Evaluated by MRT, Ultrasound and Clinical Examination.

BACKGROUND: Women with an increased hereditary risk of breast cancer can undergo prophylactic mastectomy (PM), which provides a significant, but not total, risk reduction. There is an ongoing discussion about how much skin and subcutaneous tissue should be resected to perform an adequate PM while leaving viable skin flaps. METHODS: Forty-five women who had undergone PM were examined with magnetic resonance tomography (MRT), ultrasound (US) and clinical examination (CE) by a plastic surgeon and a general surgeon to estimate skin flap thickness. RESULTS: The estimated mean skin flap thickness

Pyoderma Gangrenosum after Fat Grafting in Alloplastic Breast Reconstruction: An Unusual Outcome.

Pyoderma gangrenosum (PG) is a rare and painful inflammatory skin disorder that has been recently associated with breast surgery. It is commonly mistaken for postoperative ischemia or wound infection and does not show response to antibiotics or debridement. We describe the first case of post-surgical PG (PSPG) after alloplastic breast reconstruction involving fat grafting. A 47-year-old woman underwent bilateral mastectomy and 2-stage alloplastic breast reconstruction, with fat grafting from the abdomen. Two days post-surgery, she developed bilateral erythema with tender grouped pustules that

Treatment of postmastectomy lymphedema by bypassing the armpit with implanted silicone tubings.

BACKGROUND: Women treated for breast cancer are facing a life-time risk of developing lymphedema in up to 40% of this population. In advanced cases of lymphedema main lymphatics are obstructed and tissue fluid accumulates in the interstitial spaces forming fluid "lakes"and "channels". The only solution for fluid drainage would be creating artificial channel for flow away to the non-obstructed regions. The aim of this study was to form artificial pathways for edema fluid flow by subcutaneous implantation of silicone tubes into the swollen limb. METHODS: Implantation was carried out in ten pati

Necrotizing fasciitis of the breast with extension to the chest wall myofascial layer in a lactating mother: a rare case report.

INTRODUCTION AND IMPORTANCE: Necrotizing fasciitis is one of the deadliest forms of infection to affect human kind. It usually happens in the presence of risk factors and often involves the trunk, the limbs and the perineum. Primary breast involvement is quite rare. Early diagnosis and management is of paramount importance to decrease morbidity and death. CLINICAL PRESENTATION: A 35 years old female patient presented to our facility with progressive left breast swelling and pain of 05 days. She also had discoloration of skin, anorexia and high grade fever. She was lactating mother to a 1 year

Correction of gynecomastia in body builders and patients with good physique.

BACKGROUND: Temporary gynecomastia in the form of breast buds is a common finding in young male subjects. In adults, permanent gynecomastia is an aesthetic impairment that may result in interest in surgical correction. Gynecomastia in body builders creates an even greater distress for patients seeking surgical treatment because of the demands of professional competition. The authors present their experience with gynecomastia in body builders as the largest study of such a group in the literature. METHODS: Between the years 1980 and 2013, 1574 body builders were treated surgically for gynecoma

Sentinel lymph node biopsy in multicentric breast cancer. The experience of the European Institute of Oncology.

AIM: To present our experience with sentinel lymph node biopsy (SLNB) performed in patients with multicentric breast cancer. METHODS: Between May 2001 and May 2004, 42 patients with multicentric breast cancer and a clinically negative axilla underwent lymphatic mapping either by a single subareolar (n = 25) or a double peritumoral/subdermal injection (n = 17) of 99Tc-HSA nanocolloids. The sentinel lymph node (SLN) was evaluated by intraoperative frozen section and axillary dissection was performed only in case of positive SLN. RESULTS: Mean age was 49 years (range 25-78). Mean number of SLNs

One-Stage Gender-Confirmation Surgery as a Viable Surgical Procedure for Female-to-Male Transsexuals.

BACKGROUND: Female-to-male gender-confirmation surgery (GCS) includes removal of breasts and female genitalia and complete genital and urethral reconstruction. With a multidisciplinary approach, these procedures can be performed in one stage, avoiding multistage operations. AIM: To present our results of one-stage sex-reassignment surgery in female-to-male transsexuals and to emphasize the advantages of single-stage over multistage surgery. METHODS: During a period of 9 years (2007-2016), 473 patients (mean age = 31.5 years) underwent metoidioplasty. Of these, 137 (29%) underwent simultaneou

Mastectomy Skin Flap Necrosis after Implant-Based Breast Reconstruction: Intraoperative Predictors and Indocyanine Green Angiography.

BACKGROUND: Nipple-sparing mastectomy (NSM) carries significant risk of mastectomy skin flap necrosis, which can compromise oncologic, surgical, and quality-of-life outcomes. Prospective data on intraoperative predictors of mastectomy skin flap necrosis could help to mitigate this risk. METHODS: This was a single-institution prospective trial of patients undergoing NSM implant-based reconstruction for breast cancer or prophylaxis (2021 to 2024). Flap perfusion was evaluated using clinical assessment, and indocyanine green (ICG) angiography perfusion values and angiogram patterns. Ischemic com

Case report on investigational modular cardiac rhythm management system: wireless communication between leadless pacemaker and subcutaneous defibrillator in cardiac sarcoidosis with limited vascular access.

BACKGROUND: Cardiac sarcoidosis (CS) is a granulomatous inflammatory disorder that can result in high-grade conduction disease and ventricular arrhythmias, often necessitating permanent pacing and defibrillator support. The transvenous system is particularly challenging in CS with immunosuppression, limited vascular access, and a history of chemotherapy. Traditional transvenous systems may be poorly suited for this immunocompromised population. CASE SUMMARY: A 52-year-old woman with a history of breast cancer treated with neoadjuvant chemotherapy and biopsy-confirmed systemic sarcoidosis who
