Recurrent cochlear implant infection treated with exteriorization and partial mastoid obliteration.

OBJECTIVE AND IMPORTANCE: Preoperative chronic otitis media (COM) is a risk factor for postoperative infection after cochlear implantation (CI), but its management varies by surgeon. Our case highlights a strategy for implant preservation in a patient with a history of recurrent cochlear implant infection. CLINICAL PRESENTATION: A 70-year-old woman with a history of chronic lymphocytic leukemia presented in 2005 with bilateral COM and sensorineural hearing loss meeting CI candidacy. Four months after left mastoid obliteration with abdominal fat graft and external auditory canal closure, a lef…

Mastoid trepanation in a deceased from medieval Croatia: a case report.

We present a rare case of infratentorial mastoid trepanation, by drilling, from medieval Croatia. An artificial ante-mortal opening was found in a male skeleton from the 11th century cemetery Zvonimirovo. It was placed roughly at the intersection of the Frankfurt's plane and the midline of the right mastoid. The right posterior parietal of the deceased also exhibited a callus-like formation consistent with the linear cranial fracture. Our aim was to investigate by computed tomography (CT) a possible presence of otopathology--a chronic middle ear infection--MEI/mastoiditis or cholesteatoma. On …

Mastoid emissary vein: anatomy and clinical relevance in plastic & reconstructive surgery.

The mastoid emissary vein (MEV) is an anatomical structure with limited description in the literature and its importance is even less recognized in the plastic surgical field. Investigations in its anatomy and physiology have described its anthropological significance in transition to bipedalism and preferential intracranial venous flow into the vertebral plexus in the upright man. Inadvertant injury of vessels of this size pose a significant problem due not only to difficulty with haemostasis but also from their bidirectional flow and close proximity to the sigmoid sinus where cases of thromb…

Squamous cell carcinoma of the mastoid - a report of two cases.

SummaryMalignant tumours of the mastoid are rare, the majority being squamous cell carcinomas. We report two cases whose clinical presentation mimicked mastoid abscess with intracranial complications. The first case is a twenty year Nigerian lady who presented to the Emergency Room of the Otorhinolaryngology Department with a one month history of headache, low grade fever, left facial palsy, neck stiffness and left post-auricular swelling on a background of left chronic suppurative otitis media since childhood. An initial diagnosis of meningitis and mastoid abscess secondary to chronic suppura…

Sigmoid and transverse sinus thrombosis caused by mastoid implant -- case report.

INTRODUCTION: Extraoral craniofacial implants are often used for retention of auricular prostheses. In this case report, the rare complication of sigmoid and transverse sinus thrombosis after implant placement in the mastoid is described. CASE REPORT: A 36-year-old man with congenital hypoplasia of the right ear was treated with an implant-based auricular prosthesis. One of the three implants in the mastoid area had to be removed because of an infection, whereas the other two implants were removed at the patient's request. Twenty-four days after implant removal, he was seen with sigmoid and t…

Posterior canal wall reconstruction tympanoplasty for operated ears with open mastoid.

We investigated the postoperative conditions of ears and the hearing acuity of patients who underwent posterior canal wall reconstruction tympanoplasty for operated ears with open mastoid. The study included 118 ears of 105 patients (57 males, 48 females; age range 5-75 years) and the patients were followed for > 1 year after the final operation. Although 114 ears (97%) were dry after the final operation, retraction pockets and eardrums in a lateral position were observed in 5 ears and in 1 ear, respectively. Ossicular reconstruction was performed in 83 ears and in these the mean air conductio…

Effects of endolymphatic mastoid shunt operation for patients with Menière's disease.

During the past 10 years, the endolymphatic mastoid shunt operation was carried out on 108 patients with Meniere's disease: 54 men and 54 women between 22 and 72 years old. According to criteria AAOO proposed in 1972, 86 cases (79.6%) belonged to class A, 19 cases (17.7%) to class B and 3 cases (2.7%) to class C. Forty patients took the body sway test before and after the operation. Four of the 40 patients were found to have Meniere's disease on the contralateral side within 12 months after the operation and one patient was found to have a complicating psychogenic disease. The abnormal body sw…

[The mastoid cavity and auditory canal meatoplasty. Relations between corresponding indications].

The indications for conservation of the posterior wall of the auditory canal versus mastoid cavity are described. For a dry cavity the relation of volume of cavity to entrance of meatus is of considerable importance. The necessity of meatoplasty is pointed out. Classical techniques described 100 years ago can still be used today with only slight modifications.

High risk and low prevalence diseases: Acute mastoiditis.

INTRODUCTION: Acute mastoiditis is a serious condition that carries with it a high rate of morbidity and mortality. OBJECTIVE: This review highlights the pearls and pitfalls of mastoiditis, including the presentation, diagnosis, and management in the emergency department (ED) based on current evidence. DISCUSSION: Acute mastoiditis most commonly affects pediatric patients and is a suppurative infection of the mastoid air cells. It is often associated with otitis media, and common bacteria include Streptococcus and Staphylococcus. History and examination may reveal tympanic membrane erythema,…

Clarifying the Diagnosis and Management of Acute Uncomplicated Pediatric Mastoiditis.

INTRODUCTION: Acute pediatric mastoiditis is a bacterial infection of the mastoid bone most commonly associated with acute otitis media. Complicated mastoiditis is traditionally characterized by intracranial complications or subperiosteal abscess, but definitions are inconsistent in the literature. Surgical intervention is identified as the main treatment for complicated mastoiditis, but there is some evidence to support medical management of uncomplicated mastoiditis. This study sought to clarify the diagnostic criteria and management of uncomplicated acute mastoiditis. METHODS: All cases of…

Complications in Pediatric Acute Mastoiditis: HCUP KID Analysis.

OBJECTIVE: A small proportion of children with otitis media develop acute mastoiditis, which has the potential to spread intracranially and result in significant morbidity and mortality. The aim of this study was to evaluate the incidence and management of complications related to pediatric acute mastoiditis using a national database. STUDY DESIGN: Retrospective review of 2016 Kids' Inpatient Database, part of the Healthcare Cost and Utilization Project. SETTING: Academic, community, general, and pediatric specialty hospitals in the United States. METHODS: International Classification of Di…

Cerebral Sinus Venous Thrombosis in the Setting of Acute Mastoiditis.

Cerebral sinus venous thrombosis (CSVT) is a rare complication of acute mastoiditis with a declining incidence in the post-antibiotic era. In the adult population its incidence ranges from three to four cases per million. Here we present a case of a 47-year-old female with triple negative breast cancer on chemotherapy who underwent a molar tooth extraction, which was followed two weeks later by the sudden onset of left-sided frontotemporal headache radiating down the face, left ear fullness with associated hearing loss, toothache, and left orbital pain. Imaging studies performed included magne…

Non-Hodgkin's Lymphoma of the Middle Ear Presenting as Mastoiditis.

Lymphoma originating from the middle ear is rare. The diagnosis of lymphoma as with other cancers of the temporal bone is often made late, and this has a negative implication on the treatment and prognosis of the condition. The delay of diagnosis is mainly due to the similar presentation shared with other benign conditions of the middle ear. We present a case of a 62-year-old man who was treated as a case of chronic otitis media for a period of time before presenting with advanced symptoms; a final diagnosis of lymphoma of the middle ear was given. Other similar cases in the literature are dis
