[A toddler with complicated otorrhea].

A 4-year-old boy was diagnosed with intracranial abscess following adequately treated otorrhea and mastoiditis. This is an uncommon finding. He underwent mastoidectomy with drainage of the abscess. In cases where patients clinically deteriorate during treatment for otorrhea and mastoiditis, further examination using CT-scan is recommended.

Trainee-Reported Outcomes of Otology Simulation Training: Examining Effectiveness of the VOXEL-MAN Temporal Bone Simulator as a Training Adjunct in Mastoid Surgery.

INTRODUCTION: Mastoid surgery is technically challenging with complex three-dimensional (3D) anatomy. With reduced access to traditional training methods and increasing new technologies available, virtual reality simulation is being evaluated in a regional training program as an adjunct to existing training structures. METHODS: Twenty higher surgical trainees were instructed to complete a cortical mastoidectomy and posterior tympanotomy on a VOXEL-MAN simulator (VOXEL-MAN, Hamburg, Germany). A pre- and post-session questionnaire was completed. RESULTS: The visual analogue score for familiari…

A Rare Case of Refractory Mycobacteroides Abscessus Otomastoiditis in an Immunocompetent Child: A Multidisciplinary Treatment Approach.

This case report details a rare and severe presentation of refractory otomastoiditis caused by Mycobacterium abscessus in an immunocompetent pediatric patient who presented with a 1-month history of left ear pain, swelling, and fever unresponsive to antibiotics. Computed tomography imaging was suggestive of coalescent otomastoiditis, and she underwent urgent left mastoidectomy, subperiosteal abscess drainage, and myringotomy with tube insertion, followed by conventional antibiotic management. Despite these interventions, she remained symptomatic and cultures revealed M. abscessus 1 week later.…

[Research of modified skin flap of external canal in auditory canal shaping to enlarge].

OBJECTIVE: To investigate the new method of modified skin flap of external canal in auditory canal shaping to enlarge at radical mastoidectomy in endaural incision. METHOD: From 1999 to 2009, 412 patients with 428 ears in operation were randomly divided into two groups. Patients underwent open mastoidectomy and some patients were performed concurrent tympanoplasty. A group (control group)were operated in traditional skin flap of external canal, B group is modified group. Bleed and effusion from the wound surface, the time of dry ear and the change of diameter of extermal acoustic meatus after…

[Experience in overlay tympanoplasty].

OBJECTIVE: To explore the experience and value of overlay tympanoplasty. METHODS: Sixty-three ears with overlay tympanoplasty were reviewed and followed up for the external auditory canal, tympanic membrane and hearing. RESULTS: The diseases of the patients included middle ear cholesteatoma in 25 ears and chronic suppurative otitis media in 38 ears. The surgical techniques involved three kinds: overlay tympanoplasty, overlay tympanoplasty with canal wall up mastoidectomy and overlay tympanoplasty with canal wall down mastoidectomy. In middle ear cholesteatoma and suppurative otitis media pat…

Postmastoidectomy Hyperacusis Syndrome: Clinical Features and Treatment.

OBJECTIVE: We report a novel postmastoidectomy hyperacusis syndrome (PMHS) in patients who have had cortical mastoidectomies and experience hyperacusis to stimuli involving touch of the pinna and periauricular area. This report aims to describe the clinical characteristics of patients predisposed to this disabling complication after mastoid surgery and describes surgical treatment with mastoid cortex resurfacing with hydroxyapatite bone cement. PATIENTS: Three patients who have undergone intact canal wall mastoidectomies for nonchronic middle ear-related pathologies all reported a similar con…

Mastoid Obliteration With Freeze-Dried Bone Allograft in Canal Wall Down Tympanoplasty: Description of a Novel Technique and Case Report.

Mastoid obliteration can be performed after canal wall down (CWD) mastoidectomy with various materials. Homologous bone tissue harvested from cadaver donor represents a feasible option with advantages. The purpose of the study is to describe the case of a patient diagnosed with middle ear cholesteatoma treated with mastoidectomy of the CWD and mastoid obliteration with homologous freeze-dried corticocancellous bone particulate in the Cittadella Hospital Ear, Nose, Throat (ENT) unit. The preoperative characteristics of the patients, the procurement and processing of bone allografts, the surgica…

Right hemiparesis caused by massive otogenic brain abscess in children: Unusual case report and review of the literature.

INTRODUCTION: Hemiparesis caused by otogenic brain abscess in children is an unusual complication of Chronic Suppurative Otitis Media. Complications can occur when the disease is not treated properly so that the infection in the middle ear spreads intracranially. CASE PRESENTATION: We report a case of Chronic Suppurative Otitis Media with cholesteatoma in a 14-year-old boy with complications of right hemiparesis caused by an otogenic brain abscess. His management included open craniotomy, drainage of the abscess, radical mastoidectomy and intravenous antibiotics according to the result of the…

Experience in the management of sigmoid sinus thrombophlebitis secondary to middle ear cholesteatoma.

OBJECTIVE: To discuss the management of sigmoid sinus thrombophlebitis secondary to middle ear cholesteatoma. METHODS: We retrospectively analyzed all cases of sigmoid sinus thrombophlebitis caused by middle ear cholesteatoma over a period of 7 years. 7 male and 2 female patients, ranging in age from 9 to 66 years, were diagnosed with sigmoid sinus thrombophlebitis by clinical presentation and radiological examination. By executing a modified mastoidectomy and tympanoplasty (canal wall-down tympanoplasty) to entirely remove the cholesteatoma-like mastoid epithelium, all patients were effectiv…

Keyhole Mini-Craniotomy Middle Fossa Approach for Tegmen Repair: A Case Series and Technical Instruction.

Background and Importance  Tegmen defects associated with cerebrospinal fluid (CSF) leaks are a rare pathology that can result in severe complications if left untreated. There is no universal optimal surgical algorithm for repair, although the most common techniques are the middle fossa craniotomy (traditionally 25 cm 2 in area), the transmastoid approach, or both. Here, we describe successful use of a keyhole mini-craniotomy, only 6 cm 2 in area, without mastoidectomy or days of lumbar drainage. Clinical Presentation  Three patients presented with right-sided CSF otorrhea and hearing loss, wi…
