Complications Rate and a Multidimensional Analysis of Their Causes of Tube Thoracostomy: A Mixed-Methods Study.

INTRODUCTION: Tube thoracostomy (TT) complications are common in respiratory medicine. However, the prevalence of complications and risk factors is unknown, and data on countermeasures are lacking. METHODS: This was a mixed-methods retrospective observational and qualitative study. This retrospective observational study included TT performed on patients admitted to the Department of Respiratory Medicine at our University Hospital between January 1, 2019, and August 31, 2022 (n=169). The primary endpoint was the incidence of TT-related complications. We reviewed the association between complic

The application of the percutaneous suturing technique in thoracoscopic repair of congenital diaphragmatic hernia.

BACKGROUND: Thoracoscopic surgery of congenital diaphragmatic hernia (CDH) is connected with a higher incidence of recurrence than open repair is. This is usually caused by the dehiscence of sutures in the lateral part of the defect. This area is characterized by increased tension on proximate tissues and difficult thoracoscopic suturing. For more effective repair, the authors adopted a variant of percutaneous internal ring suturing (PIRS) technique. OBJECTIVES: To present and evaluate the efficacy of the PIRS technique for the repair of CDH. MATERIAL AND METHODS: The study is based on retro

Hospitalizations associated with pandemic influenza A (H1N1) 2009 in asthmatic children in Japan.

BACKGROUND: The pandemic influenza A (H1N1) 2009 [pdm (H1N1) 2009] spread through the world in 2009, producing a serious epidemic in Japan. Since it was suggested early that asthma is a risk factor for an increased severity of the infection, the Japanese Society of Pediatric Allergy and Clinical Immunology (JSPACI) organized a working group for countermeasures, and investigated asthmatic children admitted to the hospitals for pdm (H1N1) 2009 infection. METHODS: An appeal was made on the home page of the JSPACI to medical practitioners to input clinical information about asthmatic and non-asth

Reliability and validity of maternal recall of injuries in Pacific children: findings from the Pacific Islands Families Study.

BACKGROUND: Childhood injury is one of the leading causes of morbidity and mortality worldwide. Many epidemiological studies use self-report injury data to understand the pattern of injury, and to identify risk factors and potential injury countermeasures, but the veracity of these data is not without question. AIMS: To investigate the reliability and validity of the use of maternal recall of childhood injuries in a birth cohort study of Pacific mothers residing in New Zealand. METHODS: As part of the Pacific Islands Families Study (PIFS), this study included 1,354 Pacific children born in A

Using forensic autopsy data to estimate the age-specific infection fatality risk of COVID-19.

The infection fatality risk indicates the probability of death among infected individuals. The age-dependent heterogeneity of infection fatality risk is crucial for severity assessment and prioritization of countermeasures. However, infection fatality risk estimation requires infection data from a large-scale seroepidemiological survey combined with either direct ascertainment of deaths caused by infection or excess mortality estimates. To overcome the difficulty in ascertaining death, we propose an alternative approach to estimating the age-specific infection fatality risk for SARS-CoV-2 usin

Pathogenesis and natural history of the Bundibugyo species of Orthoebolavirus in nonhuman primates.

The current outbreak of Bundibugyo virus (BDBV) in Africa is a global public health concern particularly as there are no licensed medical countermeasures (MCM). Well characterized animal models that accurately replicate human BDBV infection are needed to develop effective MCM. We exposed 21 cynomolgus monkeys (CM) to BDBV to examine the progression and natural history of BDBV disease (BVD). BVD was more protracted than reported for Ebola and Sudan infection in CM with a lower lethality rate of 67% consistent with lower human BVD mortality rates. IHC and spatial proteomics identified CD209+, CD…

Exploration of refined humane endpoints for melioidosis in BALB/c mice.

The development of humane endpoints is critical for refining scientific studies involving animals. Body weight and clinical signs of disease data collected in four recent studies assessing medical countermeasures for utility against the disease melioidosis in mice were further analyzed. Here we used this information to ascertain whether a suitable alternative humane endpoint could be identified. A total of 66 possible alternative humane endpoints were explored, which varied the threshold values of the 'percentage body weight loss post-challenge' and 'the clinical signs over time' following ces…

Organoids in respiratory virus research: advances and perspectives.

The pandemics of respiratory viruses pose a worldwide public health problem and bio-safety threat. Therefore, the development of high-throughput and accurate infection models is crucial for elucidating viral pathogenesis and accelerating countermeasures to address the evolving respiratory viruses and the unexpected outbreaks of emerging variants. Compared to traditional 2D cultures, organoids exhibit pronounced intercellular interactions, extracellular matrix signaling, and tissue-specific multicellular cooperation, thereby more accurately recapitulating the in vivo microphysiological environm

SARS-CoV-2 Entry Can Be Mimicked in C. elegans Expressing Human ACE2: A New Tool for Pharmacological Studies.

Testing medical countermeasures for SARS-CoV-2 transmission using vertebrates can be hindered by legislation regulating animal experimentation, high costs, and ethical concerns. To overcome these challenges, we propose a new Caenorhabditis elegans strain that constitutively expresses the human angiotensin-converting enzyme 2 receptor (ACE2). This resulted in significant impairment of reproduction and a defect in pharyngeal function compared to wild-type (WT) worms. SARS-CoV-2 infection was simulated by treating worms with the receptor-binding domain (RBD) of the spike protein, which caused dos…

Towards an HCV vaccine: an overview of the immunization strategies for eliciting an effective B-cell response.

INTRODUCTION: Fifty-eight million people worldwide are chronically infected with hepatitis C virus (HCV) and are at risk of developing cirrhosis and hepatocellular carcinoma (HCC). Direct-acting antivirals are highly effective; however, they are burdened by high costs and the unchanged risk of HCC and reinfection, making prophylactic countermeasures an urgent medical need. HCV high genetic diversity is one of the main obstacles to vaccine development. The protective role of the humoral response directed against the HCV E2 glycoprotein is well established, and broadly neutralizing antibodies pl

Reducing the threats of rodent-borne zoonoses requires an understanding and leveraging of three key pillars: disease ecology, synanthropy, and rodentation.

Rodents have co-existed with humans for centuries, and frequently exchange pathogens. Historically, rodent-driven plague outbreaks scoured the Old World, resulting in substantial human mortality. Although such pandemics have not occurred for centuries, serious threats from rodent-borne infections, such as the global emergence of mpox, still exist. Moreover, endemic and emerging rodent infections continue to cause substantial human morbidity and mortality in low-income and middle-income countries. Efforts by the medical community to control rodent-borne zoonoses primarily focus on treating or p

Next-Generation Vaccines Against COVID-19 Variants: Beyond the Spike Protein.

Vaccines are among the most effective medical countermeasures against infectious diseases. The current Coronavirus disease 2019 (COVID-19) pandemic, caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), has spurred the scientific strategies to fight against the disease. Since 2020, a great number of vaccines based on different platforms have been in development in response to the pandemic, among which mRNA, adenoviral vector, and subunit vaccines have been clinically approved for use in humans. These first-generation COVID-19 vaccines largely target the viral spike (S) protei…

Toxicokinetics of plasmatic VX in a swine model: comparison of a simple enzymatic titration method with a mass spectrometry method.

Recent events have shown that organophosphorus nerve agents (OPNAs) are a serious threat. Cholinesterase inhibition by OPNAs results in acetylcholine accumulation, a cholinergic crisis leading to death if untreated. Efficacy assessment of new medical countermeasures against OPNAs relies on translational animal models. We developed a swine model of percutaneous VX intoxication and a simple plate reader-based enzymatic method to quantify plasmatic VX over time. Juvenile pigs anesthetized with sevoflurane were poisoned with a single supralethal (n = 5; 1200 μg/kg) or sublethal (n = 6; 320 μg/kg) …

How spaceflight challenges human cardiovascular health.

The harsh environmental conditions in space, particularly weightlessness and radiation exposure, can negatively affect cardiovascular function and structure. In the future, preventive cardiology will be crucial in enabling safe space travel. Indeed, future space missions destined to the Moon and from there to Mars will create new challenges to cardiovascular health while limiting medical management. Moreover, commercial spaceflight evolves rapidly such that older persons with cardiovascular risk factors will be exposed to space conditions. This review provides an overview on studies conducted
