"There's more to recovery than just weight gain": Ethical challenges for justice and respect in adjudication of capacity for patients with anorexia nervosa.

Anorexia nervosa (AN) is a complex psychiatric illness with a high mortality rate. When patients with AN refuse medical treatment, it may be ethically justifiable to provide treatment in their best interests if they are found incapable to make the decision themselves. In Ontario, Canada, any person deemed incapable has the right to challenge that decision at the Consent and Capacity Board (CCB), an independent tribunal that adjudicates hearings under six Acts, including the Health Care Consent Act. Recognising both the complexity of capacity assessment in the context of AN and widespread stigm

Addressing the false dichotomy between autonomy and preservation of life: Clinical, legal, and ethical considerations in severe and longstanding anorexia nervosa.

Anorexia nervosa (AN) is a serious metabo-psychiatric disorder. Despite a mortality rate approximately five times higher than that of age-matched controls, most deaths result from delayed recognition, fragmented care, and limited access to integrated, evidence-based treatment. Refusal of life-sustaining nutrition in severe or longstanding AN presents profound ethical and legal dilemmas, challenging the balance between autonomy, protection, and the duty to preserve life. This paper provides a critical narrative and normative review of how clinical uncertainty, systemic failure, and legal interp

Gendered denials: Vulnerability created by barriers to legal capacity for women and disabled women.

This article explores the vulnerability that is created when legal capacity is denied to women and disabled women. It argues that vulnerability is largely contingent on social constructs - as opposed to being an inherent quality of disability or gender. It discusses barriers to the exercise of legal capacity that women and disabled women experience - such as limitations on reproductive choice, higher rates of substituted decision-making, and unique experiences with forced mental health treatment. It then explores evidence that such barriers are disempowering and can leave women and disabled wo

Effect of a Website That Presents Patients' Experiences on Self-Efficacy and Patient Competence of Colorectal Cancer Patients: Web-Based Randomized Controlled Trial.

BACKGROUND: Patients often seek other patients' experiences with the disease. The Internet provides a wide range of opportunities to share and learn about other people's health and illness experiences via blogs or patient-initiated online discussion groups. There also exists a range of medical information devices that include experiential patient information. However, there are serious concerns about the use of such experiential information because narratives of others may be powerful and pervasive tools that may hinder informed decision making. The international research network DIPEx (Databa

Memory for murder. A psychological perspective on dissociative amnesia in legal contexts.

There is currently a complex and inconsistent state in the law relating to dissociation and dissociative amnesia (McSherry, 1998). Although dissociative amnesia in defendants is relevant to both competency to stand trial and criminal responsibility in principle, courts have typically assumed a skeptical stance toward such claims in practice. However, there is considerable evidence from both nonoffender and offender populations to support the validity of dissociative amnesia in defendants. Further, there is information available to aid in the evaluation of amnesia, such as the quality of the re

Here it is everything at once: a qualitative study of district nurses' experiences caring for older individuals with multiple chronic health problems in primary care.

BACKGROUND: Over the next 10-year period in Sweden, the number of people aged 80 and older is will increase by about 50%. The care needs of the population are changing as more people live at home longer with chronic conditions. District nurses in primary care often have the first contact with patients of all ages and are especially involved in care of older individuals with multiple chronic conditions, common mental health problems, and complex care needs. Understanding how district nurses currently see their work with older people with complex multimorbidity is a key step in developing new mo

Higher-level competence: Results from the Integrated Longitudinal Studies on Aging in Japan (ILSA-J) on the shape of associations with impaired physical and cognitive functions.

AIM: This study aimed to examine the relationships between levels of competence and impaired physical and cognitive functions in older adults. METHODS: We used a data set of the Integrated Longitudinal Studies on Aging in Japan for 2017 including 5475 community-dwelling older adults. Levels of competence were assessed using the Japan Science and Technology Agency Index of Competence (JST-IC). Grip strength (low grip strength: <28 kg for men and <18 kg for women) and gait speed (slow gait speed: <1.0 m/s for both sexes) were evaluated as physical function measurements, and the Mini-Mental Stat

Factors Related to Perceived Stigma in Parents of Children and Adolescents in Outpatient Mental Healthcare.

Little is known about factors contributing to perceived stigma in parents of children and adolescents with behavioral and emotional problems in outpatient mental healthcare. We aimed to identify the most relevant factors related to perceived parental stigma using least absolute shrinkage and selection operator (LASSO) regression including a broad range of factors across six domains: (1) child characteristics, (2) characteristics of the primary parent, (3) parenting and family characteristics, (4) treatment-related characteristics, (5) sociodemographic characteristics, and (6) social-environmen

Medical assessment of fitness to dive. Part I.

Good physical and mental health is a prerequisite for anyone planning to scuba dive. A fitness to dive certificate for those willing to enter a scuba diving course as well as for active divers, either amateur or occupational, can only be issued if there are no medical contraindications to dive. It is usually within the competence of a diving instructor, a manager of underwater work or a physician to assess a person's mental and physical health and grant them permission to stay under hyperbaric conditions. The legal requirements for issuing a fitness to dive certificate are different for recrea

Basic Psychological Needs and Mental Health in Primary Care: A Self-Determination Theory Perspective.

Mental health concerns are increasingly encountered in primary care, yet clinical approaches often focus on symptom management rather than underlying psychological processes. Self-Determination Theory (SDT) proposes that psychological well-being depends on the satisfaction of three basic psychological needs-autonomy, competence, and relatedness-while their chronic frustration contributes to psychological distress. Despite extensive evidence linking these needs to mental health outcomes, their relevance in primary care remains underexplored. This single-center cross-sectional study examined ass

Effectively addressing mental health issues in permanency-focused child welfare practice.

The purpose of this paper is two-fold: to highlight to the professional community the ongoing and shifting mental health needs of children exiting the child welfare system into permanent plans, dismissed from public caseloads yet still much in need of services, and, to present a model of one agency's integrated, outpatient intervention with promising preliminary data. The model offers a specialized; effective approach to the field to address the needs of families built by adoption and relative caregiving. The increasing number of adoptions, and relative and near-kin placements, mandate that co

Deaf and hard-of-hearing children and adolescents' mental health, Quality of Life and communication.

Mental health problems and lower Quality of Life (QoL) are more common in deaf and hard-of-hearing - (D)HH - children than in typically hearing (TH) children. Communication has been repeatedly linked to both mental health and QoL. The aims of this study were to compare mental health and QoL between signing deaf and hard-of-hearing (DHH), hard-of-hearing (HH) and TH children and to study associations between mental health/QoL and severity of hearing loss and communication. 106 children and adolescents (mean age 11;8; SD = 3.42), 59 of them DHH and 47 HH, and their parents reported child mental

The mental health needs of elementary schoolchildren.

More than 500 elementary school personnel from a large Colorado school district provided perceptions of the unmet mental health needs of their students, the likely causes of the needs, and suggestions about solutions or programs to meet the needs. The district's definition of "mental health"--positive self-image, healthy interrelationships with peers and adults, and acquisition of school skills and competencies--guided the questionnaire design. Respondents perceived numerous unmet mental health needs, most involving about 15% of the student population. The most frequently perceived problems we

[Dissociation (conversion) - malingering - antisocial personality disorder: differential diagnostic reflection on the basis of a case study].

In this case report we refer to the big challenge of making a diagnosis in a deliberate malingering in the field of mental disorders. We specifically describe the difficulty regarding the differentiation between a conversion disorder and malingering of a serial delinquent. For such a person avoiding criminal persecution is one of the most frequent reason to deceitfully simulate a mental illness. In this field, symptoms of conversion disorders exceed the average; furthermore, a great number of organic-neurological illnesses may appear to be very similar to a conversion disorder or in many cases

Mental health, coping, and protective factors in mothers of children with 22q11.2 deletion syndrome.

Compared to the large body of maternal mental health research for other pediatric disorders, we know far less about the experience of mothers of children with 22q11DS. This study investigates the coping methods, protective factors, and mental health of this population. These findings might lead to better support for 22q11DS maternal mental health. An international sample of 71 mothers (M = 40.5 years) of children with 22q11DS (M = 9.2 years) was recruited and completed an online survey assessing maternal mental health (symptoms of depression, anxiety, traumatic stress, general stress, and alco

From Awareness to Action: Advancing Counselor Education Policy to Improve Early Detection of Eating Disorders Through Embodied Reflection.

Eating disorders are associated with substantial morbidity and mortality yet remain underidentified in general mental health practice, delaying intervention and worsening outcomes. Counselor preparation often provides limited eating disorder content within broader psychopathology coursework, leaving trainees underprepared to recognize subtle, culturally diverse, and nonstereotypical presentations. This conceptual paper draws on scholarship in counselor education, body image, embodied self-awareness, and eating disorder training to propose a four-domain framework intended to strengthen counselo

Gender-affirming healthcare for incarcerated transgender and gender diverse people: An international scoping review.

BACKGROUND: Transgender and gender diverse (TGD) people face unique challenges and have distinct needs while incarcerated. Gender-affirming healthcare improves mental health outcomes and supports gender transition. While in recent years, correctional institutions have begun to recognize and address the gender-affirming healthcare needs of TGD people, there is a lack of understanding and awareness of TGD people's experiences when accessing gender-affirming healthcare while incarcerated. AIMS/METHOD: We conducted a scoping review on international empirical research published from 2018 to 2024 o

Young Urban Adolescents' Activity Spaces, Close Peers, and the Risk of Cannabis Use: A Social-Spatial Longitudinal Analysis.

BACKGROUND: An understudied, yet important area of youth development research is the examination of how place affects critical psychosocial processes such as identity formation, problem solving, emotional regulation, and in particular with adolescents, belongingness, autonomy, social competency, and behavioral health. A growing spatially informed literature indicates that youth interact with meaningful places as environmental strategies, shaping developmental trajectories related to behavioral health. OBJECTIVES: The objective is to investigate the relationship between place preference and he
