An acute carpometacarpal (CMC) dislocation of the thumb is itself an extremely rare injury, but one associated with a fracture of the distal radius has never before been reported. In our case, the fracture of the distal radius was treated by closed reduction and immobilization with a plaster cast, whereas the dislocation of the CMC of the thumb was treated with closed reduction, percutaneous fixation with Kirschner wires, and immobilization with a plaster cast. At the patient's two-year follow-up examination, the distal radius fracture displayed successful functional and radiographic results,

A type-III trilateral external skeletal fixation device was used to repair comminuted fractures of metacarpal and tibial bones in 3 deer. Mean weight of the deer was 25 kg, and mean age was 4 months. Mean interval from application until removal of the device was 6.5 weeks. All deer had complete function of their previously fractured limbs and had minimal complications during healing. Several factors should be considered when choosing a method of repair for wildlife with fractured limbs. Economic constraints and demeanor of the animal may limit the number of options. Analysis of results of our

In 15 cases, a small L-shaped, cut-to-size, reversible plate was used either to stabilize metaphyseal and metaphyseal-epiphyseal fractures of metacarpal bones and first phalanxes or to correct malunions and non-unions. The material and operating technique are described and the advantages and disadvantages of this and other techniques are discussed.

The author reports on a 42-year-old male patient who presented in our clinic with swelling over the left wrist bones after a fall. Dorsal and palmar wrist movement was painful, yet nearly equal on both sides. The X-ray showed an isolated osseous tendon rupture of the extensor carpi radialis brevis muscle at the base of the third metacarpal. This finding was confirmed upon surgical exploration, and a traction band was applied. After osseous healing of the fragments, the metal was removed early due to irritative synovialitis. Thereafter, the patient's wrist mobility soon became free and undistur

Fifth metacarpal bone shortening (brachymetacarpia V) was recently described to be highly prevalent in children with familial short stature (FSS). To characterize the hand bones of FSS patients with and without brachymetacarpia V, the left hand bone age radiographs of 26 FSS children were reviewed. In 16/19 patients with clinical brachymetacarpia V radiographs revealed fifth metacarpal bone shortening with a gap of 2 mm or more between the distal end of the fifth metacarpal bone and a tangential line connecting the distal ends of the third and fourth metacarpal bones. Only one of 7 patients wi

Although a relatively rare entity, post-traumatic arthrosis of the carpometacarpal joint of the index finger can be a disabling problem. Its diagnosis must be differentiated from other sources of local pathology. A rationale to the diagnosis and treatment of this entity is presented, demonstrating the usefulness of test injections of xylocaine, an intra-articular injection of a steroid preparation when a relatively congruous joint exists, and arthrodesis of the joint when an incongruous joint is encountered.

Carpometacarpal dislocations occur in less than 1% of hand injuries. Since 1973, five cases were treated at Henry Ford Hospital. Three of the dislocations were unstable and required internal fixation. Of these dislocations only one needed open reduction. The other two dislocations were successfully treated by closed reduction and cast immobilization. Follow-up ranged from 2 to 13 months. Three patients were asymptomatic, and the other two, both of whose injuries had been treated by internal fixation, reported pain with activity.

A case of volar dislocation involving the four ulnar metacarpals treated by open reduction and fixation with Kirschner wires is reported. A review of the literature leads the authors to suggest several pathogenic hypotheses. The mechanism may be direct or indirect according to whether the trauma is sustained on the metacarpal heads or on the bases of the metacarpals. Emergency treatment by open reduction and fixation usually provides excellent results.

A modified osteocutaneous digital fillet flap was used to reconstruct the thumb of a fifty-three year old male with a severe gunshot wound to the dominant hand. An arthrodesis in the central portion of the bone graft sustained a fatigue fracture that progressed to union without treatment. The one stage procedure avoided the immobilization of a distant pedicle flap, revisional surgery of a complex free flap, and donor site morbidity.

A case of monostotic fibrous dysplasia of a long bone of the hand is reported. There has been only one similar case recorded, though it is not uncommon in other long bones. The diagnosis was proved by histology, and the tumour treated by subperiosteal excision. There has been no recurrence after 31 months, though in similar cases excision including the periosteum has been advised to reduce the likelihood of recurrence.

Bone density was studied in ten patients before and after release of an idiopathic carpal tunnel syndrome. A corticomedullary ratio was measured at the mid-point of the metacarpals and proximal phalanges of both hands. There was a significant increase in the mean bone density over the period of observation in the thumb and middle finger metacarpals and in the index, middle, and ring finger proximal phalanges of the operated hand. There was no significant increase in the density of either metacarpal or proximal phalanx on the non-operated side. Release of the carpal tunnel syndrome provides not
