Autoimmune rheumatic diseases in women with coronary microvascular dysfunction: a report from the Women's Ischemia Syndrome Evaluation-Coronary Vascular Dysfunction (WISE-CVD) project.

BACKGROUND: While autoimmune rheumatic diseases (ARDs) have been linked with coronary microvascular dysfunction (CMD), the relationship between ARD and CMD in women with signs and symptoms of ischemia and no obstructive arteries (INOCA) are not well described. We hypothesized that among women with CMD, those with ARD history have greater angina, functional limitations, and myocardial perfusion compromise compared to those without ARD history. METHODS: Women with INOCA and confirmed CMD by invasive coronary function testing were included from the Women's Ischemia Syndrome Evaluation-Coronary V

Allergic angina syndrome, allergic myocardial infarct or Kounis syndrome: insights on epidemiology, etiology, diagnosis and treatment. A case report by gadolinium anaphylaxis.

Kounis syndrome (KS) was first described in 1991 by Kounis and Zavras and is also known as allergic angina syndrome or allergy myocardial infarction. It is a rare, and frequently undiagnosed syndrome that is characterized by an anaphylactic reaction. Allergens cause massive degranulation of mast cells leading to coronary spasm, microvascular angina, and/or endothelial dysfunction with myocardial infarction. The annual incidence of severe, life-threatening anaphylaxis with circulatory symptoms is about 7.9-9.6 cases per 100,000 people. More than 300 cases of KS have been described after exposur

A Case Report on Diagnostic Approach of a Complex Clinical Scenario: Differentiation Between Coronary Artery Disease and Apical Hypertrophic Cardiomyopathy as a Cause of Recurrent Debilitating Angina.

Apical hypertrophic cardiomyopathy (ApHCM) is a clinical challenge when presenting with symptoms of angina plus shortness of breath. An appropriate diagnosis of concurrent coronary artery disease (CAD) is needed for proper diagnosis, risk stratification, and management. We present a case of a 64-year-old gentleman with a history of ApHCM and CAD with previous percutaneous intervention presenting with recurrent angina. A repeat coronary angiography showed no new obstructive lesions. A stress cardiac magnetic resonance imaging was performed, which showed perfusion defect in the apex with apical

Patients with angina and non-obstructive coronary artery disease undergoing comprehensive invasive coronary function testing: long-term clinical outcomes.

BACKGROUND AND AIMS: Comprehensive evaluation by invasive coronary function test (CFT) of patients with angina and non-obstructive coronary artery disease is recommended in current guidelines. The prognosis of patients with CFT-proven coronary vasomotor dysfunction (CVDys) is less well-known. METHODS: Patients that provided consent for the previously published NL-CFT registry who underwent clinically indicated invasive CFT between February 1st, 2019 and June 1st, 2024 were included. Patients with CVDys defined as an abnormal spasm provocation test and/or microvascular functional assessment, w

Coronary vasomotor dysfunction and anesthesia management in noncardiac surgery.

PURPOSE OF REVIEW: This review addresses an increasingly recognized but still underdiagnosed group of patients presenting for noncardiac surgery with angina with nonobstructive coronary arteries (ANOCA) driven by coronary vasomotor dysfunction (CVDys). It synthesizes current knowledge on the pathophysiology, clinical presentation, diagnosis, and treatment of CVDys - encompassing endothelial dysfunction, epicardial and microvascular spasm, and structural and functional coronary microvascular dysfunction - and provides anesthesiologists with phenotype‑guided recommendations for preoperative asse

Association between Diabetic Retinopathy and Left Ventricular Dysfunction in Diabetic Patients with Unstable Angina.

INTRODUCTION: Diabetes mellitus (DM) is associated with serious complications including macro- and microvascular problems such as diabetic retinopathy. Coronary involvement in diabetic patients is believed to be a consequence of microvascular complications. However, the available data are inconclusive and scarce. This study aimed to evaluate the probable association between diabetic retinopathy and left ventricular dysfunction in diabetic patients with unstable angina (UA). METHODS: In this cross-sectional study, 200 diabetic patients with UA (100 cases with diabetic retinopathy and 100 cases

Comprehensive Management of ANOCA, Part 1-Definition, Patient Population, and Diagnosis: JACC State-of-the-Art Review.

Angina with nonobstructive coronary arteries (ANOCA) is increasingly recognized and may affect nearly one-half of patients undergoing invasive coronary angiography for suspected ischemic heart disease. This working diagnosis encompasses coronary microvascular dysfunction, microvascular and epicardial spasm, myocardial bridging, and other occult coronary abnormalities. Patients with ANOCA often face a high burden of symptoms and may experience repeated presentations to multiple medical providers before receiving a diagnosis. Given the challenges of establishing a diagnosis, patients with ANOCA

Assessment of 25-OH vitamin D levels and abnormal blood pressure response in female patients with cardiac syndrome X.

OBJECTIVE: Vitamin D deficiency is associated with coronary artery disease, hypertension, heart failure, endothelial dysfunction, and metabolic syndrome. The pathophysiology of cardiac syndrome X (CSX) involves many pathways that are influenced by vitamin D levels. This study aimed to investigate the relationship between vitamin D deficiency and abnormal blood pressure response to exercise in patients with CSX. METHODS: This was a cross-sectional and observational study. Fifty females with normal epicardial coronary arteries who presented with typical symptoms of rest or effort angina and 41

The incremental diagnostic value of microvascular resistance reserve in the assessment of coronary microvascular dysfunction.

BACKGROUND: Patients with coronary microvascular dysfunction (CMD) are at increased risk of major adverse cardiovascular events. The index of microcirculatory resistance (IMR) is more specific than coronary flow reserve (CFR) for CMD diagnosis, but the microvascular resistance reserve (MRR) shows potential due to its hemodynamic adjustments. Our study evaluated the diagnostic utility of IMR, MRR, and their combination in CMD diagnosis for patients experiencing angina and non-obstructive coronary arteries (ANOCA). METHODS: This observational study analyzed data from the Coronary Microvascular

Absolute coronary blood flow across different endotypes of ANOCA.

BACKGROUND: Intracoronary continuous thermodilution is a novel technique to quantify absolute true coronary flow and microvascular resistance. However, few data are available in patients with angina with non-obstructive coronary arteries (ANOCA). AIMS: This study aimed to investigate the diagnostic potential of hyperaemic absolute coronary flow (Qmax) and absolute microvascular resistance (Rμ,hyper) among different ANOCA endotypes, and to determine the correlation between continuous - and bolus - thermodilution indexes. METHODS: A total of 222 patients were scheduled for clinically indicated

The long and winding road to diagnosis: diagnostic delays and healthcare utilization in patients with CFT-confirmed INOCA.

BACKGROUND: Patients with Ischemia and No Obstructive Coronary Arteries (INOCA) experience recurrent angina, report reduced quality of life, and are at increased risk for major adverse cardiovascular events. Unfortunately, INOCA is often unrecognized and can be difficult to diagnose. METHODS: We included patients with angina and INOCA diagnosed by invasive coronary function testing (CFT) and characterized their interactions with the healthcare system. Data on initial symptom onset, frequency and type of healthcare encounters, specialty referrals, diagnostic testing (including non-invasive and

Microvascular resistance reserve: impact on health status and myocardial perfusion after revascularization in chronic coronary syndrome.

BACKGROUND AND AIMS: The microvascular resistance reserve (MRR) is a novel invasive index of the microcirculation, which is independent of epicardial stenoses, and has both diagnostic and prognostic implications. This study investigates whether MRR is associated with health status outcomes by revascularization in patients with moderate coronary stenoses. METHODS: Consecutive patients with stable chest pain and moderate (30%-90% diameter) stenoses on invasive coronary angiography (n = 222) underwent invasive physiology assessment. Revascularization was performed according to guideline recommen

Microcirculation in Hypertension: A Therapeutic Target to Prevent Cardiovascular Disease?

Arterial hypertension is a common condition worldwide and an important risk factor for cardio- and cerebrovascular events, renal diseases, as well as microvascular eye diseases. Established hypertension leads to the chronic vasoconstriction of small arteries as well as to a decreased lumen diameter and the thickening of the arterial media or wall with a consequent increased media-to-lumen ratio (MLR) or wall-to-lumen ratio (WLR). This process, defined as vascular remodeling, was firstly demonstrated in small resistance arteries isolated from subcutaneous biopsies and measured by micromyography

The possible role of PCSK9 inhibitors in cardiac syndrome X: Bridging microvascular dysfunction, endothelial pathobiology, and molecular pharmacotherapy.

Cardiac syndrome X (CSX), historically used to describe angina with normal coronary angiography and now largely encompassed within microvascular angina and INOCA, is an ischemic disorder characterized by angina pectoris without narrowing of coronary arteries. CSX, which primarily affects postmenopausal women, is driven by coronary micro-vascular dysfunction, endothelial impairment, oxidative stress, and chronic low-grade inflammation. Notably, the Conventional anti-anginal therapies often yield limited symptom relief, underscoring the need for novel mechanism-based approaches. Proprotein Conve

Microvascular spasm in non-ST-segment elevation myocardial infarction without culprit lesion (MINOCA).

BACKGROUND: Up to 30% of patients with acute coronary syndrome have no culprit lesion. Coronary microvascular spasm is an alternative cause for such a clinical presentation. However, this has rarely been investigated systematically. The aim of our study was to assess the frequency of coronary microvascular spasm in patients with NSTEMI without culprit lesion (MINOCA) by intracoronary acetylcholine testing (ACH-test). METHODS: Between 2014 and 2017, 940 patients with NSTEMI underwent coronary angiography and 125 (13%) had no culprit lesion (< 50% stenosis on visual assessment). Of the latter,

Estimating Quality of Life Decrements Due to Diabetes Complications in the United States: The Health Utility Index (HUI) Diabetes Complication Equation.

OBJECTIVE: Health utility decrements associated with diabetes mellitus complications are essential for calculating quality-adjusted life-years (QALYs) in patients for use in economic evaluation of diabetes interventions. Previous studies mostly focused on assessing the impact of complications on health utility at event year based on cross-sectional data. This study aimed to separately estimate health utility decrements associated with current and previous diabetes complications. RESEARCH DESIGN AND METHODS: The Health Utilities Index Mark 3 (HUI-3) was used to measure heath utility in the Act

The resting status of the coronary microcirculation is a predictor of microcirculatory function following elective PCI for stable angina.

BACKGROUND/OBJECTIVES: We performed a prospective study to investigate markers of percutaneous coronary intervention (PCI) related microvascular injury. METHODS: Consecutive patients undergoing PCI for stable angina were studied. The index of microvascular resistance (IMR) was measured using a temperature and pressure sensing guidewire (TPSG) before and after single vessel PCI. Basal transit-time (TmnBase), that reflected non-hyperemic blood flow was also measured. Fasting bloods were taken to measure blood sugar, HbA1c and lipids. Asymmetric dimethylarginine (ADMA) was also measured as a mar
