Obstructive Sleep Apnea and Cerebral Microbleeds in Middle-Aged and Older Adults.

IMPORTANCE: The association of obstructive sleep apnea (OSA) with risk of incident cerebral microbleeds (CMBs) is unknown. OBJECTIVE: To investigate the association between OSA severity and risk of incident CMBs in the late middle-aged general population. DESIGN, SETTING, AND PARTICIPANTS: This cohort study included eligible participants who had in-home overnight polysomnography data and brain magnetic resonance imaging done at baseline (2011-2014) and 4-year follow-ups (first follow-up, 2015-2018, and second follow-up, 2019-2022) from an ongoing longitudinal cohort study of a Korean communi

Intracranial atherosclerosis increases risk of incident dementia in middle-aged general population.

BACKGROUND: The impact of intracranial atherosclerosis, assessed using precise methods, on cognitive function and dementia remains an area of ongoing research. This study aimed to explore the association of intracranial atherosclerosis with incident dementia in middle-aged community dwellers. METHODS: A total of 1009 subjects from the Shunyi study started from 2013, with a mean age of 55.3 ± 8.6 years, were included. With magnetic resonance angiography (MRA) and high-resolution magnetic resonance imaging (HRMRI), intracranial artery stenosis and plaque were assessed. Cognitive function was ev

Unmasking the hidden culprit: neurosyphilis mimicking parkinsonism in a middle-aged male.

BACKGROUND: General paresis, a tertiary manifestation of neurosyphilis affecting the brain, is characterized by mental and behavioral disorders, such as attention disorder, cognitive impairment, and personality changes. But parkinsonism is rarely reported in patients with neurosyphilis, let alone general paresis. This study reports a case suffering from both general paresis and parkinsonism. CASE PRESENTATION: A 50-year-old man was initially misdiagnosed with "alcohol-related psychiatric and behavioral disorders" due to the onset of psychiatric symptoms following alcohol abstinence. The exces

Coronary Microvascular Function in Asymptomatic Middle-Aged Individuals With Cardiometabolic Risk Factors.

BACKGROUND: In patients with ischemic heart disease, coronary microvascular dysfunction is associated with cardiovascular risk factors and poor prognosis; however, data from healthy individuals are scarce. OBJECTIVES: The purpose of this study was to assess the impact of cardiovascular risk factors and subclinical atherosclerosis on coronary microvascular function in middle-aged asymptomatic individuals. METHODS: Myocardial perfusion was measured at rest and under stress using cardiac magnetic resonance in 453 individuals and used to generate myocardial blood flow (MBF) maps and calculate my

An unusual cause of recurrent abdominal pain in a middle-aged man.

A 54-year-old man was hospitalized with intermittent periumbilical pain for 1-month duration. Abdominal contrast-enhanced computed tomography (CT) revealed target-sign and a fat density mass measuring 2.0 × 2.5-cm in the distal ileum. Part mesenteric tissues and blood vessels were embedded and the wall of the affected intestinal tube was thickened and edematous. His symptom was alleviated after conservative treatment and he refused further management. The patient was hospitalized again with the same symptoms and abdominal CT findings 4 years later. Exploratory laparotomy was performed. A palpa

Accelerated myocardial fibrosis in young to middle-aged patients with hypertrophic cardiomyopathy.

BACKGROUND: The extent of late gadolinium enhancement (LGE) on cardiovascular magnetic resonance (CMR) in patients with hypertrophic cardiomyopathy (HCM) is associated with an increased risk of sudden cardiac death events. However, the clinical significance of age-specific longitudinal changes in LGE is not well characterized in HCM. We sought to assess whether the risk of LGE progression diverges between young to middle-aged (ages 20-59 years) and older (≥ 60) adults with HCM. METHODS: A total of 102 HCM patients (age <60 years; n=75, age ≥60 years; n=27) undergoing serial CMR studies from t

Middle-aged females are resistant to LPS-induced learning deficits: Sex comparison.

Preclinical data have repeatedly shown learning and memory disruption following administration of the bacterial endotoxin lipopolysaccharide (LPS). Normal aging is reported to enhance vulnerability to LPS-induced cognitive impairments. However, a limitation is the primary use of male subjects. Recent evidence indicates sex-related differences in vulnerability to LPS-induced cognitive deficits [1,2], with young females showing resilience. Whether middle-aged females are susceptible to LPS-induced cognitive impairment is unknown. The current experiment compared associative learning in young and

Categorising Subjective Perceptions of Middle-Aged Breast Cancer Patients Using Q Methodology.

BACKGROUND/OBJECTIVES: This study explores the characteristics of different perception types in middle-aged female breast cancer patients and proposes psychological counselling interventions tailored to each type. METHODS: The study used the Q methodology, starting with the construction of 40 Q samples and 39 P samples. RESULTS: We categorised middle-aged female patients' subjective perceptions of battling breast cancer into five types along a spectrum: Type 1, 'Embracing a New Life'; Type 2, 'Finally Focusing on Myself'; Type 3, 'Struggling Amidst Confusion'; Type 4, 'Withdrawing in Despair

Primary pulmonary meningioma mimicking a carcinoid tumor in a middle-aged female.

A 46-year-old female complained of cough and dyspnea. A chest X-ray and CT scan showed a solitary subpleural pulmonary nodule in the left upper lobe. Surgical resection was performed. The frozen examination was presumptive of a benign/low grade lesion, likely a carcinoid. Definitive histology revealed uniformly sized eosinophilic cells arranged in solid nests, immunoreactive for epithelial membrane antigen and progesterone receptors. The diagnosis of grade 1 meningothelial meningioma was finally achieved. This case of a meningioma mimicking a carcinoid tumor highlights its rarity and warns dia

Reference ranges for ambulatory heart rate measurements in a middle-aged population.

BACKGROUND: Elevated heart rate (HR) predicts cardiovascular disease and mortality, but there are no established normal limits for ambulatory HR. We used data from the Swedish CArdioPulmonary Imaging Study to determine reference ranges for ambulatory HR in a middle-aged population. We also studied clinical correlates of ambulatory HR. METHODS: A 24-hour ECG was registered in 5809 atrial fibrillation-free individuals, aged 50-65 years. A healthy subset (n=3942) was used to establish reference values (excluding persons with beta-blockers, cardiovascular disease, hypertension, heart failure, ana

Unusual Cause of Wheezing in a Middle-Aged Woman: A Case Report.

Endobronchial carcinoid tumors, a subset of neuroendocrine tumors, represent a rare but significant entity within pulmonary neoplasms, constituting less than 2% of all lung cancers. Our case report details the clinical presentation, diagnosis, and management of a 56-year-old female patient who presented with intermittent wheezing, mucoid cough, and recurrent pneumonia. Initial imaging and bronchoscopy identified an obstructive mass in the left lower bronchus. Histopathological examination of the bronchoscopic biopsy confirmed the diagnosis of a typical endobronchial carcinoid tumor. The patien

Atraumatic Bilateral Patella Fracture in Middle-Aged Female: A Rare Case Report.

The patella is the largest sesamoid bone in the body and an important structure of the extensor apparatus which is under undue stress during flexion and extension of the knee. Bilateral fracture of the patella without trauma is a very rare event and may be multifactorial without a single cause. A repetitive stress reaction in a previously predisposed bone can be an important cause. We report a case of a 45-year-old female with a nontraumatic bilateral transverse patella fracture with loss of extensor mechanism. A stable surgical fixation for such a displaced fracture with a good rehabilitation

Cortical and Trabecular Bone Deficit in Middle-Aged Men Living with HIV.

A significant increase in the risk of hip fracture was observed in middle-aged men living with human immunodeficiency virus (MLWH), almost a decade earlier than those without infection. Data regarding cortical and trabecular bone deficit of hip, an important determinant of bone strength, in MLWH are limited. Quantitative CT was performed in consecutive MLWH aged ≥30 years between November 2017 and October 2018 at Severance Hospital, Seoul, Korea. Volumetric bone mineral density (vBMD) and cortical bone mapping parameters of hip (cortical thickness [CTh], cortical bone vBMD [CBMD], cortical mas

Effects of Multi-Component Exercise on Sleep Quality in Middle-Aged Adults.

Sleep is a crucial factor in healthy aging. However, most middle-aged adults experience high levels of sleep disorders. While previous findings have suggested exercise training could benefit the quality of sleep, the effects of multi-component exercise on sleep quality are less examined. Accordingly, the current study aimed to assess the effectiveness of a multi-component exercise program on the quality of sleep among middle-aged adults. Twenty-four middle-aged adults were randomly assigned either to a multi-component exercise (MCE) group or a control group. The participants in the MCE group a

Hashimoto Encephalopathy of a Middle-Aged Man With Progressive Symptoms of Dementia.

Autoimmune encephalitis is caused by immunological reactions showing unconsciousness, agitation, and other neurological symptoms. Autoimmune diseases can be related to autoantibodies, causing encephalitis. These autoantibody-related encephalitides could appear in various clinical courses. As laboratory tests for detecting these antibodies are limited, diagnosis is difficult. Hashimoto's encephalopathy is autoimmune encephalitis caused by antibodies against the thyroid gland. This time, we experienced a case of a 69-year-old man with a chief complaint of subacute progression of amnesia and susp

Association of work and hypertension among middle-aged adults and elderly Indians.

PURPOSE: Over two-fifth of middle-aged adults and elderly (45 +) in India are hypertensive. Though studies examined prevalence, awareness and control of hypertension, little is known on the association of hypertension with work status in India. This study examines the variations of hypertension by types of work among middle-aged adults and the elderly in India. METHODS: Data were drawn from the Longitudinal Aging Survey of India (LASI), Wave 1, 2017-18, and analysis was restricted to participants aged 45 and above with complete information on employment and blood pressure (N = 59,196).
