Perinatal mortality and morbidity in caesarean section.

The study was conducted on 350 babies born by caesarean section. There were 29 perinatal deaths among 350 births giving a gross perinatal mortality rate of 8.3 per 1000 live births. Corrected perinatal mortality rate was 7.1%. The stillbirth rate was 2%. It was high for cases of abruptio placentae, transverse lie and cord prolapse. Septicaemia was the commonest cause of perinatal death followed by asphyxia and prematurity. Birth weight played an important role in the survival of babies. There was no foetal loss among babies in weight group of 3501-4000 g. Perinatal morbidity was mainly due to …

Early post-operative morbidity of total prostatectomy.

Complications were analysed in a contemporary series of 100 total prostatectomies performed, for carcinoma of the prostate, under the supervision of one surgeon. There were 7 major complications including 1 death. No patient had a rectal injury. Minor complications occurred in 33 patients. Severe stress incontinence persisted in only 2 patients and 12 had minor stress incontinence. Of 38 patients potent before surgery and in whom it was possible to preserve both "neurovascular bundles of Walsh", 45% retained post-operative potency. Preservation of potency was related to the pathological extent

Factors predicting postoperative morbidity in biliary calculi.

Statistical analysis of a group of 1,114 patients operated for biliary lithiasis shows that the gravity of the disease, the nature of the complications that accompany it, the need to approach the common bile ducts (CBD) and age, stand at the origin of postoperative complications. To reduce postoperative morbidity elective surgery should be performed in cases of biliary lithiasis with paucity of symptoms during periods free of an attack and before the patient ages.

Morbidity and mortality in mitral valve surgery.

To develop strategies for the management of high-risk patients, contemporary risk factors for operative mortality and postoperative ventricular dysfunction were identified in 214 patients undergoing mitral valve surgery in 1982 and 1983. Thirty-eight preoperative and perioperative variables were prospectively collected and analyzed by univariate and multivariate statistics. The overall mortality was 4.6% and the incidence of postoperative low-output syndrome (LOS) was 18.7%. Forty-seven patients with coronary artery disease (CAD) had a higher mortality and incidence of LOS (as evidenced by the

Heatwave morbidity and mortality in old age.

Mortality in the elderly is affected by external temperature at both extremes. Admissions to a geriatric unit during the hot summer of 1983 were compared with those in the same period in 1982, a typically British summer. There were 382 admissions and 69 deaths in 1982, and 468 admissions and 90 deaths during the same 22-week period in 1983. The principal difference in the cause of admissions was in the group cerebrovascular accidents/transient ischaemic attacks/subarachnoid haemorrhages which rose from 10% in 1982 to 15% in 1983. There was also a small increase in the proportion of respiratory

Influence of regional anaesthesia on postoperative morbidity.

A short review on the influence of regional anaesthesia on various parameters of postoperative morbidity is given. Only data from controlled studies have contributed to the conclusions. It appears that regional anaesthesia with local anaesthetics reduces intraoperative blood loss and postoperative thromboembolic complications after hip surgery and prostatectomy, while data from other procedures are inconclusive. Mortality is probably minimized by regional anaesthesia after acute hip surgery. No firm conclusion can be made with regard to the influence on postoperative cardial or pulmonary compl…

Motorcycling morbidity and mortality--an unstudied epidemic.

South Africa has the highest motorcycle accident and associated casualty rate in the world. In dealing with the problem South Africa lags far behind other countries where rider training programmes have had astonishing success. Accident statistics, problems relating to motorcycling and protective clothing are discussed. Recommendations are made which if implemented should reduce the accident and casualty rate by at least 50%; this indicates the size of the problem in this neglected field of automotive preventive medicine.

Typhoid perforation: factors affecting mortality and morbidity.

Thirty-six patients were operated upon for terminal ileal perforations in a two-year period; four cases were due to trauma, four to ascariasis and 28 to typhoid. Ileal perforations due to ascaris worms are differentiated from typhoid ileal perforations because worms are usually found lying freely in the peritoneal cavity or in close association with the perforations. Also, tests for Salmonellae and Shigella are usually negative. Factors affecting mortality and morbidity in typhoid ileal perforation include the age of the patient, duration of perforation before surgery, presence of additional c…

Perinatal mortality and morbidity in breech presentation.

The perinatal mortality associated with breech presentation at the Royal Women's Hospital, Melbourne, between 1974 and 1976 was 10.4%, or almost 5 times the overall hospital figure. Nine of 487 infants (1.8%) weighing greater than or equal to 2500 g died in the perinatal period, but 7 were already dead at the onset of labor or had congenital abnormalities incompatible with life. Sixty of 177 infants (33.9%) weighing 1000-2499 g died in the perinatal period, but 28 of these died due to prematurity alone or from complications of intrauterine hypoxia or birth trauma. Although elective cesarean se

Continuing mortality and morbidity from smallpox vaccination.

Three cases of cross-infection after smallpox vaccination are described, in two of which the outcome was fatal. Probably all occurred because simple precautions were not observed at the time of vaccination--for example, exclusion of contraindications and warnings about risks. If those countries still requiring evidence of vaccination for entry were to abolish this rule, however, the risk of cross-infection could be eliminated. Vaccinating a person with contraindications is justified only when exposure to smallpox has occurred.

Reducing the morbidity of vacuum aspiration abortion.

This paper reports an analysis of the measures taken at Preterm, Inc, of Washington, DC, to reduce the incidence of complications of first trimester outpatient abortions. Data are presented on the 291 complications (40.0/1000) resulting from 7272 procedures performed during 1976. Forty-one women were hospitalized (5.6/1000). It has been our experience that routine use of ultrasonography when gestational age is unclear, careful examination of tissue especially when a small quantity is present and early use of reevacuation to diagnose and treat postabortal complications are essential in reducing

Neonatal morbidity among infants of diabetic mothers.

While the modern approach to management of diabetic pregnancy has reduced the perinatal mortality significantly, the neonatal morbidity remains high. This study has investigated factors which may account for the persisting high neonatal morbidity when birth trauma has been virtually eliminated and the incidence of respiratory distress syndrome (RDS) considerably reduced. Major congenital malformations emerge not only as the leading cause of perinatal losses but also as an important cause of morbidity. Delivery before 37 weeks increased the incidence of RDS and hypocalcemia, and it is suggested…
