Profiling "Noncombat" Musculoskeletal Injuries in Special Operations Forces: A Systematic Review.

BACKGROUND: Special Operations Forces (SOF) personnel are at a high risk of musculoskeletal (MSK) injury. The aims of this systematic review were to a) profile MSK injuries sustained by SOF personnel and b) identify evidence-based injury prevention strategies. METHODS: Registered with the Open Science Framework, the protocol followed the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines. Of the 3,773 studies identified, 14 met the eligibility criteria, with 6 additional studies identified following screening of the reference lists of the included studies. Extracted

Musculoskeletal disorders associated with obesity: a biomechanical perspective.

Despite the multifactorial nature of musculoskeletal disease, obesity consistently emerges as a key and potentially modifiable risk factor in the onset and progression of musculoskeletal conditions of the hip, knee, ankle, foot and shoulder. To date, the majority of research has focused on the impact of obesity on bone and joint disorders, such as the risk of fracture and osteoarthritis. However, emerging evidence indicates that obesity may also have a profound effect on soft-tissue structures, such as tendon, fascia and cartilage. Although the mechanism remains unclear, the functional and str

Oligonucleotide Therapeutics for Age-Related Musculoskeletal Disorders: Successes and Challenges.

Age-related disorders of the musculoskeletal system including sarcopenia, osteoporosis and arthritis represent some of the most common chronic conditions worldwide, for which there remains a great clinical need to develop safer and more efficacious pharmacological treatments. Collectively, these conditions involve multiple tissues, including skeletal muscle, bone, articular cartilage and the synovium within the joint lining. In this review, we discuss the potential for oligonucleotide therapies to combat the unmet clinical need in musculoskeletal disorders by evaluating the successes of oligon…

Circadian rhythm: The hidden "puppet master" behind musculoskeletal disorders.

Circadian rhythm (CR), regulated by the central biological clock in the hypothalamus and synchronized with local clocks in peripheral tissue … ers through multiple pathways. In this review, we summarize the role of CR in the physiology of the musculoskeletal system-encompassing bone, cartilage, tendons, and muscles-alongside the mechanisms by which CR disruption contributes to various musculoskeletal disorders. Additionally, we discuss interventions for treating musculoskeletal disorders based on CR, including …

Pain and discomfort in the musculoskeletal system were assessed using Nordic Musculoskeletal Questionnaire. RESULTS: Pb-exposure was significantly associated with musculoskeletal discomfort of the lower back (P<0.001), upper back (P<0.001), and ankle/foot (P=0.011). Among bone formation markers, s …

Role of musculoskeletal ultrasound in diagnosing and managing chronic inflammatory arthritis in overweight and obese patients: a narrative review.

Obesity and metabolic syndrome play a significant role in the complexity of chronic inflammatory arthritis. By promoting systemic inflammation and altering immune responses, these conditions can amplify joint-related symptoms, such as pain, synovitis, and enthesitis. This inflammatory and mechanical burden complicates clinical evaluation, as traditional disease activity scores may be skewed by excess weight, leading to inaccurate assessments. Imaging techniques like musculoskeletal ultrasound (MSUS) offer a promising tool to detect subclinical inflammation and improve diagnostic accuracy. This

Musculoskeletal Ultrasonography in Normal Feral Pigeons (Columba livia F. dom.).

Avian orthopedic disorders are common but can be challenging to diagnose. Radiographic examination and, to a lesser extent, computed tomogra … ificant. This study provides evidence that B-mode ultrasonography can qualitatively assess pigeons' musculoskeletal systems, notably in the pectoral area and proximal limbs. This technique can also be used to acquire measurements of the structures thicker than 0.2 cm, which may be used for osteosynthesis planning or monitoring neoplastic conditions posttreatment.

Macrophage Mechano-Responsiveness Within Three-Dimensional Tissue Matrix upon Mechanotherapy-Associated Strains.

Mechano-rehabilitation, also known as mechanotherapy, represents the forefront of noninvasive treatment for musculoskeletal (MSK) tissue disorders, encompassing conditions affecting tendons, cartilage, ligaments, and muscles. Recent emphasis has underscored the significance of macrophage presence in the healing of MSK tissues. However, a considerable gap still exists in comprehending how mechanical strains associated with mechanotherapy impact both the naïve and pro-inflammatory macrophage phenotypes within the three-dimensional (3D) tissue matrix, as well as whether the shift in macrophage ph

Importance of strength training to prevent secondary lower limb musculoskeletal injury in military personnel.

Considerable operational and financial costs are associated with musculoskeletal injury (MSKI) across military organisations. Following an MSKI, reaching 'medically fully deployable' status is often contingent on meeting physical employment standards. Strength training has a critical role to play in military physical preparation, injury mitigation and guiding rehabilitation practice. Low levels of muscle strength are associated with increased MSKI risk, and previous MSKI is a leading risk factor for re-injury. When an MSKI does occur, it is essential that all attempts to mitigate the risk of s

Ultrasound Examination of Pediatric Musculoskeletal Diseases and Neonatal Spine.

Ultrasound (US) is a simple, non-invasive imaging modality which allows high-resolution imaging of the musculoskeletal (MSK) system. Its increasing popularity in pediatrics is due to the fact that it does not involve radiation, has an ability to visualize non-ossified cartilaginous and vascular structures, allows dynamic imaging and quick contralateral comparison. US is the primary imaging modality in some pediatric MSK conditions like infant hip in developmental dysplasia (DDH), hip joint effusion, epiphyseal trauma and evaluation of the neonatal spine. US is the modality of choice in infants

Musculoskeletal manifestations of Ehlers-Danlos syndrome.

Ehlers-Danlos syndrome is a disorder caused by mutations in connective tissue and muscle tissue genes. Alteration in genes disrupts the functional ability of muscles, ligaments, and joints. Adaptation in the formation of collagen also affects the support and elasticity that collagen provides to the musculoskeletal system. The pathogenesis of Ehlers-Danlos syndrome, as it relates to the musculoskeletal system, is presented.

Greater focus on clinical rheumatology is required for training in internal medicine.

Early intervention for rheumatic disease has been shown to improve prognosis, but is dependent on the recognition of pertinent symptoms and signs. This study was undertaken to identify the attitudes of junior doctors toward musculoskeletal assessment. The charts of 100 randomly selected medical patients admitted via the Emergency Department were reviewed for documentation of the musculoskeletal system. In addition, junior doctors were interviewed regarding their views on musculoskeletal assessment. Only 16% of medical admission notes contained a reference to the musculoskeletal system and the …
