Treatment of infra-isthmal femoral fracture with an intramedullary nail: Is retrograde nailing a better option than antegrade nailing?

INTRODUCTION: Antegrade intramedullary (IM) nailing is ideal for femoral shaft fractures, but fixing the fracture distal to the isthmal level may be difficult because of medullary canal widening and the proximity of fracture location from the distal femoral joint line. This study aimed to compare treatment results between antegrade and retrograde nailing for infra-isthmal femoral shaft fracture, and to identify influencing factors of nonunion and malalignment. MATERIALS AND METHODS: Sixty patients with infra-isthmal femoral shaft fractures treated with IM nailing and followed-up for > 1 year

Intramedullary nailing of subtrochanteric fractures in elderly patients: Comparative study of helical blade cephalomedullary nail versus reconstruction nail.

INTRODUCTION: Intramedullary nailing (IMN), which is a common method for treating subtrochanteric fractures, is conducted as cephalomedullary (CMN) or reconstruction (RCN) nailing. Numerous studies have reported the effectiveness of CMN, which requires a shorter surgery time and provides stronger fixation strength with blade-type devices. However, the radiographic and clinical outcomes of the use of CMN and RCN in elderly patients aged ≥65 years have not been compared yet. This study aimed to investigate whether CMN offers superior outcomes over RCN in the treatment of subtrochanteric fracture

Effect of Nail Size, Insertion, and Δ Canal-Nail on the Development of a Nonunion After Intramedullary Nailing of Femoral Shaft Fractures.

OBJECTIVE: To determine whether intramedullary nail (IMN) diameter, antegrade versus retrograde insertion, or the difference between the canal and IMN diameter affect femoral shaft fracture healing. DESIGN: Retrospective analysis of a prospective database. SETTING: Level One Regional Trauma Center. PATIENTS/PARTICIPANTS: Seven hundred thirty-three femoral shaft fractures (OTA/AO 32) treated with an IMN between 1999 and 2017. After exclusion criteria, 484 fractures remained in the final analysis. INTERVENTION: Closed section, cannulated, interlocked, titanium alloy IMN using a reamed insert

Proximal Femoral Nail Antirotation versus Gamma3 nail for intramedullary nailing of unstable trochanteric fractures. A randomised comparative study.

The purpose of this study was to compare the clinical results and the complication rates of a new generation of two intramedullary fixation devices: Proximal Femoral Nail Antirotation® (PFNA) and Gamma3®. We conducted a prospective randomised multicentre trial of 61 patients who underwent a PFNA fixation treatment (31 patients) or a Gamma3 nail (30 patients). We documented the fixation failure complications as well as data of the surgical procedure, the clinical and functional outcomes (the EuroQoL5 questionnaire, the Katz index score, the Short Form 36 (SF-36), and the Harris Hip Score) and t

Compatibility of Retrograde Intramedullary Nails With Knee Prostheses: A Comprehensive Analysis.

BACKGROUND: Periprosthetic distal femoral fractures pose major challenges in orthopaedic surgery, particularly among aging populations with total knee arthroplasties. Retrograde intramedullary nailing has emerged as a viable fixation option due to its biomechanical stability, less invasive nature, and facilitation of early weight-bearing. However, the compatibility between retrograde nails and various knee prostheses remains a critical determinant of surgical success. This review consolidated existing knowledge and evaluated current nail-prosthesis compatibility and options for lateral femoral…

Raman spectroscopy for molecular diagnosis of psoriatic nails with SEM validation.

Nail involvement is commonly observed in psoriasis, a chronic immune-mediated disorder; however, the diagnosis of mild or asymptomatic cases remains clinically challenging. In this study, Raman spectroscopy and scanning electron microscopy (SEM) were employed to distinguish among symptomatic psoriatic nails, asymptomatic psoriatic nails, and healthy controls. Raman spectral analysis revealed a reduction in α-helix content (I1652/I1002) in symptomatic nails and a significantly elevated I877/I1002 ratio (p < 0.001) in asymptomatic cases, suggesting that this ratio may serve as a potential early …

Outpatient limb lengthening using magnetic intramedullary nails: A single Institution's experience.

INTRODUCTION: The advantages of outpatient surgery have been shown in other orthopedic subspecialities to be a means of driving down costs and reducing the average length of hospital stay. However, there is a scarcity of literature examining the utility of a specific, hospital-based facility performing such procedures for limb lengthening. Considering this, we aimed to investigate surgical factors, patient characteristics, and the incidence of outpatient postoperative complications for patients undergoing surgery and subsequent distraction osteogenesis utilizing the Precice® nail, a state-of-t

Shoreline Nails: A New Nail Sign of SARS-CoV-2 Infection.

With the onset of the SARS-CoV-2 pandemic, a number of nail unit changes have been associated with SARS-CoV-2 infection. We report a new unique nail finding within one month of infection and RT-PCR test positivity characterized by an abrupt proximal superficial nail plate change characteristic of shoreline nails. The possibility that this nail change may represent a Koebner phenomenon in patients prone to lichen planus is raised. J Drugs Dermatol. 2022;21(5):521-522. doi:10.36849/JDD.2105.

Comparison of carbon fibre and titanium intramedullary nails in orthopaedic oncology.

AIMS: Due to their radiolucency and favourable mechanical properties, carbon fibre nails may be a preferable alternative to titanium nails for oncology patients. We aim to compare the surgical characteristics and short-term results of patients who underwent intramedullary fixation with either a titanium or carbon fibre nail for pathological long-bone fracture. METHODS: This single tertiary-institutional, retrospectively matched case-control study included 72 patients who underwent prophylactic or therapeutic fixation for pathological fracture of the humerus, femur, or tibia with either a tita…

Outcomes after unstable pertrochanteric femur fracture: intermediate versus long cephalomedullary nails.

PURPOSE: Optimal cephalomedullary nail (CMN) length for unstable pertrochanteric femur fractures is controversial. Long CMNs (L-CMNs) are currently recommended; however, intermediate-length CMNs (I-CMNs) may provide stable fixation without the additional surgical steps required by L-CMNs. We analyzed outcomes after unstable pertrochanteric femur fractures treated with L-CMNs or I-CMNs to determine whether functional outcomes, perioperative measures, complications, and mortality and reoperation rates differ by CMN length. METHODS: We retrospectively reviewed medical records at our institution

Trochanteric Entry for Femoral Lengthening Nails in Children: Is It Safe?

BACKGROUND: Antegrade intramedullary (IM) nailing for skeletally immature femur fractures can damage the capital femoral epiphysis blood supply, leading to avascular necrosis (AVN) of the femoral head. Reported AVN rates are 2% for piriformis entry and 1.4% for trochanteric entry. None of previous reports described IM lengthening nails for limb lengthening procedures. We have used self-lengthening telescopic nails with a proximal Herzog bend and standard trochanteric entry for femoral lengthening in children. The purpose of this study is to determine whether trochanteric entry IM lengthening n…

[Infections of finger and toe nails due to fungi and bacteria].

Infections of the finger and the toe nails are most frequently caused by fungi, primarily dermatophytes. Causative agents of tinea unguium are mostly anthropophilic dermatophytes. Both in Germany, and worldwide, Trichophyton rubrum represents the main important causative agent of onychomycoses. Yeasts are isolated from fungal nail infections, both paronychia and onychomycosis far more often than generally expected. This can represent either saprophytic colonization as well as acute or chronic infection of the nail organ. The main yeasts causing nail infections are Candida parapsilosis, and Can…
