Acute Longus Colli Calcific Tendinitis: A Rare Cause of Neck Pain.

UNLABELLED: A 48-year-old man presented to our emergency department with neck pain without sensorimotor deficit and with a sore throat without signs of infection. Magnetic resonance imaging was performed because the patient had not responded to regular treatment and a blood test had showed inflammation. The images revealed cervical prevertebral fluid collection and calcification, compatible with acute calcific tendinitis of the longus colli muscle. Prednisolone 50 mg with pantoprazol 40 mg was administered for 5 days with rapid resolution of symptoms. Acute calcific tendinitis of the longus co…

Misconceived Retropharyngeal Calcific Tendinitis during Management of Myofascial Neck Pain Syndrome.

Differential diagnosis of posterior neck pain is very challenging based on symptoms and physical examination only. Retropharyngeal calcific tendinitis is a rare and frequently misdiagnosed entity in various causes of neck pain. It results from calcium hydroxyapatite deposition in the longus colli muscle which is characterized by severe neck pain, painful restriction of neck movement, dysphagia, and odynophagia. We herein report a case of a patient with acute retropharyngeal calcific tendinitis, who complained of posterior neck pain, initially diagnosed and treated as a myofascial neck pain syn…

Persistent axial neck pain after cervical disc arthroplasty: a radiographic analysis.

BACKGROUND CONTEXT: There is very little literature examining optimal radiographic parameters for placement of cervical disc arthroplasty (CDA), nor is there substantial evidence evaluating the relationship between persistent postoperative neck pain and radiographic outcomes. PURPOSE: We set out to perform a single-center evaluation of the radiographic outcomes, including associated complications, of CDA. DESIGN: This is a retrospective review. PATIENT SAMPLE: Two hundred eighty-five consecutive patients undergoing CDA were included in the review. OUTCOME MEASURES: The outcome measures wer…

Standing balance: a comparison between idiopathic and whiplash-induced neck pain.

Disturbances of balance have been found both in patients with whiplash-associated disorders and idiopathic neck pain. This study directly compared balance between these groups to determine if neck pain precipitated by trauma resulted in greater or different balance impairments. The study was a comparative, observational design. Thirty subjects with whiplash, 30 with idiopathic neck pain and 30 healthy controls, took part in the study. Subjects performed balance tests in comfortable, narrow and tandem stances. Balance disturbances (sway energy and/or root mean squared (RMS) amplitude) were evid…

Neck pain and disability following motor vehicle accidents--a cohort study.

The primary aim of the study was to compare the prevalence of neck pain and disability in a group exposed to motor vehicle accidents (MVAs) with those in the general population. The secondary aim was to assess the prevalence of a past history of exposure to an MVA with sequelae of neck pain in the general population. The exposed group consisted of 121 patients with neck complaints following an MVA in 1983. The control group, consisting of 1,491 subjects, was randomly selected, with attention to the distribution of age and gender in the exposed group. A neck-pain questionnaire was mailed to the…

Relationship between sleep, neck muscle activity, and pain in cervical dystonia.

OBJECTIVE: The interactions between sleep, neck muscle activity, and cervical spinal pain were examined in a controlled study with nine patients suffering from idiopathic cervical dystonia (ICD; also referred to as spasmodic torticollis), and nine gender- and age-matched controls. METHODS: From each participant, two all-night polysomnograms with additional electromyographic recordings from the sternocleidomastoid and upper trapezius muscles were obtained. The first night was for habituation to the laboratory environment; the second night for experimental data collection. Visual analogue scale

Articular mass fracture: a neglected cause of post-traumatic neck pain?

Fifty-one compressed distorted cervical articular masses were found in 29 patients seen at this institution over a nine-month period. The si … graphy. Articular mass deformity frequently appears to be associated with persistent post-traumatic neck pain and a relationship between this deformity and primary or secondary degenerative cervical disc and joint disease is suggested.

Van Neck-Odelberg Disease: A Rare Cause of Pediatric Hip Pain.

INTRODUCTION: Synchondrosis ischiopubic syndrome (SIS), also known as Van Neck-Odelberg disease, is a benign skeletal abnormality in children due to delayed closure in the ischiopubic synchondrosis (IPS). Patients may present with vague groin, hip, or buttock pain that can cause limitation of hip movement or limp. Few descriptions of this disorder exist, and it can easily be mistaken for other pathologies, including stress fracture, osteomyelitis, or tumor. CASE REPORT: We report the case of a 9-year-old female gymnast presenting with right hip pain who was diagnosed with SIS. She was managed

Neck Pain and Headache After Pinealectomy: Improvement with Multimodal Chiropractic Therapies.

BACKGROUND Pineal gland tumors are rare central nervous system tumors, and while neck pain and headaches may be common among those who have had these tumors removed, there is little research regarding management of these symptoms. CASE REPORT A 45-year-old man with a history of pineal germinoma treated with pinealectomy, chemotherapy, radiation therapy, and ventriculoperitoneal shunt placement at age 21 presented with chronic neck pain and headaches, which initially improved following his surgery and concurrent therapies, yet progressively worsened over the following years. He required thyroid…

Local heating of trigger points reduces neck and plantar fascia pain.

BACKGROUND: Heating the skin and muscles is a commonly accepted method of pain relief and a modality to increase relaxation in muscles and increase tissue blood flow. OBJECTIVE: The purpose of the present study was to examine the effect of local heat applied to trigger points and to determine if there was pain relief in the neck and plantar fascia. METHODS: Forty adults were divided into 2 different groups according to their pain; twenty subjects had plantar foot pain and the other 20 had nonspecific neck pain. The 20 subjects in each group were randomly subdivided into a heat and a sham gro…

Neck pain patterns and subgrouping based on weekly SMS-derived trajectories.

BACKGROUND: Neck and low back pain represent dynamic conditions that change over time, often with an initial improvement after the onset of a new episode, followed by flare-ups or variations in intensity. Pain trajectories were previously defined based on longitudinal studies of temporal patterns and pain intensity of individuals with low back pain. In this study, we aimed to 1) investigate if the defined patterns and subgroups for low back pain were applicable to neck pain patients in chiropractic practice, 2) explore the robustness of the defined patterns, and 3) investigate if patients with…

Relation Between Neck Pain and Modic Changes in Cervical Spondylotic Myelopathy.

BACKGROUND Modic changes on magnetic resonance imaging (MRI) represent lesions in the bone marrow of the vertebral body that may be associated with pain. This study investigated the relationship between neck pain and cervical Modic changes, and risk factors for neck pain (NP). MATERIAL AND METHODS This study included 168 patients diagnosed with cervical spondylotic myelopathy between February 2013 and April 2017. Basic patient information about demographic and lifestyle variables was obtained from medical records. Radiographic images provided cervical alignment (C2-C7 Cobb), T1 slope and C7 sl…

Does physical activity buffer insomnia due to back and neck pain?

INTRODUCTION: Musculoskeletal pain is highly prevalent and a burden to society, recurrent and persistent low back pain (LBP) and neck pain (NP) being the most common conditions. They are associated with other poor health outcomes such as sleep problems. Physical activity (PA) prevents LBP and NP, but the direct effect on sleep is unclear. This study explored the effect of pain on insomnia, and examined if adherence to moderate-to-high intensity levels of PA influenced this relationship. METHODS: In this prospective observational study, 1821 workers were followed over 3 years. Data included se…

Unusual case of acute neck pain: acute calcific longus colli tendinitis.

Acute calcific longus colli tendinitis (ACLCT), a very rare cause of severe neck pain, dysphagia and odynophagia, is often mistaken for other common causes of neck pain. However, prompt recognition of this uncommon presentation is important to prevent unnecessary medical and surgical intervention. A 46-year-old Caucasian man presented with a 1-day history of severe neck pain, headache and odynophagia. The patient was afebrile with stable vital signs, however, the laboratory data showed mildly elevated C reactive protein and erythrocyte sedimentation rate. The physical examination was remarkabl…
