Percutaneous Nephrolithotomy under Local Infiltration Anesthesia in Kneeling Prone Position for a Patient with Spinal Deformity.

Urolithiasis, a common condition in patients with spinal deformity, poses a challenge to surgical procedures and anesthetic management. A 51-year-old Chinese male presented with bilateral complex renal calculi. He was also affected by severe kyphosis deformity and spinal stiffness due to ankylosing spondylitis. Dr. Li performed the percutaneous nephrolithotomy under local infiltration anesthesia with the patient in a kneeling prone position, achieving satisfactory stone clearance with no severe complications. We found this protocol safe and effective to manage kidney stones in patients with sp

Laparoscopic-assisted mini percutaneous nephrolithotomy in the ectopic pelvic kidney: Outcomes with the laser dusting technique.

INTRODUCTION: The treatment of renal lithiasis has undergone a sea change with the advent of extracorporeal shock wave lithotripsy (ESWL) and endourological procedures such as percutaneous nephrolithotomy (PCNL), ureterorenoscopy and retrograde intrarenal surgery (RIRS). The presence of anatomical anomalies, such as ectopic pelvic kidney, imposes limitations to such therapeutic procedures. This study is aimed to find a simple and effective way to treat the stones in ectopic kidney. MATERIALS AND METHODS: From 2010 to 2014, nine patients underwent laparoscopic-assisted mini PCNL with Laser dus

Percutaneous nephrolithotomy in renal anomalies of fusion, ectopia, rotation, hypoplasia, and pelvicalyceal aberration: uniformity in heterogeneity.

CONTEXT: Percutaneous nephrolithotomy (PCNL) in renal fusion, ectopia, renal rotational anomalies, renal hypoplasia, and pelvicaliceal aberration for complex calculi is challenging for fear of inaccessibility, vascular, colonic, and splanchnic injury. We report our experience with 48 such patients. PATIENTS AND METHODS: From 2002 to July 2007, 48 patients (mean age, 28 years) underwent PCNL for mean stone size of 39 mm. Fourteen patients had malrotated kidneys, 12 patients had bifid renal pelvis with complete or incomplete double ureter, 10 horseshoe kidneys, 8 hypo plastic kidneys, 3 with cr

[Percutaneous nephrolithotomy (PCNL) in subjects over the age of 70: a multicentre retrospective study of 210 cases].

INTRODUCTION: Percutaneous nephrolithotomy (PCNL) in subjects over the age of 70 is considered to be associated with a higher risk than extracorporeal shock-wave lithotripsy (ESWL). However this technique is sometimes necessary for very large or complex stones in patients with several comorbidities. STUDY OBJECTIVE: To evaluate the predictive factors of success and operative risks likely to influence the results of PCNL in a population of patients over the age of 70. MATERIAL AND METHOD: Retrospective study of 203 patients (110 males, 97 females) over the age of 70, in whom a total of 210 PC

Minimally invasive surgery via minimally invasive anesthesia Is laryngeal mask airway sufficient for supine percutaneous nephrolithotomy?

INTRODUCTION: While endotracheal tube (ETT) is the traditional airway modality for percutaneous nephrolithotomy (PCNL), the adoption of supine positioning has opened the door for alternative strategies. Laryngeal mask airway (LMA) offers potential advantages, but its safety profile in supine PCNL remains understudied. This study aimed to compare urologic and anesthesia-related outcomes between LMA and ETT in patients undergoing ultrasound-guided supine PCNL. METHODS: In this retrospective study, 206 adult patients undergoing supine PCNL under general anesthesia between March 2023 and June 202

Urological and nursing-related predictors of unplanned hospital readmission after percutaneous nephrolithotomy: A prospective cohort study.

BACKGROUND: Percutaneous nephrolithotomy (PNL) is the standard treatment for medium-sized and large kidney stones. Many potential complications of PNL may warrant hospital readmission (HR) after discharge, threatening patient safety and increasing the costs. AIM: To estimate the rate of unplanned HR after PNL and identify its urological and nursing-related predictors. METHODS: One hundred sixty-one patients were prospectively studied for HR after PNL from April 2022 to December 2022. The relevant urological and nursing-related characteristics of patients with and without unplanned HR after P

The Role of Renal Pelvis Urine Attenuation Value in Forecasting Infection Risk After Mini-Percutaneous Nephrolithotomy.

Objective: To evaluate whether renal pelvis urine density (RPUD), measured on preoperative computed tomography (CT), predicts infectious complications following percutaneous nephrolithotomy (PCNL). Methods: This retrospective study included patients who underwent PCNL between June 2019 and June 2024 at a tertiary care center. Patients with preoperative infection signs, drainage devices, or incomplete data were excluded. All included patients had sterile urine cultures preoperatively. RPUD was measured on noncontrast CT by two independent urologists, and interobserver agreement was calculated.

Cone Beam Computed Tomography-Assisted Percutaneous Nephrolithotomy in a Hybrid Operating Room: Optimization of Patient Selection.

Background: Cone beam computed tomography (CBCT) enables intraoperative cross-sectional and three-dimensional imaging of the urinary tract. CBCT in a hybrid operating room can be used for intraoperative detection of residual stones and potential additional stone extraction at the end of percutaneous nephrolithotomy (PCNL). This study describes our initial experience with intraoperative CBCT during PCNL and analyzes its role in potentially improving its outcomes. Methods: We conducted a single-center retrospective cohort study at a tertiary referral hospital between 2018 and 2021. The study aim

A Novel Method for Repositioning Suboptimally Preoperatively Placed Nephrostomy Tubes for Percutaneous Nephrolithotomy Without Renal Repuncture.

Objective: Nephrostomy tubes (NTs) inserted in emergency settings by interventional radiologists are frequently unsuitable for subsequent percutaneous nephrolithotomy (PCNL). We report a novel method of adjusting these NTs to be used as PCNL tracts and avoid renal repuncture. Patients and Methods: A retrospective search of 981 consecutive PCNLs performed in our institution between 2002 and 2017 identified all patients with preoperatively inserted NTs. The NTs unsuitable for PCNL were adjusted by a novel approach in which a 5-mm incision was made at the ideal puncture location (IPL) as indicate

Laparoscopy-assisted tubeless percutaneous nephrolithotomy in previously operated ectopic pelvic kidney with fragmented J-J stent.

Nephrolithiasis management within an ectopic kidney presents a challenge to the urologists. Several treatment modalities are possible in this group of patients. Although percutaneous nephrolithotomy (PNL) is an accepted treatment modality in anatomically normal kidneys, ectopic pelvic kidneys require a different and more complicated approach for PNL. We recently treated a 24-year-old patient with calculus and an encrusted J-J stent fragment in pelvic ectopic kidney with a previous history of open pyelolithotomy. Laparoscopy-assisted tubeless percutaneous nephrolithotomy was performed. The tech
