In order to test the functional implication of herpes simplex virus (HSV) vector-mediated gene transfer after axonal injury, we injected replication-incompetent HSV vectors coding for the anti-apoptotic peptide Bcl-2 and the glial cell-derived neurotrophic factor (GDNF), separately or in combination into ventral spinal cord 30 min after a crush injury to the proximal spinal root that was combined with moderate mechanical traction. HSV-mediated expression of Bcl-2 or GDNF enhanced functional recovery assessed by histologic, electrophysiologic, and behavioral parameters up to 5 months after inju

Patients suffer bilateral sacral plexus injuries experience severe problems with incontinence. We performed a cadaveric study to explore the anatomical feasibility of transferring ipsilateral S2 nerve root combined with a sural nerve graft to pudendal nerve for restoration of external anal and urethral sphincter function. The sacral nerve roots and pudendal nerve roots on the right side were exposed in 10 cadavers. The length from S2 nerve root origin to pudendal nerve at inferior border of piriformis was measured. The sural nerve was used as nerve graft. The diameters and nerve cross-sectiona

BACKGROUND: Arthrogryposis multiplex congenita (AMC) is a rare disease which affects mainly upper and lower extremities. Affected patients are not able to eat unassisted due to elbow contracture and nonexistent active elbow flexion. In traumatic brachial plexus palsies, a nerve transfer from either median or ulnar nerve to the musculocutaneous nerve has proved to induce active elbow flexion, and we report our results of such a procedure in a nontraumatic condition, that is, arthrogryposis.
METHODS: We selected four patients with AMC type 1 (6 extremities, 2 males, 2 females) diagnosed with AM…

INTRODUCTION: Restoration of elbow flexion is the primary treatment goal in adults with supra-clavicular traumatic brachial plexus injuries (ATBPI). To date, many nerve surgeons will initially await spontaneous recovery when dealing with an incomplete brachial plexus injury to allow sufficient recovery in the median and ulnar nerves to enable distal fascicular nerve transfers (DFNT). An alternative strategy is reconstruction by supraclavicular nerve grafting, which should be performed as early as possible after trauma to minimize denervation time.
RESEARCH QUESTION: Does early supraclavicular…

In the management of upper type of brachial plexus injury, reconstruction to restore shoulder function is accomplished by multiple nerve transfers. We used the accessory nerve to neurotize the suprascapular nerve in 12 patients (11 men, 1 woman) from 1989 to 2003. The average age at the time of operation was 28.1 years (range 16 to 53). The mean preoperative time was 3.6 months. The type of paralysis was C5-C6 type in four cases, C5-C7 type in five cases, and C5-C8 type in three cases. The average time of follow-up was 28.5 months. All the patients showed reinnervation of the supraspinatus and…

The primary goal for many individuals with tetraplegia is restoration of hand and arm function. Recent advancements in surgical techniques f … ame function. Tendon transfers can be used secondarily; in the same way, reanimated muscles through nerve transfer can also be transferred at a later stage. Strong consideration should be given to a nerve transfer reconstruction when surgery is performed early. Secondary surgery may be required to further adjust the hand position to optimize balance and maximize …

PURPOSE OF REVIEW: The purpose of this review is to summarize recent advances in facial nerve repair and facial reanimation as it relates to neurorrhaphy techniques, bioengineered options and nerve transfers.
RECENT FINDINGS: Direct neurorrhaphy is the gold standard for facial nerve repair when tension-free repair is achievable. Interposition grafting with autologous donor nerves facilitates repair when a tension-free repair is not feasible. Adjunctive bioengineered materials have been developed to enhance nerve repair, but high-quality clinical evidence supporting their use in facial nerve r…

BACKGROUND: End-to-side nerve transfer (ETSNT) for treatment of peripheral nerve injuries is controversial given the myriad anatomic locations, injury types, and indications. Efficacy of ETSNT remains debated. We hypothesized differences in age, sex, transfer location, and time to surgery influence outcomes.
METHODS: We performed a search of the PubMed database for ETSNT in the upper extremity from 1988 to 2018. Age, sex, transfer location, time to surgery, donor and recipient axons, and strength and sensation outcomes as measured by Medical Research Council scale were extracted from articles…

BACKGROUND: Acute flaccid myelitis (AFM) is a rare disease that commonly affects young children. AFM's pathophysiology involves loss of lower motor neurons following a viral infection and induces acute asymmetric flaccid paralysis most commonly in the upper extremities. Nerve transfers have emerged as a treatment option for these patients with permanent motor deficits.
OBJECTIVE: To summarize the literature and report safety and efficacy outcomes following nerve transfers for recovery of shoulder abduction and external rotation, and elbow flexion and extension in pediatric patients with AFM. …
