Cervical spinal stenosis with cord neurapraxia: evaluations and decisions regarding participation in athletics.

Cervical spinal cord neurapraxia (CCN) leads to transient episodes ranging from paresthesia to paresis to plegia (complete paralysis), and occurs in athletes with some demonstrable degree of cervical spinal stenosis. Determination of spinal stenosis requires demonstrating a sagittal diameter of the spinal canal less than 14 mm from C4 to C6. Because radiologic techniques vary affecting the accuracy of this measure, a ratio method was developed comparing the spinal canal to the vertebral body width, demonstrating that a ratio of less than 0.8 is indicative of cervical spinal stenosis. Although

[Multifocal motor neuropathy with partial block of nerve conduction].

INTRODUCTION: Multifocal motor neuropathy with partial conduction block is characterized by being a chronic, demyelinating, autoimmune severely disabling neuropathy. In Cuba three cases were reported by Dr. Estrada et al in 1999. This neuropathy presents clinically in relatively young persons. The arms are predominantly affected and the typical signs are of severe asymmetrical weakness, with atrophy which is less marked than the weakness, fasciculations, cramps and myokymiae of the affected muscle. Neurophysiological study shows partial block of motor nerve conduction. Clinical interest is due

Electrodiagnostic studies in amyotrophic lateral sclerosis and other motor neuron disorders.

The clinical electrodiagnostic medicine (EDX) consultant asked to assess patients with suspected amyotrophic lateral sclerosis (ALS) has a number of responsibilities. Among the most important is to provide a clinical assessment in conjunction with the EDX study. The seriousness of the diagnoses and their enormous personal and economic impact require a high-quality EDX study based on a thorough knowledge of and experience with motor neuron diseases (MNDs) and related disorders. Clinical evaluation will help determine which of the EDX tools available to the EDX consultant should be applied in in

Classification and clinical features of motor neurone diseases and motor neuropathies in adults.

The term motor neurone disease encompasses combined upper and lower motor neurone disorders (amyotrophic lateral sclerosis), pure lower motor neurone disorders (spinal muscular atrophies, multifocal motor neuropathies, post irradiation lumbosacral radiculopathy, post-polio syndrome, hereditary bulbar palsy) and pure upper motor neurone disorders (primary lateral sclerosis, hereditary spastic paraplegia, neurolathyrism, Konzo). The chief clinical and electrophysiological criteria for these different disorders are discussed, with particular attention to diagnostically distinctive characteristics

A case of multifocal motor neuropathy.

We report here a patient with multifocal motor neuropathy (MMN). He had typical clinical features, and electrophysiological investigations confirmed the diagnosis. This is the first case of MMN to be documented in Sri Lanka.

Myokymic discharges of the tongue after radiation to the head and neck.

We report two patients who developed myokymic discharges of the tongue after radiation therapy. One patient had carcinoma of the nasopharynx, and the other had a chemodectoma of the middle ear. Radiation therapy may damage the hypoglossal nerve and cause myokymia of the tongue, similar to myokymic discharges associated with brachial plexopathies after radiation therapy.

Postpartum neuralgic amyotrophy.

We report 11 women with at least one episode of neuralgic amyotrophy occurring postpartum. One woman had three episodes-two postpartum and a third occurring after minor foot surgery. Two others had a second episode, one following a first trimester spontaneous abortion and one after a viral syndrome. Pain followed delivery from as little as 1 to 2 hours to up to 2 weeks and usually lasted from a few weeks to several months. Weakness, if delayed, followed onset of pain by 2 or 3 days up to 5 weeks. Four of the episodes were bilateral. Clinical weakness and electrodiagnostic findings varied widel

Bifid median nerve compression due to a musculotendinous anomaly of FDS to the middle finger.

A case is presented of a bifid median nerve whose longest portion had a normal course while the other portion passed through a hole in the FDS tendon of the middle finger, at its musculotendinous junction. This caused nerve compression during muscle contraction, producing pain and dysaesthesia in the middle finger suggesting carpal tunnel syndrome.

Sensitivity of the various tests for the diagnosis of carpal tunnel syndrome.

The electrophysiological results of a prospective study of 150 successive cases of carpal tunnel syndrome in 96 patients are reported to evaluate the sensitivity of nine different electrophysiological tests and thresholds for the diagnosis of carpal tunnel syndrome. The distal motor latency was > 5 ms in 35% of cases and > or = 4 ms in 55% of cases. The distal sensory latency from the PIP joint of the middle finger was up to 3.4 ms in 61% of cases. The middle finger orthodromic conduction velocity (14 cm) was < or = 45 m/s in 66% of cases and the orthodromic conduction velocity after palmar st

Preservation of the masseter reflex in Friedreich's ataxia.

In Friedreich's ataxia, the muscle stretch reflexes are absent or hypoactive in the upper and lower extremities because of pathologic involvement of the dorsal root ganglion cells. Paradoxically, the masseter reflex remains unaffected and may even be hyperactive in some cases. The masseter reflex is hypothesized as unique among stretch reflexes in that its afferent nerve cell body (the mesencephalic nucleus) is located intra-axially within the brainstem rather than in the craniospinal ganglia, where the afferent nerve cell bodies subserving the other stretch reflexes are located. This study su

[Multifocal demyelinating neuropathy after tetanus vaccine].

A 39 year old man presenting multifocal demyelinating neuropathy (MFDN) is reported. Fifteen days before onset he had been vaccinated with 75 IU of tetanus toxoid. Although recent histories of vaccination or viral infection support the immunological hypothesis proposed for chronic acquired demyelinating neuropathies (CADN) it is reported for the first time in MFDN, suggesting that MFDN and CADN may share common pathophysiological mechanisms.

An electrophysiological study of the posterior abdominal vagus nerve in man.

1. The compound action potential was recorded in segments of posterior abdominal vagus removed from patients undergoing elective vagotomy and the conduction velocities of the major components were measured. 2. The conduction velocities ranged from 0.15 m/s to 3.9 m/s. The majority of compound potentials recorded had latencies appropriate to C fibre conduction.

Pneumothorax: a complication of nerve conduction studies using needle stimulation.

Two patients developed left pneumothorax after nerve conduction studies were done as part of their evaluation for left thoracic outlet syndrome (TOS). Chest x-rays indicated a 20% and 10% left pneumothorax, respectively. Stimulation was accomplished at Erb point with a 1 1/2-inch needle electrode after unsuccessful attempts at percutaneous stimulation. Because of this complication, this procedure has been discontinued and only percutaneous stimulation at Erb's point is being used. The diagnosis of TOS is determined by other criteria.

Electromyographic and histologic paraspinal abnormalities in polymyositis/dermatomyositis.

A 2-year study was undertaken which identified 60 patients who had an abnormal electromyogram (EMG), muscle biopsy, and/or clinical examination suggestive of myopathy. The clinical records were reviewed, and 12 patients were found to fulfill the criteria of Bohan and Peter for definite diagnosis of polymyositis or dermatomyositis. Electromyography was performed on distal and proximal muscles including the paraspinals on 1 side of the body. Biopsies were taken contralaterally from muscles determined to be abnormal from electrodiagnostic tests. The most significant finding was the degree of invo
