Care of the pediatric near-drowning victim. A nursing challenge.

In the pediatric near-drowning victim, hypoxia and related pathophysiologic factors affect the pulmonary, neurologic, cardiovascular, gastrointestinal, and renal systems. Other complications arise from the potential for infection; the immobile state; and anxiety and fear related to the hospitalization. Altered coping mechanisms and knowledge deficits related to home care and drowning prevention offer problems that the family must overcome during the recovery of their child from the near-drowning insult. Using this knowledge and the accurate, comprehensive ongoing nursing assessments, the nurse

Paraneoplastic syndromes.

The neuromuscular complications of systemic cancer include direct effects (compression or infiltration of peripheral nerve tissues by tumor cells), effects of radiation and chemotherapy, and remote effects. Remote effects constitute the paraneoplastic syndromes--a heterogeneous group of disorders that include motor neuron disease, axon-loss and demyelinating polyneuropathies, Lambert-Eaton myasthenic syndrome, and inflammatory myopathies.

Attention deficits and hyperactivity.

The precise nature of the relationship among attention deficit, hyperkinesis, and specific learning disabilities remains a mystery. They are encountered in one another's company with far greater than chance frequency. It is clear, on the one hand, that attention deficit and learning disability aggravate one another; that is, that what is difficult to learn is difficult to attend to, and vice versa. Furthermore, in children at least, attending ability and activity level have a reciprocal relationship; that is, improvement of attention tends to reduce activity level, and vice versa. On the other

Sequential trigeminal sensory neuropathy.

In a patient presenting bilateral sensory neuropathy, sequential involvement of the two sides of the face with spontaneous recovery on the initially altered side is a peculiar feature which emphasizes the benign character of this unusual condition. Provided that repeated clinical evaluation discloses no underlying pathological process, sensory trigeminal neuropathy may be considered a self-limited benign condition, comparable to Bell's palsy.

[Therapy in neurology. Part 8: Diagnosis and treatment of vertigo].

Acute vertigo episodes in patients suffering from Menière's disease, vestibular neuropathy or ischaemic attacks in the vertebrobasilar system are reason for hospitalization of these patients, if possible in intensive care units. Improvement of microperfusion, administration of antivertigenous drugs and digitalization are most important. Usual clinical symptoms are spontaneous nystagmus and ataxia, seen only if the patient is able to be out of bed. Cranial computed tomography, x-ray examinations of the skull and an internal check-up are basis for differential diagnosis and treatment is necessar

Brain MRI and clinical exam findings in women with multiple gadolinium-based contrast agent (GBCA) exposures due to screening breast MRIs.

OBJECTIVES: Multiple exposures to gadolinium-based contrast agents (GBCAs) is known to be associated with gadolinium deposition in the brain in certain patients. Such deposition has been correlated with specific brain MRI findings, although most available data is in patients with underlying neurologic disorders. We aim to prospectively evaluate brain MRI signal changes as well as neurologic and neuropsychologic testing results in women undergoing screening breast MRI. METHODS: In this IRB-approved, HIPAA-compliant prospective study, 9 women with 5 or more exposures to linear and/or macrocycli

Magnetic resonance imaging of the lumbosacral spine in children with chronic constipation or non-retentive fecal incontinence: a prospective study.

OBJECTIVE: To determine the prevalence of lumbosacral spine (LSS) abnormalities in children with defecation disorders, intractable constipation, or non-retentive fecal incontinence (NRFI) and evaluate whether LSS abnormalities on magnetic resonance imaging (MRI) are clinically detected by neurologic examination. STUDY DESIGN: MRI of the LSS and complete neurologic examination by a pediatric neurologist blinded to the MRI results were performed in patients with intractable defecation disorders. RESULTS: Patients with intractable constipation (n = 130; 76 males; median age, 11 years; range, 6-…

Providing trauma-informed neurological care across clinical settings.

Background: Trauma is a far-reaching experience, with consequences that can extend long after the inciting event. The link between adverse childhood events and neuropsychiatric conditions are relevant to providers performing a neurologic exam, as a person's trauma can influence their medical diagnoses (as in functional neurologic disorder) and their receptivity to medical care. Objective: To provide a framework for clinicians to approach neurologic exams and procedures in a trauma-informed manner.Method/Results: Both prior to and during the visit, every effort should be made to create a welcomi

The International Standards for Neurological Classification of Spinal Cord Injury: Classification Accuracy and Challenges.

BACKGROUND: Successful utilization of the International Standards for Neurological Classification of Spinal Cord Injury (ISNCSCI) requires a comprehensive understanding of its rules, terminology, and several complex concepts. There have been no studies investigating classification accuracy since the newest ISNCSCI revision (2019). OBJECTIVES: To evaluate classification accuracy of SCI professionals using the 2019 ISNCSCI edition, identify common mistakes and areas of confusion, and assess associations between experience in ISNCSCI classification and performance. METHODS: Members of the Inter

Utilization of Neurophysiological Classification Systems in Determining Interventions for Patients with Carpal Tunnel Syndrome.

BACKGROUND: Median mononeuropathy at or distal to the wrist, or carpal tunnel syndrome (CTS), is the most common peripheral nerve compression disorder in the upper extremity. Neurophysiological classification systems for patients with CTS have been developed and implemented to provide health care providers an enhanced system of electrophysiological evaluation with a grading scale, so that they may evaluate their patients with CTS within a system that confers relative severity. Electrophysiological data collected within these classification systems includes either nerve conduction studies (NCS)
