Eighteen patients with dural arteriovenous fistulas or intradural arteriovenous malformations underwent clinical and neurophysiological examination. Bladder disturbances, pain, sensory abnormalities and involvement of both upper and lower motor neurons were commonly observed. Abnormal findings were obtained both in electromyography (11/18) and somatosensory evoked potentials (16/18). The motor evoked potentials were abnormal in all but one patient and showed a prolonged central (n = 14) or peripheral motor conduction time (n = 6). In three cases both values were prolonged. The results of nerve

58 patients with trigeminal nerve neuralgia (TNN) were examined in terms of somatosensory evoked potentials (SSEP). The increase of latence as well as the decrease of contra- and ipsilateral SSEP components amplitude were observed in the interval N13-P45. Either bylateral hypersynchronization or desynchronization of SSEP were observed at affect side of face during TNN paroxismal period. It was believed that both variations of entrance of two currents of impulses which had been caused either by face electrostimulation or by pathological trigeminal irritation in brain cortex. Neurologically critically ill patients, more often than others, are unable to communicate and, for a crucial period of time, have the vital functions of their brains hidden in the "black box" of the cranial vault behind a curtain of ambiguity and immobility. Customarily--and naively--we have relied upon beside clinical observations to pierce these barriers. Recent insights lead us to conclude that these "neurochecks" no longer suffice. This article has surveyed four major monitoring systems relied upon by neurointensivists to evaluate the pathophysiology of their patients. Of these, ICPM has t

The last decade has seen a rapid growth in surgical techniques directed at resection of skull base tumors. Lesions that were previously inoperable can now be resected either partially or in some cases completely. The morbidity in these procedures has diminished with increased surgical experience. Additional aids to successful surgical resection include intraoperative neurophysiologic monitoring techniques. The following text outlines methods of monitoring cranial nerve and brain function in a manner that is directly relevant to cranial base surgery in which so many structures are potentially a

Using brainstem auditory evoked potentials and tympanometry the authors have examined 120 children aging from several months to five years and they have found a high percentage of children with hearing impairment with sensorineural deafness. The children had also secretory inflammation of the middle ear. It has been suggested that secretory inflammation of the middle ear deteriorates the basic hearing impairment because of which it should be early diagnosed and surgically treated. Owing to the complex problem of children deafness, the authors recommend that several methods of examination shoul

There are dramatic quantitative and qualitative differences in the locomotor behavior of larval and juvenile frogs. Larvae (tadpoles) are primarily herbivourous and rely heavily on locomotion via undulations to acquire food and avoid predation. After metamorphosis, juvenile frogs adopt a carnivorous lifestyle and capture prey and avoid predators by remaining motionless in a place of concealment. When they must move, frogs locomote by means of ballistic hops or by more conventional walking. However, locomotion of both tadpoles and frogs can be considered of two fundamental functional types: (a)

Ten patients with dystrophia myotonica (8 adults and 2 prepubertal children), from three unrelated families, were investigated for diurnal sleepiness, using a sleep questionnaire and multiple sleep latency test (MSLT). Immunogenetic study was also carried out to assess the involvement of HLA region genes in modulating susceptibility to excessive diurnal sleepiness (EDS). EDS was reported by 5 patients and confirmed in each case by MSLT. In the whole patients group, mean daytime sleep latency was significantly shorter than in healthy controls matched for age and sex. At clinical or neurophysiol

Ninety six nulliparous women were investigated to establish whether childbirth causes damage to the striated muscles and nerve supply of the pelvic floor. The techniques used were concentric needle electromyography (EMG), pudendal nerve conduction tests and assessment of pelvic floor contraction using a perineometer. There was EMG evidence of re-innervation in the pelvic floor muscles after vaginal delivery in 80% of those studied. Women who had a long active second stage of labour and heavier babies showed the most EMG evidence of nerve damage. Forceps delivery and perineal tears did not affe

We studied 31 patients with acute herpes zoster (AHZ) less than 28 days' duration. Clinical characteristics (pain, allodynia, course of disease) and somatosensory perception thresholds (thermal discrimination, hot pain, and vibration) of the affected dermatome and the contralateral homologous area were assessed. Touch-evoked allodynia was found in 17 (55%) and dysesthesia in a further 5 (16%). Thermal and vibration perception thresholds demonstrated significant elevations when compared to the contralateral side. Thermal threshold abnormalities were significantly associated with the prevalence

Various neurophysiological methods, including electroencephalography, electromyography, nerve conduction velocities, and evoked potential techniques, have been used to detect early signs of neurotoxicity in humans. These methods have been applied to groups of occupationally exposed workers and their referents in epidemiologic studies, to patients with suspected or proven diseases after long-term work in toxic environment, and to human subjects during or after experimental exposure. The main body of knowledge arises from epidemiologic studies of occupationally exposed subjects, and several chem

1. Neurophysiologic testing is as accurate as CT scanning or myelography in the diagnosis of root compression due to disk disease. 2. The most reliable findings are abnormal H reflex, and positive sharp waves or fibrillations, or both, in the appropriate muscles. 3. Neurophysiologic testing may be more accurate than clinical examination or radiologic study in patients with canal stenosis. 4. Symptomatic post-laminectomy patients present a difficult diagnostic problem, even when all available methods are used.

In 3 cases of xeroderma pigmentosum showing signs of peripheral neuropathy, electrophysiological studies were made. In one of them, a sural nerve biopsy was performed. MCV obtained from the lower limbs were moderately reduced in all cases, and SCV could be obtained in only 1 case from the median nerve. Auditory brain stem responses were not obtainable in all cases. Sural nerve biopsy revealed a marked decrease of myelinated fibers and also suggested severe damage in both myelinated and unmyelinated fibers. Teased fiber study showed myelin ovoids and indicated axonal degeneration. Sensory nerve

Sixteen autogenous sural nerve grafts used for ulnar and median injuries in the forearm have been studied neurophysiologically up to two and a half years after operation. Motor and sensory nerve conduction studies revealed a slow but sustained improvement during the follow-up period. By two years, motor conduction velocity across the graft itself reached in most cases 40 to 85% of the conduction velocity in the contralateral normal limb. Some reduction of motor conduction velocity was observed in the uninjured nerve proximal to the graft but this was less marked. Sensory nerve action potential

In attempting to understand the mechanism of pain production in tic douloureux, one must account for the myelination pathology seen in the primary afferent fibers, the cases where trigger is in a different division than the pain, the frequent lack of a fixed neurologic deficit, the effective trigger stimuli corresponding to large caliber axons which would not seem to involve the small axons usually associated with pain production, and similar puzzling features of the disease. We present a theory which satisfactorily predicts, or is consistent with, most known features of tic; it is based upon

Fatigue is one of the most frequent and disabling non-motor symptoms in Parkinson's disease (PD). Its multidimensional nature and overlap with related symptoms contribute to diagnostic and therapeutic challenges. A distinction has been proposed between fatigue, defined as the subjective perception of lack of energy and fatigability, defined as an objective decline in performance during sustained activity. However, their underlying pathophysiological mechanisms are still unclear. We conducted a narrative review of studies investigating PD-related fatigue and fatigability with neurophysiological

Individuals with depression often engage in iterative "rumination" about challenging situations and potential outcomes. Although the state of rumination has been associated with diverse univariate neurophysiological features, the potential to use multivariate patterns to decode it remains uncertain. In this proof-of-principle study, we trained participant-specific linear support vector machines to differentiate state rumination from distraction using patterns in the alpha, beta, and theta bands, as well as inter-channel connectivity. We used validated tasks to induce rumination or distraction
