Assessment of neurosurgical workforce, infrastructure, and service delivery capacity in East Africa: A collaborative survey.

Workforce development in healthcare is critical, especially in resource-limited regions such as East Africa, where a scarcity of neurosurgeons hinders patient care. This study aims to analyze neurosurgical workforce trends in Kenya, Rwanda, Tanzania, and Uganda over two decades, informing strategies for capacity enhancement. An online survey targeting neurosurgical institutions across the four countries was conducted, focusing on consultants, residents, and general practitioners for the target population. Traumatic brain injury (TBI) served as a proxy for neurosurgical conditions. Data on work…

Spina bifida transition care in India: strengths amidst challenges.

In India, adult neurosurgeons are required to care for children regularly because the concept of dedicated pediatric specialty care is not yet entirely established in the subcontinent. Likewise, pediatric neurosurgeons do not exclusively offer their services to the young, but they also provide care to adult patients with neurosurgical disorders. This creates a medical system where the transition between specialties is not often a formal and recognized aspect of neurosurgical care because most neurosurgeons provide care for patients of all ages. Additionally, there are very few teams geared tow…

LONG-TERM NATURAL HISTORY OF GIANT NULL CELL PITUITARY ADENOMA.

Pituitary adenomas that extend to the ventricular system are extremely rare. We present a 5-year natural history of a giant null cell pituitary adenoma with invasion into the cavernous sinus extending to the third ventricle. MRI series that were available could be useful for neurosurgeons, ophthalmologists, and endocrinologists as well as radiologists. Patients with the diagnosis of pituitary adenoma that are certain according to the radiological and clinical examination should be consulted by a neurosurgeon experienced in endoscopic endonasal surgery, a neuroendocrinologist, and an ophthalmol…

Sensitivity of high-resolution three-dimensional magnetic resonance angiography and three-dimensional spoiled-gradient recalled imaging in the prediction of neurovascular compression in patients with hemifacial spasm.

OBJECT: Hemifacial spasm is a clinical syndrome caused by vascular compression of the facial nerve in the cerebellopontine angle, which can be relieved by surgical intervention. Advances in medical imaging technology allow for direct visualization of the offending blood vessels in hemifacial spasm and similar conditions (such as trigeminal neuralgia). The utility of high resolution 3D MR angiography and 3D spoiled-gradient recalled (SPGR) imaging sequences for surgical decision-making in hemifacial spasm, as measured by sensitivity, specificity, and positive and negative predictive values, has

History of vestibular schwannoma surgery.

Attempts of surgical removal of vestibular schwannomas started 150 years ago with major limitations in terms of diagnosis and understanding of the disease but also in respect of surgical technique and instrumentation. Then came Cushing followed by Dandy, two pioneers and legendary neurosurgeons who understood the natural history of the disease and set the landmarks of the current surgery of the cerebellopontine angle. In this century of medicine, results and expectations shifted from a life-threatening affection to the actual standard of cranial nerve preservation and conservation of quality o…

Technical aspects and considerations of deep brain stimulation surgery for movement disorders.

Deep brain stimulation (DBS) represents one of the more recent advancements in Neurosurgery. Even though its most successful applications evolved in movement disorders (MDs), indications now include pain, psychiatric disorders, epilepsy, cluster headaches and Tourette syndrome. As this type of surgery gains popularity and the indications for DBS surgery increase, so it will certainly increase the number of neurosurgeons who will use this neuromodulatory technique. A detailed description of the technical aspects of the DBS procedure, as it is performed in our department, is presented. In our op…

Reinitiation of Anticoagulation After Surgical Evacuation of Subdural Hematomas.

BACKGROUND: Chronic subdural hematoma (cSDH) is an increasingly common condition due to the growing use of anticoagulation. Currently, there remains a lack of evidence to guide the optimal timing of anticoagulant reinitiation for stroke prevention in atrial fibrillation after cSDH evacuation. We aimed to better understand the perceived risks of hemorrhagic and embolic complications along with current practice patterns on restarting anticoagulation after surgical evacuation of cSDH. METHODS: We conducted a survey of Canadian neurosurgeons and stroke neurologists using a novel self-administered…
