Exstrophy of the bladder is a rare congenital defect that occurs when the abdominal wall and underlying structures, including the ventral wall of the bladder, fail to fuse in utero. As a result, the lower urinary tract is exposed, and the everted bladder appears through the abdominal opening. Various surgical interventions have been employed with variable success in the hope of achieving complete dryness, full control over delivery of urine, freedom from catheters and external appliances, and a protected upper urinary tract. The most popular surgical approach is the primary bladder closure wit

BACKGROUND: Treatment decisions for apnoea of prematurity (AOP) are usually based on nursing staff's documentation of pulse oximeter and heart rate alarms.
OBJECTIVE: In an observational study, to compare the accuracy of oxygen saturation (SpO2) and heart rate alarm documentation, and the resulting interventions by nursing staff, with objectively registered events using polysomnographic and video recording.
METHODS: Data on 21 preterm neonates (12 male) with a diagnosis of AOP were analysed. Nursing staff's desaturation (<80% SpO2) and bradycardia (<80/min) alarm documentation was compared w

This article has reviewed current thought on the pathophysiology, medical management, and nursing implications of PDA in the premature newborn. The ductus arteriosus is a normal vascular channel that provides a route for blood flow to the descending aorta in the fetus; and it is an abnormal channel in the newborn that allows additional pulmonary blood flow to be shunted from the higher pressured aorta. Left heart volume overload and additional insults in connection with concurrent RDS and BPD were discussed. Current management for closure advocates indomethacin administration, and ligation, sh

Effective communication among health care team members is a mainstay of patient safety, especially in a neonatal ICU (NICU), given small errors can have serious and life-threating consequences. Ineffective communication with families of hospitalized children can lead to decreased satisfaction and trust in the health care team. To enhance communication, the NICU nursing staff at the Johns Hopkins Children's Center spearheaded an initiative to create an enhanced nursing role in multidisciplinary patient rounds. Education of the nursing staff and other team members and the development of a roundi

While the methods of establishing and maintaining organ perfusion differ from one clinician to the next, the underlying physiological rationale remains constant. The gestalt for correcting circulatory compromise is generally performed in a stepwise manner; first ensuring that the vasculature is filled, then administering medications to tighten the vasculature, and lastly, compensating for an immature vasculature. This stepwise approach is reflected in the pharmacological interventions of providing fluid boluses (filling the pump), giving catecholamines (tightening the pump), and starting hydro

BACKGROUND: Nurses in neonatal intensive care units (NICUs) face significant emotional and clinical demands affecting caring behaviours. Psychological empowerment is linked to improved nursing outcomes, yet evidence from conflict-affected settings remains limited.
OBJECTIVE: This study examined the relationship between psychological empowerment and caring behaviours among NICU nurses in the West Bank, Palestine and identified predictors of caring behaviours.
METHODS: A multicentre descriptive correlational cross-sectional design was used. In total, 178 NICU nurses across 12 hospitals partici

UNLABELLED: The number of newborns with neonatal opioid withdrawal syndrome is alarming, due to the increase in maternal opioid use. Despite the critical health and socio-economic impact, a precise clinical and therapeutic approach for opioid using infants and mothers remains undefined.
AIM: The main objective is to update the therapeutic approach and the role of nursing in neonates with neonatal opioid withdrawal syndrome due to mothers who use opioids.
DESIGN: We conducted a systematic review, as a meta-analysis was not feasible due to heterogeneity across study designs and outcomes, in ac

BACKGROUND: Fluid overload-a critical risk in pediatric/neonatal care-is often driven by excessive fluid during medication dilution, increasing complications like acute kidney injury(AKI), prolonged ventilation, and mortality.
METHODS: A 2024 cross-sectional study across 14 pediatric units (PICUs, NICUs, surgical/general wards) evaluated fluid administration practices and nurses' knowledge. Observations included 150 patients and 200 nurses administering 8 medications. Data on fluid volumes, infusion durations, compatibility, and storage compliance were collected via checklist and questionnair

3367 consecutive births were reviewed prospectively. Population belongs mainly to a community with relatively underprivileged living conditions. Perinatal mortality was found at a rate of 22.4/1000 B. Early neonatal mortality formed 6.66/1000 B and stillbirth formed 15.83/1000 B. Low birth weight rate was 5.43% of live birth. Analysis of our findings suggests the need to improve follow-up during gestation to avoid complications resulting in macerated stillbirths, and to review the routine of follow-up and care in the immediate period before delivery, during delivery, in the immediate post part

BACKGROUND: As part of an ongoing study about nurse staffing during labor and birth sponsored by the Association of Women's Health, Obstetric, and Neonatal Nurses (AWHONN), outcomes that may be linked to aspects of labor nursing were considered. The purpose of this study was to see if labor nurses felt they influenced whether a woman has a cesarean birth. These data were used to determine if cesarean birth should be included as an outcome measure in the multistate labor nurse staffing study.
METHODS: Focus groups were used to explore the role of labor nurses and cesarean birth. Participants w

The first part of this two part series on the neonatal energy triangle gives a general overview of the transition period during the first six to ten hours of life. Although all elements of the triangle, hypothermia, hypoglycaemia and hypoxia, are interlinked this first part of the series describes the normal metabolic adaptation at birth and the difficulties involved in recognising and treating hypoglycaemia. In the second part of the series the two other elements of the triangle, hypoxia and hypothermia, will be addressed.

BACKGROUND: Nurses and other hospital workers are exposed to antineoplastic drugs during daily activities. Previous studies suggest that antineoplastic drugs at occupational exposure levels may be toxic to reproduction, but these studies are not consistent or conclusive.
METHODS: Self-administered questionnaires were completed by 4393 exposed and nonexposed nurses employed between 1990 and 1997 (79% response). Questions were asked about pregnancy outcome, work-related exposures, and lifestyle. Exposure to antineoplastic drugs was estimated using task-based dermal exposure measurements and sel

The relative size of a neonate impacts many aspects of prenatal and postnatal surveillance and care. The designations of appropriate for gestational age, small-for-gestational age, intrauterine growth restriction, and large-for-gestational age are systematic categorizations used to assess and monitor growth throughout pregnancy and delivery. Each abnormal growth descriptor aids in anticipating neonatal needs after birth because each has the potential for complications related to feeding, glucose utilization, short- and long-term growth, and development. Maternal risk factors that impact the ne

Although preterm labor and birth continues to plague us as an unsolved mystery in maternal/child health, much has actually been learned over the past few decades. Most reviews of the preterm labor literature examine the research in an interdisciplinary manner; this article instead looks at the active role that nurses have taken in the research concerning preterm labor and birth. This article will categorize research conducted by nurses over the past 30 years on preterm labor and birth, emphasizing some of the changes in nursing practice engendered by nursing's important research effort.
