Providing nutritional support to patients with thoracic cancer: findings of a dedicated rehabilitation service.

BACKGROUND: National guidelines recommend screening patients with thoracic cancer to identify those requiring nutritional support. To help quantify this area of need, the associated workload and explore its impact, we report findings from a dedicated rehabilitation service. METHODS: Patients were screened soon after diagnosis to determine the prevalence of malnutrition, and various aspects compared between malnourished and not malnourished groups. A nutritional care plan was instigated and all contacts recorded, together with follow-up body weight. RESULTS: Of 243 patients seen, 35% were mal…

Metabolic response and nutritional support in traumatic brain injury: evidence for resistance to renutrition.

Abstract Traumatic brain injury (TBI) is one of the most severe injuries encountered in intensive care units. TBI patients exhibit protein wasting and gastrointestinal dysfunction, which may be risk factors for a septic state. Specific nutritional support may be required for these patients, and we hypothesize that standard nutritional support does not allow restoration of the nutritional state of TBI patients. A well-validated rat model of TBI by fluid percussion was used. Rats were randomized into three groups: healthy rats receiving standard chow diet ad libitum (AL), rats sustaining TBI and…

Should patients with cancer be offered nutritional support: does the benefit outweigh the burden?

Nutrition support has been widely advocated as adjunctive therapy for a variety of underlying illnesses, including surgery and medical oncotherapy (radiation or chemotherapy for cancer). Both parenteral and enteral nutrition have been mistakenly viewed as feeding, when, in fact, they are medical interventions with associated risks and costs. The argument that nutrition support has to be provided to patients to prevent 'starving to death' confuses the difference between dying in a malnourished state and dying as a direct consequence of nutrient deprivation; cancer patients fit into the former c

Early versus late enteral nutritional support in adults with burn injury: a systematic review.

BACKGROUND: Burn injury increases the body's metabolic demands, and therefore nutritional requirements. Provision of an adequate supply of n … (from 1982) and EMBASE (from 1980). RESULTS: The trial evidence about the benefit of early enteral nutritional support on standardized clinical outcomes such as length of hospital stay and mortality remained inconclusive. Similarly, the question of whether early enteral feeding influenced or decreased metabolic rate, reduced septic and other complications remained u …

[Nutritional support in a pregnant woman with brain death. Case report and literature review].

The occurence of brain death in pregnancy represents a catastrophic entity although infrequent. The aims of continue medical management are focused in a double purpose: to preserve intrauterine product's life and fetal maturation until delivery and to consider the mother as a potential organ donor. Ethical considerations together with gestational age, fetal well being and relatives' wishes are cardinal for continuing medical support. Modern critical care units allow us to obtain favourable results, supported in scientifical reports that describe successful outcomes. Nutritional aspects plays a

Nutritional support and quality of life in stable chronic obstructive pulmonary disease (COPD) patients.

RATIONALE: Nutritional depletion is a common problem in chronic obstructive pulmonary disease (COPD) patients. It is caused, to a large extent, by an imbalance between low-energy intake and high-energy requirements. This problem adversely affects morbidity and mortality. However, the use of nutritional supplements to reach their energy necessities requires optimisation between positive and adverse effects on outcome before being used systematically as part of their comprehensive care. PURPOSE: The aim of our study was to investigate the effects of oral nutritional repletion on quality of life

Therapeutic approaches to malnutrition in chronic dialysis patients: the different modalities of nutritional support.

Protein-energy malnutrition (PEM) is a common complication in maintenance hemodialysis and chronic peritoneal dialysis patients and is a powerful predictor of morbidity and mortality. Although this association does not prove that malnutrition is a cause of this increased morbidity and mortality, it is consistent with this possibility. There are a number of modalities of nutritional support for the prevention or treatment of PEM in maintenance dialysis patients. Routine methods include preventing PEM before the onset of maintenance dialysis therapy, dietary counseling, maintenance of an adequat…

[Clinical benefit of nutritional assessment and support in patients with chronic obstructive pulmonary disease].

Weight loss is common in patients with chronic obstructive pulmonary disease (COPD). Comprehensive nutritional assessment was conducted in two large groups of patients with COPD who were enrolled in the Respiratory Failure Research Program sponsored by the Japanese Ministry of Health and Welfare and the Kinki COPD Research Group. The incidences of mild malnutrition (%IBW < 90%) were 74% and 62%, respectively. The incidences of hypoalbuminemia were low: 10.0% and 6.5%, respectively. The incidence of imbalance in plasma amino acids, which was defined as an abnormally low BCAA/AAA ratio, was as h

The use of bioelectrical impedance analysis for monitoring body composition changes during nutritional support.

Body composition was measured with bioelectric impedance analysis (BIA) in 30 patients with protein malnutrition following biliopancreatic diversion. Determinations were carried out prior to, during, and at the completion of intravenous nutritional support when the nutritional parameters had completely reverted to normal. Before treatment, body weight (BW), lean body mass (LBM), and body fat (BF) values were similar to those of controls, whereas the total body sodium/total body potassium (TBNa/TBK) and extracellular mass/body cell mass (ECM/BCM) ratios were considerably higher. During the supp…

The case for nutritional support. Eliminating hospital-acquired malnutrition improves outcomes and reduces costs.

Taking steps to eliminate hospital-acquired malnutrition in patients with disease or trauma significantly improves treatment outcomes. Patie … n appropriately selected patients will decrease morbidity and length of stay. Hospitals that ignore nutritional support therapy for patients may be in violation of quality-of-care standards. And hospitals that do not have hyperalimentation or nutritional support teams, or that have disbanded such teams for economic reasons, are putting their critically ill patients a …

[Clinical and laboratory evaluation of the efficacy of parenteral nutritional support in malnourished elderly].

The study was carried out on twenty nine geriatric patients with malnutrition of variable aetiology. The effectiveness of a temporary parenteral nutritional supplement was evaluated both clinically and by means of some metabolic indexes: nitrogen balance, serum albumin, blood electrolytes, acid-base parameters, hemocytometry. The treatment was devoid of metabolic complications, and promoted a positive nitrogen balance as well as clinical improvement in most patients.

Nutritional support and measured energy expenditure of the child and adolescent with head injury.

Energy expenditure, nitrogen excretion, and serum protein levels were studied from the time of hospital admission until 2 weeks after severe head injury in eight adolescents and four children with peak 24-hour Glasgow Coma Scale scores ranging from 3 to 8. The mean measured energy expenditure (MEE) was 1.3 times Harris and Benedict's predicted value for energy expenditure. Seventy percent of the patients achieved caloric balance (MEE X 1.2) by 4 to 14 days after injury, but balance was not consistently maintained. Five of the 12 patients had intermittent diarrhea, and two had increased gastric

Enhanced mobilization of iron from body stores in malnourished patients during intravenous nutritional support.

Abnormal iron metabolism, characterized by low serum iron concentration and diminished saturation of iron-binding capacity, is known to occur in patients with malignant or inflammatory diseases. In this study, serum iron concentration and iron-binding capacity were measured in 21 patients with malignant disease, trauma or inflammatory illness before and during intravenous nutritional support without iron supplementation. Prior to nutritional support, the serum iron value averaged 26.0 +/- 3.5 micrograms per deciliter and the iron-binding capacity, 160.0 +/- 14.0 micrograms per deciliter. After…

Total body potassium depletion and the need for preoperative nutritional support in Chrohn's disease.

Significant intracellular potassium depletion was documented in 57 patients with Crohn's disease by measurements of total body potassium, bo … aimed at normalizing serum albumin and other standard serum parameters. In conclusion, preoperative nutritional support in Crohn's disease is recommended for patients with a total body potassium level less than 70% of normal. If whole body counting for direct measurement of total body potassium is not available, a Crohn's Disease Activity Index above 225 is proposed …

Association of phosphate decline after nutritional support with extubation failure in critically ill patients.

BACKGROUND & AIMS: While hypophosphatemia is known to complicate weaning, the impact of early phosphate dynamics following nutritional support remains unclear. This study aimed to determine the association between the relative rate of phosphate decline after initiation of nutritional support and extubation outcomes in critically ill patients. METHODS: This retrospective cohort study was conducted in the medical intensive care unit of a tertiary academic hospital. Adult patients receiving invasive mechanical ventilation who underwent planned extubation after initiation of enteral and/or parent…

The Effect of Protein Nutritional Support on Inflammatory Bowel Disease and Its Potential Mechanisms.

Inflammatory bowel disease (IBD), a complex chronic inflammatory bowel disorder that includes Crohn's disease (CD) and Ulcerative Colitis (UC), has become a globally increasing health concern. Nutrition, as an important factor influencing the occurrence and development of IBD, has attracted more and more attention. As the most important nutrient, protein can not only provide energy and nutrition required by patients, but also help repair damaged intestinal tissue, enhance immunity, and thus alleviate inflammation. Numerous studies have shown that protein nutritional support plays a significant…

Nutritional Support of Crocin on Neurobehavioral Disabilities Induced by Cisplatin-Based Chemotherapy in Rats.

Cisplatin, a chemotherapy drug containing platinum, is considered a neurotoxic agent. On the other hand, crocin, the primary component of saffron, possesses neuroprotective and antioxidant properties. In this study, 28 healthy adult male Wistar rats weighing 200-250 g were used (6-7 weeks old). Rats were divided into a control group (Ctr), a crocin group (Cro), a cisplatin group (Cis), and a crocin with cisplatin group (Cro + Cis). Rotarod, open field, and shuttle box tests were performed to assess balance, explorative behavior, and avoidance memory. After behavioral testing, the hippocampus w
