Hypertension and obesity after pediatric kidney transplantation: management based on pathophysiology: a mini review.

Hypertension after pediatric renal transplant is a common and important risk factor for graft loss and patient survival. The mechanism of post kidney transplant hypertension is complex and multifactorial. Control of blood pressure in renal transplant patients is important but often times blood pressures remain uncontrolled. The management of hypertension and obesity in pediatric kidney transplant patients is based on the pathophysiology. Compared to the general pediatric hypertensive population, special attention needs to be focused on the additional impact of immunosuppressive medications sid

Management of obesity in the National Health and Nutrition Examination Survey (NHANES), 2007-2008.

PURPOSE: The prevalence of obesity has been increasing in the United States. We set out to investigate the use of pharmacologic and non-pharmacologic therapy for the treatment of obesity in recent years. METHODS: We included 2630 men and 2702 women who took part in the National Health and Nutrition Examination Survey from 2007 to 2008. We analyzed their demographic and anthropometric data and their weight and drug history. RESULTS: A total of 45.9% of men and 45.0% of women were candidates for treatment (body mass index ≥30 kg/m(2), or ≥27 kg/m(2) with risk factors). Among these participants

A Systematic Review of the Management of Maternal Obesity in Pregnancy: Antenatal Management, Outcomes, and Long-Term Implications on Maternal Health.

Maternal obesity is a looming worldwide health challenge with dire consequences regarding pregnancy outcomes and future maternal and offspring health. This systematic review synthesizes evidence from eight high-quality studies published between 2015 and 2024, discussing the management of maternal obesity during pregnancy, antenatal care strategies, associated perinatal outcomes, and long-term maternal health risks. The review identifies a strong correlation between maternal obesity and the increased chances for gestational diabetes mellitus (GDM), hypertensive disorders, preterm birth, cesarea

Aerobic Training for Obesity Management in Individuals with Down Syndrome: A Bibliometric and Meta-Analyses.

Background/Objectives: Down syndrome (DS), the most common chromosomal disorder, is associated with obesity and related metabolic complications. Although physical activity (PA) improves health outcomes in individuals with DS, global research trends in this field have not been systematically synthesized, and evidence regarding the effects of aerobic training (AT) on obesity-related parameters in individuals with DS remains inconsistent. This study incorporated a dual bibliometric and meta-analytical approach. Methods: First, the bibliometric analysis included 321 original research articles publ

Relevance of endoscopy and Helicobacter pylori infection in the preoperative management of obesity with sleeve gastrectomy.

OBJECTIVES: Obesity has reached pandemic dimensions, representing a large health burden. Conservative therapy has recently been improved, but the most definitive therapy is still surgery. Gastric sleeve is one of the most frequent techniques. Preoperative guidelines sometimes recommend preoperative endoscopy. The aim of this study is to review the preoperative use of endoscopy in bariatric surgery and the possible importance of Helicobacter pylori therapy to avoid surgical complications. PATIENTS AND METHODS: Retrospective case series of patients undergoing sleeve gastrectomy in two large hos

Advances in biomedical applications of vitamin D for VDR targeted management of obesity and cancer.

BACKGROUND: 1,25(OH)2D3 is a fat-soluble vitamin, involved in regulating Ca2+ homeostasis in the body. Its storage in adipose tissue depends on the fat content of the body. Obesity is the result of abnormal lipid deposition due to the prolonged positive energy balance and increases the risk of several cancer types. Furthermore, it has been associated with vitamin D deficiency and defined as a low 25(OH)2D3 blood level. In addition, 1,25(OH)2D3 plays vital roles in Ca2+-Pi and glucose metabolism in the adipocytes of obese individuals and regulates the expressions of adipogenesis-associated gene

Contingency management for smoking cessation for individuals with overweight or obesity: A randomized controlled trial.

BACKGROUND: Interventions for quitting smoking and weight control among individuals with excess weight are scarce. Our study evaluated the effectiveness of cognitive behavioral therapy (CBT) plus contingency management (CM) in this population, and examined whether CM for smoking cessation improved CBT treatment outcomes at end of treatment (EOT) and at 1-, 3-, 6-, and 12-month follow-ups (FU). METHODS: In an 8-week randomized clinical trial, 120 adults who smoke with overweight or obesity (54.16% females; MBMI = 31.75 ± 4.31) were randomly assigned to CBT for both quitting smoking and weight

The Impact of Nutritional Therapy in the Management of Overweight/Obese PCOS Patient Candidates for IVF.

Polycystic ovary syndrome (PCOS) is the most common female endocrine disorder, and it has two main pathological aspects: reproductive and metabolic. Overweight/obesity is a risk factor in terms of adverse effects during hormone stimulation, a reduced response to ovulation induction regimens, reduced success of IVF, and an increased risk of obstetric complications. To resolve this vicious cycle of pathological events, weight loss and lifestyle modifications are promising strategies. Among these possible approaches, the consumption of a very-low-calorie ketogenic diet (VLCKD) or Mediterranean di

Anesthesia management of morbid obesity and ankylosing spondylitis with a difficult airway: a case report.

BACKGROUND: The anesthesia management in morbidly obese patients is challenging, because there may also be a number of complications in addition to predictable difficulties associated with airway and ventilation management. Potential complications require careful preoperative evaluation and intraoperative management to ensure rapid recovery and minimize adverse effects from anesthesia. CASE PRESENTATION: This study involved a 39-year-old male patient who had ankylosing spondylitis complicated with morbid obesity (body mass index =48.8). He had severely limited head and neck mobility, severe o

The development of a core outcomes set for self-management interventions for patients living with obesity.

Self-management interventions (SMIs) can improve the life of patients living with obesity. However, there is variability in the outcomes used to assess the effectiveness of SMIs and these are often not relevant for patients. In the context of COMPAR-EU, our aim was to develop a core outcome set (COS) for the evaluation of SMIs for patients with obesity. We followed a four steps multimethod approach: (1) the development of the initial catalogue of outcomes; (2) a scoping review of reviews on patients' values and preferences on outcomes of self-management (SM); (3) a Delphi survey including pati

Musculoskeletal pain profile of obese individuals attending a multidisciplinary weight management service.

Obesity is associated with numerous chronic diseases, including musculoskeletal (MSK) pain, which affects on quality of life (QoL). There is, however, limited research providing a comprehensive MSK pain profile of an obese cohort. This retrospective study used a patient database at a national weight management service. After ethical approval, anonymized patient data were statistically analyzed to develop a pain profile, investigate relationships between pain, sleep, and function, and explore variables associated with having low back pain (LBP) and knee pain. Overall, 915 individuals attended t

Managing skin health in obese children with spina bifida: an overview and case study.

Chronic wound and skin care issues in obese children tend to occur in specific disease-related groups such as patients with neural tube deficits. While the incidence of neural tube deficits has declined as a result of increased awareness about the importance of prenatal folic acid intake, neural tube deficits remain some of the most common congenital anomalies. General care of spina bifida patients must address their physical limitations, structure, decreased sensation, limb weakness, and psychosocial needs related to dependence on others and being "different." Patients and their families also

The complex role of PPARgamma in renal dysfunction in obesity: managing a Janus-faced receptor.

Obesity is frequently accompanied by insulin resistance, type II diabetes, hypertension and atherosclerosis, a cluster of pathologies that are the major components of the metabolic syndrome. Obesity is a known cause for renal dysfunction that leads to two major renal pathologies: hypertension and glomerular and tubulointerstitial injury. Peroxizome proliferator activated receptors (PPARs) are transcription factors belonging to the nuclear hormone receptor superfamily with important functions in the regulation of metabolism. The role of PPARgamma isoforms in adipogenesis and vascular inflammati
