A prospective study researches features of patient-controlled epidural analgesia (PCEA) in 42 women after cesarean section, mid- and high-traumatic gynaecological surgery. It was found out that the quality of while using the PCEA is significantly better than while using a traditional parenteral infusion of analgesics. Use of PCEA favours the circulatory and breathing systems functioning. Compared to continuous epidural infusion of analgesics and anaesthetics, use of PCME returns the expenses better due to less consumption of the drugs up to 35% during the first 24 hours after the surgery. The study presents a literature review on the benefits and risk of the routine episiotomy during the second stage of labour. Perineal trauma complications as well as perinatal outcomes are discussed. The risk of stress incontinence and sexual dysfunction are described. New techniques for improve of perinatal outcomes and prevention of post partum incontinence are described. Routine episiotomy gives poor effects in many cases. Perineal massage during pregnancy, waterbirth, are most interesting methods to avoid routine episiotomy and improve the quality of life in post partum women.

One hundred and forty-one cases of obstructed labour was referred to RG Kar Medical College and Hospital in the last one year. Out of 12,034 total hospital deliveries, incidence of obstructed labour was 1.17% whereas 36 cases (0.29%) accounted for intra-uterine foetal death (IUD). Of the IUD with obstructed labour, 69.4% were delivered by destructive operation and 30.6% by lower segment caesarean section (LSCS). Results with real-time processable three-dimensional ultrasound (US) were assessed in 57 normal fetuses and two fetuses with hydramnios. Volume scanning time was 4 seconds, and a complete three-dimensional fetal image was displayed at the end of scanning. Although this method was limited in some respects, such as viewing direction, it was very useful when performed in conjunction with conventional tomographic US because of its speed and because some components were depicted more clearly.

STUDY DESIGN: A review of the world literature on gas embolism.
CONCLUSION: Gas embolism is an unusual complication and has increased in frequency since the introduction of new invasive procedures. Since the clinical presentation of gas embolism has many faces, it is important to identify it as early as possible: timely treatment may be life saving, while a delay may have serious consequences.

A total of 140 patients underwent emergency obstetric hysterectomy at Smt Sucheta Kriplani Hospital, New Delhi, India, over a 15-year period between 1976 to 1990. Rupture of uterus and obstetric hemorrhage were two major indications for obstetric hysterectomy. Maternal mortality occurred in 18 i.e. 12.8% of the cases.
