Failed DBS for palliation of visual problems in a case of oculopalatal tremor.

OBJECTIVE: To report the results of attempted bilateral red nucleus (RN) deep brain stimulation (DBS) for the palliative treatment of visual problems associated with oculopalatal tremor (OPT). BACKGROUND: It is hypothesized that OPT results from a defect in the Guillain-Mollaret triangle, a circuit that includes connections with the dentate nucleus, the contralateral red nucleus, and the inferior olive. We present a high functioning patient (an accountant) who underwent a palliative trial of RN region DBS in an approach targeted through the subthalamic nucleus region. The aim was to reduce ey

A case of convergence spasms - do not be caught off-guard.

A hypertensive patient presented with complaints of headache and fluctuating double vision and deterioration in vision from 2010. She displayed symptoms associated with convergence spasms, which could be confused with sixth cranial nerve palsies. A few pointers are given to prevent clinicians from being caught off-guard when encountering this clinical condition.

Electroretinographic findings in congenital oculomotor apraxia (Cogan's syndrome).

We performed visual evoked potential testing and electroretinography in five patients with Cogan's oculomotor apraxia. The visual evoked potentials were normal, confirming previously reported data. Electroretinograms were abnormal in two of the five patients examined. A previous study showed normal electroretinogram responses bilaterally in one patient. Our findings suggest the existence of two variants of the syndrome; the one with electroretinogram anomalies has a worse visual prognosis.

Neuro-Ophthalmic Emergencies.

Neuro-ophthalmic emergencies include optic nerve, central visual pathway, and ocular motility disorders that, if not identified and treated promptly, may lead to permanent vision loss, other significant morbidity, or mortality. This article provides a framework for approaching patients with neuro-ophthalmic symptoms and reviews the presentation, evaluation, and treatment of select emergent conditions that can cause them. Emergent causes of blurry vision, transient vision loss, papilledema, and diplopia, including giant cell arteritis, cardioembolic disease, and aggressive infection, are discus…

Oculomotor apraxia and dilated cardiomyopathy with ataxia syndrome: A case report.

Dilated cardiomyopathy with ataxia syndrome (DCMA) is a rare mitochondrial condition associated with early onset cardiomyopathy and non-progressive ataxia. The cardiac manifestations may be progressive and often severe, resulting in significant morbidity and mortality. While optic nerve atrophy has been described in patients with DCMA, to our knowledge, there have been no reports of additional ocular phenotypes. We present two related Dariusleut Hutterite patients with documented DCMA syndrome and disorders of ocular motility: poor smooth pursuit and difficulty initiating saccadic eye movement

Ocular findings in the facioauriculovertebral sequence (Goldenhar-Gorlin syndrome).

We reviewed the ocular findings in 57 consecutive patients with the facioauriculovertebral sequence (Goldenhar-Gorlin syndrome). Epibulbar choristomas were detected in 18 cases (32%), a much lower occurrence than reported previously. Various motility disorders (11 cases, 19%), blepharoptosis or narrow palpebral fissures (seven cases, 12%), eyelid colobomas (six cases, 11%), and lacrimal drainage system anomalies (six cases, 11%) were more frequent than previously noted. These ocular findings were more common in the patients with epibulbar choristomas. Of the various features of the Goldenhar-G

Peripheral ocular motor disorders.

In the reviewed period, articles on peripheral eye movement disorders covered interesting aspects. Localizing value of associated signs, repetitive presentations of palsies, and classical quotations are stressed for the oculomotor nerve. The superior oblique is correlated to central nervous system disorders when overacting in pediatric patients or when ocular torsion is matched to the perceived vertical tilt. The family "pseudo" brought two of its members: "pseudo" myasthenia and "pseudo" myotonia. Mitochondrial citopathies with ocular manifestations can overlap among the different clinical ty

Brain structural and functional magnetic resonance imaging alterations in individuals with convergence insufficiency.

PURPOSE: Individuals with convergence insufficiency (CI) encounter challenges in turning their eyes inward during near work. It is unclear how this relates to brain structural and functional alterations. This study aimed to explore the neural mechanism underlying CI using multimodal brain magnetic resonance imaging (MRI). METHODS: Thirty-four CI participants and 35 healthy controls (HC) were recruited, who underwent visual examinations and brain MRI scanning. Structural MRI data were analysed to calculate cortical thickness, volume and surface area. Fractional amplitude of low-frequency fluct

Rehabilitation of Vertical Gaze Palsy and Impaired Eye Proprioception Following Bilateral Thalamic Infarcts: A Case Report.

IntroductionThalamic infarctions, which account for .6% of all ischemic strokes, are characterized by altered mental status, vertical gaze palsy, and memory impairment. There is only one other case report on the rehabilitation of a vertical gaze palsy due to thalamic lesions. Although rehabilitation strategies for vertical gaze palsy have been documented, the role of eye proprioception in this condition was not discussed.Case ReportA man in his late 50 s presented to outpatient occupational therapy following bilateral thalamic infarcts with bilateral vertical gaze palsy, preserved vestibular o

Abnormal Eye Position Signals in Interstitial Nucleus of Cajal in Monkeys With "A" Pattern Strabismus.

PURPOSE: Pattern strabismus is characterized by a cross-axis pattern of horizontal and vertical misalignments. In A-pattern strabismus, for example, a divergent change in the horizontal misalignment occurs on downgaze. Work with nonhuman primate models has provided evidence that this disorder is associated with abnormal cross-talk between brainstem pathways that normally encode horizontal and vertical eye position and velocity. Neurons in the interstitial nucleus of Cajal (INC) are normally sensitive to vertical eye position; in the present study, we test the hypothesis that, in monkeys with p

Eye abduction reduces but does not eliminate competition in the oculomotor system.

Although it is well established that there is a tight coupling between covert attention and the eye movement system there is an ongoing controversy whether this relationship is functional. Previous studies demonstrated that disrupting the ability to execute an eye movement interferes with the allocation of covert attention. One technique that prevents the execution of an eye movement involves the abduction of the eye in the orbit while presenting the stimuli outside of the effective oculomotor range (Craighero, Nascimben, & Fadiga, 2004). Although eye abduction is supposed to disrupt activatio

[Complications of dysthyroid ophthalmopathy].

The complications of dysthyroid ophthalmopathy should be known and screened in every patient since they can worsen visual prognosis and are critical to the decision for treatment. Ocular surface alteration can result from eyelid malposition, blinking deficiency, eye dryness, and proptosis. Oculomotor disorders are detected by coordimetry or a forced duction test. Optic neuropathy can be compressive or glaucomatous. Intraocular pressure elevation can result from elevation of intraorbital pressure, indentation by an enlarged muscle, or increased backflow in episcleral veins.

Perceived vertical and lateropulsion: clinical syndromes, localization, and prognosis.

We present a clinical classification of central vestibular syndromes according to the three major planes of action of the vestibulo-ocular reflex: yaw, roll, and pitch. The plane-specific syndromes are determined by ocular motor, postural, and perceptual signs. Yaw plane signs are horizontal nystagmus, past pointing, rotational and lateral body falls, deviation of perceived straight-ahead to the left or right. Roll plane signs are torsional nystagmus, skew deviation, ocular torsion, tilts of head, body, and perceived vertical in a clockwise or counterclockwise direction. Pitch plane signs are

[The one and a half vertical syndrome as the only evidence of bilateral thalamo-subthalamic infarction].

INTRODUCTION: The one and a half vertical syndrome consists of paralysis of upward vertical conjugated gaze and monocular paralysis of downward gaze or vice-versa. It occurs as a consequence of a mesencephalodiencephalic lesion, either unilaterally or bilaterally, due to effects on structures such as the interstitial nucleus of Cajal, posterior commissure and rostral interstitial nucleus of the medial longitudinal bundle. The arterial supply of the structures involved in the supranuclear control of vertical gaze depends on the paramedian thalamic arteries (or posterior thalamo-subthalamic arte
