Comparison of simultaneous bilateral CI surgery vs. sequential CI surgery regarding operative time, perioperative morbidity, and anesthesia risk in children.

OBJECTIVES: In recent years, simultaneous bilateral cochlear implantation (CI) has become the preferred procedure for children with bilateral deafness. The aim of this study is to compare simultaneous bilateral implantation with sequential bilateral CI concerning duration of surgery and anesthesia, perioperative morbidity, mortality, and anesthesia risk. METHODS: A retrospective data analysis was conducted on 132 children who were implanted between 2019 and 2024 and aged under 10 years at the time of either simultaneous or second CI. The age at implantation, duration of surgery and anesthesia

Operative time, postoperative pain, and functional recovery in MIPO versus ORIF for humeral shaft fractures: a retrospective cohort case series.

BACKGROUND: Minimally invasive plate osteosynthesis (MIPO) for humeral diaphyseal fractures could result in better outcomes than open reduction and internal fixation (ORIF). However, the relationship between operative time, postoperative pain, and functional outcomes with respect to the use of standard dynamic compression plates (DCP) remains unclear. Therefore, this study aimed to evaluate the correlation between surgical duration, postoperative pain, and functional outcomes. METHODS: A cohort case series of 28 patients with humeral shaft fractures was grouped into 12A1-12C3. MIPO and ORIF w

Pre-operative ultrasound before cesarean delivery: reviewing findings during the first surgical time-out to improve maternal and infant safety.

Twenty-nine million cesarean deliveries performed every year. Although often regarded as routine, even elective procedures can lead to serious complications for mothers and infants. Modern patient safety emphasizes that no incision should be undertaken without knowing what lies beneath, a principle codified by the World Health Organization in its Surgical Safety Checklist. A preoperative ultrasound represents an extension of this principle to obstetrics. Several complications exemplify face-valid safety signals: incision through an anterior placenta with maternal hemorrhage and neonatal anemia

Higher Risk of Wound Complications but No Clinically Significant Increase in Operative Time for Smokers Receiving Myomectomy for Uterine Fibroids.

Myomectomy is a common procedure to remove uterine fibroids. Smoking impacts outcomes across various surgeries; however, may limit fibroid growth. We sought to determine if smoking impacts myomectomy for uterine fibroid removal outcomes. Patients with and without a history of tobacco smoking receiving a myomectomy for uterine fibroid removal were compared. Compared to patients with no history of tobacco smoking, tobacco smokers had a significantly increased risk of wound complications. No other variables analyzed demonstrated a clinically significant difference. Future research is needed to ex

Two cases of metformin-associated lactic acidosis in post-operative period in emergency department: time to be aware-case reports.

BACKGROUND: Diabetic patients on metformin therapy may be vulnerable to lactic acidosis during acute illness. This is particularly true since the comorbid conditions among patients with diabetes and the frequent use of renin-angiotensin system blockers increase the risk of renal dysfunction. CASE DESCRIPTION: We present two cases of metformin-associated lactic acidosis (MALA) occurred after abdominal surgery. A 74-year-old woman presented to emergency department (ED) for a transient loss of consciousness. She had vomiting, diarrhea, and lack of appetite in the last 3 days and she had had an a

Introduction of ROSA robotic-arm system for total knee arthroplasty is associated with a minimal learning curve for operative time.

PURPOSE: The introduction of robotics for total knee arthroplasty (TKA) into the operating theatre is often associated with a learning curve and is potentially associated with additional complications. The purpose of this study was to determine the learning curve of robotic-assisted (RA) TKA within a multi-surgeon team. METHODS: This prospective cohort study included 83 consecutive conventional jig-based TKAs compared with 53 RA TKAs using the Robotic Surgical Assistant (ROSA) system (Zimmer Biomet, Warsaw, Indiana, USA) for knee osteoarthritis performed by three high-volume (> 100 TKA per ye

Comparison of the operative time for open door laminoplasty using titanium plate spacers or hydroxyapatite block spacers: a retrospective study.

BACKGROUND: Titanium plate (TP) and hydroxyapatite (HA) spacers are widely used during open-door laminoplasty, performed with the patient in a prone position. Reducing operative time is an important consideration, particularly to reduce the risk of postoperative complications in older patients. The purpose of this retrospective cohort study was to compare the operative time for open-door laminoplasty using TP or HA spacers. METHODS: Consecutive patients with a spinal cord injury, without bone injury, and ≥ 50 years of age were included. Multivariate regression analysis was used to compare the

The Role of Early Post-Operative Radiographs Following Distal Radius Fracture Fixation with a Volar Locking Plate: Time for Change?

Background: The role of early radiographic imaging in the management of distal radius fractures (DRFs) is unclear. The aim of this study was to assess whether early post-operative radiographs for DRFs influences the ongoing management of this patient group. We hypothesize that routine early radiographs do not influence the management of DRFs. Methods: This was a retrospective review of patients undergoing open reduction and internal fixation using a volar locking plate between 2012 and 2017 at our institution. Patients were identified using hospital electronic databases. Clinical information w

Does Body Mass Index Influence Intraoperative Costs and Operative Times for Anterior Cervical Discectomy and Fusion? A Time-Driven Activity-Based Costing Analysis.

OBJECTIVE: Spine surgeons are often unaware of drivers of cost variation for anterior cervical discectomy and fusion (ACDF). We used time-driven activity-based costing to assess the relationship between body mass index (BMI), total cost, and operating room (OR) times for ACDFs. METHODS: Total cost was divided into direct and indirect costs. Individual costs were obtained by direct observation, electronic medical records, and through querying multiple departments. Timestamps for all involved personnel and material resources were documented. Total intraoperative costs were estimated for all ACD

Time course of choice reaction time deficits in the Hdh(Q92) knock-in mouse model of Huntington's disease in the operant serial implicit learning task (SILT).

A range of transgenic and knock-in mouse models of Huntington's disease have been created since identification in 1993 of the disease mutation in the HD gene. Knock-in models that express the full-length mutant protein tend to exhibit less severe behavioural deficits than transgenic models and so require more sensitive tasks in order to reveal impairments. To achieve this, we therefore used a Serial Implicit Learning Task (SILT), which measures serial reaction times to visual stimuli, requiring detection and responding in both predictable and unpredictable locations in the 9-hole operant chamb

When should we stop operating? Multicenter analysis of operative time, outcomes, and complications in ureteroscopy with flexible navigable suction access sheath (FANS) by EAU Endourology.

Flexible ureteroscopy (FURS) is increasingly used for renal stones, with prolonged operative time (OT) linked to complications. Flexible and Navigable Suction Access Sheaths (FANS) may allow longer OTs due to better intrarenal pressure control. We aimed to evaluate risk factors for prolonged OT and associated outcomes using FANS. A retrospective analysis of a prospective multicentric database across 25 centres included 556 adults undergoing FURS with FANS between August 2023 and January 2024. Patients were stratified into three groups by OT: <60 min(group 1), 60–89 min(group 2), and ≥ 90 min(g

Brain preservation with selective cerebral perfusion for operations requiring circulatory arrest: protection at 25 degrees C is similar to 18 degrees C with shorter operating times.

BACKGROUND: Hypothermic circulatory arrest (HCA) is employed for aortic arch and other complex operations, often with selective cerebral perfusion (SCP). Our previous work has demonstrated real-time evidence of improved brain protection using SCP at 18 degrees C. The purpose of this study was to evaluate the utility of SCP at warmer temperatures (25 degrees C) and its impact on operating times. METHODS: Piglets undergoing cardiopulmonary bypass (CPB) and 60 min of HCA were assigned to three groups: 18 degrees C without SCP, 18 degrees C with SCP and 25 degrees C with SCP (n=8 animals per grou

Comparison between screw-retained and conometric technique in full-arch implant prosthetic rehabilitation regarding operative time needed: an in-vitro study.

BACKGROUND: A common issue with full-arch implant rehabilitations is the inconvenience of prosthesis removal, which must be performed every 6-12 months to ensure proper hygiene. The purpose of this study was to compare the most used technique for full arch prostheses, the screw-retained, with the conometric technique, less discussed in the literature, which could give advantages in terms of removal and reinsertion time. METHODS: The main component used was the implant-prosthetic connection Orbit (Bionica), which allowed for testing both conometric and screw-retained techniques. The first phas
