[Highly dosed Anderson and Kestenbaum operations for anomalous head posture due to nystagmus].

BACKGROUND: The correction of anomalous head posture associated with infantile nystagmus syndrome (INS) is carried out by the Kestenbaum procedure (KP) combining bilateral horizontal yoke muscle recession with resection of their antagonists or by the Anderson procedure (AP) which is confined to yoke muscle recession alone. AIM OF THE STUDY: To compare the results of highly dosed AP and KP for anomalous head turn (HT). PATIENTS AND METHODS: The AP (2013-2019) and KP (2003-2013) were used as the exclusive procedures during the respective time periods. A highly dosed AP was performed in a case

A Conjunctival Retractor during Strabismus Surgery.

The use of a conjunctival retractor that expands a small incision in the conjunctiva/Tenon's capsule facilitated the performance of strabismus surgery through a fornix incision. A wide field of exposure revealed the entire area of the muscle insertion. While there is a risk of conjunctival tearing in elderly patients, the use of the retractor is valuable.

[Sustainability of Anterior Segment Surgery].

Favorable improvement in visual function, credible, long-term sustainability are requisites for anterior eye-segment surgery of the cornea and the lens, key players in ophthalmological optics. Here we analyzed the sustainability, efficacy, and safety of anterior eye-segment surgery, using treatment data from numerous cases, observed over a long period. The effect of a pterygium on visual function is correlated with the degree of invasion of the pterygium in the central cornea. When the pterygium is quite severe, it takes a long time for visual function to stabilize after pterygium surgery. The

Positive vitreous pressure: Pathophysiology, complications, prevention, and management.

Positive vitreous pressure occurs during anterior segment intraocular surgery associated with acute hypotony and is characterized by forward displacement of the lens-iris diaphragm with shallowing of the anterior chamber resistant to reformation, repeated iris prolapse, and, in severe cases, zonular rupture and vitreous or lens prolapse that can lead to a cascade of intraoperative complications. Positive vitreous pressure is particularly common during penetrating keratoplasty, conventional nuclear expression cataract extraction, and repair of anterior open-globe injury. Hypotony resulting from

Divergence Insufficiency Esotropia: Surgical Treatment.

Divergence insufficiency esotropia, or acquired comitant esotropia that is at least 10Δ larger at distance than at near, is most often seen in older adults, and may also be known as "age-related distance esotropia." Surgical treatment is often indicated for patients who do not tolerate prism therapy, or for those with large angles of esotropia. Surgical treatments have evolved with our understanding of the disease-state. Currently, accepted treatments include lateral rectus resection and medial rectus recession. These surgeries can be performed unilaterally or bilaterally. New surgical techniq

Novel Surgical Approaches to the Orbit.

Determining safe surgical access to the orbit can be difficult given the complex anatomy and delicacy of the orbital structures. When considering biopsy or removal of an orbital tumor or repair of orbital fractures, careful planning is required to determine the ideal approach. Traditionally, this has at times necessitated invasive procedures with large incisions and extensive bone removal. The purpose of this review was to present newly techniques and devices in orbital surgery that have been reported over the past decade, with aims to provide better exposure and/or minimally invasive approach

Small Deviations: Vertical, Horizontal, and Combined.

INTRODUCTION AND PURPOSE: Symptomatic, small-angle strabismus is largely an acquired problem of adults that has previously been managed with prism spectacles. This paper will explore surgical options that are becoming increasingly popular. METHODS: The special issues that confound surgical treatment of small-angle strabismus are considered. Surgical procedures described in the literature and in the author's experience that address these issues are reviewed. RESULTS: Several recently described surgical techniques show promise for treatment of small-angle strabismus in adults. Their use is ill

Controversies in orbital reconstruction--II. Timing of post-traumatic orbital reconstruction: a systematic review.

The timing of orbital reconstruction is a determinative factor with respect to the incidence of potential postoperative orbital complications. In orbital trauma surgery, a general distinction is made between immediate (within hours), early (within 2 weeks), and late surgical intervention. There is a strong consensus on the indications for immediate repair, but clinicians face challenges in identifying patients with minimal defects who may actually benefit from delayed surgical treatment. Moreover, controversies exist regarding the risk of late surgery-related orbital fibrosis, since traumatic

Applications of femtosecond lasers in ophthalmic surgery.

PURPOSE: To review recent advancements in applications of the femtosecond (FS) laser in ophthalmology. DESIGN: Literature review and commentary. RESULTS: With the FS laser, it is now possible to produce a more precise flap diameter, hinge and depth of the flap in laser in situ keratomileusis surgery. Recently, the FS laser has been used for both flap creation and refractive correction by lenticule extraction. This laser can also be used for preparing host and donor tissue in different shapes so as to obtain faster recovery and improved postoperative visual acuity. Intracorneal ring segments

New strabismus surgical techniques.

Five new strabismus surgical procedures were developed by the authors and were presented as a workshop at the AAPOS annual meeting of the American Association for Pediatric Ophthalmology and Strabismus. They are summarized here to introduce these methods to the reader and provide surgeons with more options in approaching difficult strabismus problems. Diagnosis and correction of consecutive strabismus attributable to stretched scar is discussed. The pulley posterior fixation procedure is a unique approach to correct high accommodative convergence or to augment a maximal medial rectus recession

Current treatment options for corneal ectasia.

PURPOSE OF REVIEW: The approach to the management of various forms of corneal ectasia is changing, with the advent of new surgical and nonsurgical options. The purpose of this review is to summarize and evaluate relevant studies on new treatments for keratoconus, postrefractive surgery keratectasia, and peripheral ectatic corneal disorders. RECENT FINDINGS: Various alternatives to corneal transplantation for the management of keratoconus aim to enhance corneal rigidity by means of nonsurgical collagen cross-linking, or with the use of intrastromal corneal ring segments, and studies suggest th

Ciliary body schwannoma successfully treated by local resection.

BACKGROUND: Schwannoma arising from the ciliary body is a rare intraocular neoplasm, and most cases previously reported were diagnosed histopathologically after eyeball enucleation because of a diagnosis of ciliary body melanoma. CASE: A case of ciliary body schwannoma was dissected locally. OBSERVATIONS: A 19-year-old woman was referred to us with a diagnosis of iris cyst. Local resection of the tumor was performed because of deteriorated visual acuity caused by displacement of the lens. In histopathological analysis, the encapsulated tumor showed a proliferation of the spindle cells with n

Necrotizing scleritis after intraocular surgery associated with the use of polyester nonabsorbable sutures.

We present 2 elderly patients who developed necrotizing scleritis after cataract extraction in which the wound was sutured with polyester. In the first case, a 73-year-old woman who had right phacoemulsification via a limbal incision developed necrotizing scleritis 8 months after surgery. The eye became progressively painful and phthisical, necessitating enucleation. Microscopic examination of the enucleated globe showed a predominantly lymphocytic infiltration of the ocular tissues with no evidence of an infectious agent. In the second case, a 78-year-old woman had bilateral extracapsular cat

Eyewall resection for a uveal malignant melanoma under local anesthesia.

A 44-year-old man was referred to our hospital for ciliary body tumor management. A uveal melanoma in the left eye without systemic metastasis was our impression after a series of examinations. We treated this patient with eyewall resection under local anesthesia. Eyewall resection was previously regarded as a complicated procedure that should be performed under general anesthesia. Our surgery was successful, and there were no complications related to the local anesthesia. We followed up this patient for 3 years, and no recurrence was found. Eyewall resection is a good alternative to enucleati
