Effects of Early Life Trauma on Risks for Adult Opioid Use Disorder Are Mediated by Stress and Occur Independent of Depression and Anxiety.

OBJECTIVES: Adverse childhood experiences, or early life trauma (ELT), may be a potential risk factor for opioid use disorders (OUDs) that could be further influenced by depression, anxiety, and stress. The prevalence and strength of these associations are largely unknown. METHODS: This study examined the association between current OUD severity and lifetime history of ELT, and the degree to which current depression, anxiety, and stress influenced this association, in persons (n = 310) with at least 1 lifetime exposure to opioids using an online survey. RESULTS: Ninety-three percent of respo

Latent trajectories of anxiety and depressive symptoms among adults in early treatment for nonmedical opioid use.

BACKGROUND: Understanding the course of anxiety and depressive symptoms in early opioid use disorder (OUD) treatment may inform efforts to promote positive early treatment response and reduce treatment attrition. METHODS: Persons in treatment for nonmedical opioid use were identified from 86 addiction treatment facilities. Growth mixture modeling was used to identify trajectories of anxiety and depression symptoms during the first month of treatment among individuals who screened positive for depression (N = 3016) and/or anxiety (N = 2779) at intake. RESULTS: A three-class solution best fit

[Opioid Maintenance for Patients in Inpatient Long-Term Rehabilitation Treatment - which Patients Benefit?].

OBJECTIVE: Patients from opioid maintenance treatment (OMT) may taper off from opioids during rehab treatment. It is investigated whether this option improves treatment outcomes in particular for those patients who are affected by psychiatric or somatic comorbidity, or by social problems. METHODS: Multicenter study with 4 rehab units. Patient characteristics and treatment outcomes were collected retrospectively. There were n = 227 patients who were admitted to treatment while still receiving OMT, and n = 156 who were opioid free. RESULTS: With regard to regular completion of rehab treatment,

Hispanics, Addictions, and the Opioid Epidemic: Brief Report.

Opioid addiction is a public health concern. Opioid overdose death rates account for one third to a half of all global substance-related deaths. Opioid mortality rates increased nearly fivefold in the United States between 1999 and 2016. Recent research has found health care disparities in the United States among minority populations with opioid use disorder, particularly Hispanics. Current literature also suggests that a number of social and cultural factors, including the stigma linked to mental illness and treatment in the Hispanic culture, may further negatively contribute to the problem.

Methadone Matters: What the United States Can Learn from the Global Effort to Treat Opioid Addiction.

In the midst of an opioid epidemic, mortality related to opioid overdose continues to rise in the US. Medications to treat opioid use disorder, including methadone and buprenorphine, are highly effective in reducing the morbidity and mortality related to illicit opioid use. Despite the efficacy of these life-saving medications, the majority of people with an opioid use disorder lack access to treatment. This paper briefly reviews the evidence to support the use of medications to treat opioid use disorder with a specific focus on methadone. We discuss the current state of methadone therapy for

Opioids and Adolescents: How to Prevent and Treat Addictions Early.

Addiction is a treatable, chronic condition of pediatric onset. Providers will learn how to use evidence-based methods for prevention , screening, and treatment of substance use among adolescents. Treating adolescents who use substances can be an opportunity to provide life-saving harm reduction resources including naloxone and start medications for opioid use disorder such as buprenoprhine. Identifying early, at-risk substance use is increasingly critical given the risk of lethal overdose due to fentanyl contamination.

The evolving epidemiology of opioid use disorder: polysubstance use, drug supply transformation, and policy implications.

BACKGROUND: Synthetic Opioids and the rising polysubstance use have caused a substantial change in the epidemiology of opioid use disorder (OUD) in the United States. Current overdose patterns may no longer be adequately explained by traditional opioid-driven theories. METHODS: A structured review was conducted using PRISMA-informed methods. Literature published between 2010 and 2026 was identified through PubMed, Scopus, and Google Scholar using terms related to OUD, fentanyl, polysubstance use, and treatment strategies. Eligible sources included peer-reviewed studies and government reports

Safer opioid supply clients' values and preferences: a systematic review and thematic synthesis of qualitative research.

QUESTION: What are the values and preferences of safer opioid supply clients? STUDY SELECTION AND ANALYSIS: We conducted a systematic review of qualitative studies on safer opioid supply client experiences published between January 2016 and August 2024. Searches were conducted across seven databases-MEDLINE, Embase, PsycINFO, CINAHL, EBM Reviews, Web of Science and Scopus-and supplemented with searches in Google Scholar and relevant repositories. Study quality was assessed using the Critical Appraisal Skills Programme. Data analysis was guided by Thomas and Harden's thematic synthesis approac

Nonpharmacological and pharmacological influences on opioid effects: A review of clinical research findings.

The current opioid crisis underscores the need for continued research on variables that affect opioid self-administration and strategies to alter this behavior. Decades of human behavioral pharmacology studies suggest that a range of nonpharmacological and pharmacological factors influence the effects of opioids. This review focuses on consolidating findings on the influence of experimental conditions on opioid effects, with a particular emphasis on subjective effects and reinforcement. We describe different schedules of reinforcement that have been used in studies of opioid self-administratio

Considerations and limitations of buprenorphine prescribing for opioid use disorder in the intensive care unit setting: A narrative review.

PURPOSE: The purpose of this review is to discuss important considerations when prescribing buprenorphine for opioid use disorder (OUD) in the intensive care unit (ICU) setting, recognizing the challenges of providing detailed recommendations in the setting of limited available evidence. SUMMARY: Buprenorphine is a partial mu-opioid receptor agonist that is likely to be increasingly prescribed for OUD in the ICU setting due to the relaxation of prescribing regulations. The pharmacology and pharmacokinetics of buprenorphine are complicated by the availability of several formulations that can b

Stigma as a local process: Stigma associated with opioid dependency in a rural-mixed Indiana county.

BACKGROUND: Because the nature and magnitude of stigmatizing views associated with opioid dependency vary by social, cultural, and structural factors, strategies to reduce public stigma towards opioid dependency should vary by context. We leverage a unique dataset with evidence of multiple stigmatizing views to understand how to target interventions to reduce stigma in a state disproportionately impacted by the opioid epidemic, with a specific focus on a rural-mixed county. METHODS: Data come from the representative Person-to-Person Health Study (2018-2020) of 2,050 Indiana residents, 224 fro

The association between benzodiazepine co-prescription, opioid agonist treatment and mortality: a systematic review.

BACKGROUND: Opioid agonist treatment (OAT) is the preferred treatment for opioid dependence due to benefits such as treatment retention, reduced opioid use and mortality. Benzodiazepine co-dependence is common in OAT patients and has been linked to increased mortality. Prescribing benzodiazepines during OAT has been tried to reduce the harms of extra-medical benzodiazepine use. This systematic review examines association between benzodiazepine co-prescription during OAT and mortality. METHODS: We searched MEDLINE, Embase, Psych INFO, Cochrane Database of Systematic Reviews, Cochrane Central R
