Upper face and orbit "degloving" dog bite injury.

A 70-year-old woman who attempted suicide lay unconscious on her floor for an unknown time period while her 2 pet dachshunds chewed her upper face and bilateral periorbital areas including all 4 eyelids, both lacrimal glands, all of the conjunctiva from both eyes, the extraocular muscles on the left side only, and anterior orbital fat from both sides. A subtotal exenteration was performed on her left orbit and a temporoparietal fasciocutaneous flap was used to reconstruct her right orbit with buccal mucosa replacing both the bulbar and palpebral conjunctiva. To the authors' knowledge, this is …

Group C streptococcal subperiosteal abscess of the orbit.

Group C Streptococcus is a well recognised pathogen in acute pharyngitis and sinusitis. To our knowledge group c streptococcus has not been reported to cause orbital cellulitis or subperiosteal abscess. We wish to report a case of a child with orbital subperiosteal abscess caused by Group C streptococcus.

[3D ultrasonography diagnostics of the eye and orbit].

3D ultrasonography eye and orbit improve local topography of the eye and orbital space. 3D ultrasonography doesn't supply ultrasonography examination A and B mode. 3D ultrasonography is appropriate for diagnostics of vitreoretional proliferations, tumour diagnostics and therapy (surgical, laser, Ruthenium plate implantation), druses of the optic nerve and for education of young physicians in sonography too.

Management of the orbit in malignant sinonasal tumors.

Malignant ethmoid and maxillary sinus tumors frequently involve the orbit. Orbital involvement is an important prognostic predictor of recurrence-free, disease-specific, and overall survival. Most authors agree that orbital preservation as opposed to orbital exenteration or clearance does not result in significant differences in local recurrence or actuarial survival. The eye can be safely preserved in most patients with ethmoid or maxillary sinus cancer invading the orbital wall, including malignancies that invade the orbital soft tissues with penetration through the periorbita provided that …

Decrease in levator function in the anophthalmic orbit.

BACKGROUND/AIMS: It has been argued that levator function in anophthalmic patients is reduced, but no detailed report on this topic has been issued. This study was undertaken to investigate measured levator function in anophthalmic patients, and to determine how measured values are related to prosthesis size. METHODS: Unilateral anophthalmic patients with no significant ptosis, and no history of trauma or surgery to orbit or eyelid were recruited. Levator function was measured with patients wearing their prosthesis, test prosthesis of different heights, and no prosthesis. RESULTS: Thirty-two…

Penetrating injuries to the orbit despite safety equipment.

Penetrating injuries to the orbit are uncommon but may have catastrophic consequences ranging from blindness, cerebral damage to death. Apparently similar injuries but with a slight difference in the anatomic pathway may have minimal morbidity. Prevention by the use of full safety equipment is recommended. This case report shows that full safety equipment may not prevent injury.

Cysticercosis of the bony orbit--a rare entity.

Bone involvement in cysticercosis is a rare occurrence. A case of cysticercosis of the frontal orbital bone in a thirty-year-old female is reported. The diagnosis was established by computerized tomographic scanning of the orbit and histopathological examination of the cyst, surgically removed from the cavity in the orbital plate of the frontal bone. To our knowledge, this is the first case report of cysticercosis of the bony orbit.

Composite lymphoma of the orbit treated with rituximab.

We describe an unusual case of composite lymphoma (CL) in the orbit. The clinical history and biopsy specimen of an 82-year-old woman with a right orbital mass were evaluated. The orbital biopsy contained a dense lymphocytic infiltration and nodules of large lymphocyte that immunostained positive for CD20. Flow cytometric immunophenotyping showed two distinct populations of cells, confirming the diagnosis of a simultaneous follicular lymphoma (FL) and chronic lymphocytic leukemia/small lymphocytic lymphoma (CLL/SLL). The patient was treated with CD20 antibody (rituximab) and is in remission at…

Integrated reconstructive strategies for treating the anophthalmic orbit.

INTRODUCTION: Anophthalmia may be congenital or acquired. Congenital anophthalmia refers to any orbit that contains a severely hypoplastic eye at birth (microphthalmia), or a complete absence of the globe due to failure of optic vesicle formation. In both those cases the aim of surgery is to stimulate adequate orbital growth. Acquired anophthalmic orbit may be due to trauma or tumour. In acquired forms the goal is restoration of orbital volume with adequate replacement of orbital contents. PATIENTS AND METHODS: In this study 28 patients (6 cases of congenital and 22 of acquired anophthalmia),…

Juvenile xanthogranuloma of the orbit in an adult.

We present a case of juvenile xanthogranuloma (JXG) with unilateral involvement of the orbit and eyelid and proptosis, histologically confirmed in a 32-year-old man with a 1-year history of a pansinusitis and dacryoadenitis with rhinitis. Nine months later an infiltration of the anterior upper part of the right orbit and right eyelid appeared. Computed tomography scan and magnetic resonance imaging studies confirmed the presence of pansinusitis and infiltration. The patient underwent a blepharoplasty and excision of the infiltrated tissues of the orbit, eyelid, and levator muscle. Hematoxylin-…

Radiation therapy for benign disease of the orbit.

Benign diseases of the orbit can cause significant impairment of visual function through direct effects on the eye or supporting orbital tissues. Persistent moderate-to-severe inflammatory symptoms, diplopia, and visual loss may prompt therapeutic intervention. Low doses of external-beam irradiation (20 Gy in 2-Gy fractions) have an efficacy equivalent to corticosteroid medications in the treatment of both Graves' ophthalmopathy and orbital pseudotumor, with response rates of 50% to 80%. Appropriate patient selection, coordination with other medical subspecialties, and careful treatment planni…

[Results of operative therapy of bony orbit dystopies].

Dystopies of the bony orbit are caused mainly by craniosynostosis, facial clefts and encephaloceles. This article presents the results of the surgical correction of orbital hypertelorism in 24 patients. Using this operation technique, the bony interorbital distance was decreased from an average of 47.6 mm to 22.8 mm. However, the distance between the soft tissues was not reduced by the same amount as the distance between the bony orbits. The intercanthal distance decreased from an average of 58.5 mm to 44.5 mm, whereby an additional refixation of the medial palpebral ligament resulted in a red…

Cutaneous approaches to the lower lid and orbit.

In any discussion of surgical exposure, it is important to remember that of chief importance is what is done in the operative field, rather than how the site is exposed. Selection of approach is based on tradeoffs that must be weighed by the surgeon and tailored to the deformity for a given patient. Despite a recent surge in the popularity of the transconjunctival technique, periorbital surgery by a cutaneous approach is still a valid means of access for a variety of esthetic and particularly trauma procedures. There are few solid data comparing the two approaches, and what does exist shows li…

Subperiosteal hematoma of the orbit associated with sinusitis.

Subperiosteal hematoma of the orbit is a rare but well-recognized entity, usually caused by trauma. Two cases of subperiosteal hematoma associated with sinusitis are presented. A 44-year-old woman experienced the sudden onset of proptosis, and decreased visual acuity. Computed tomographic scanning and magnetic resonance imaging revealed a frontoethmoidal mucocele and a biconvex mass in the upper part of the left orbit. The mucocele was drained during nasal endoscopic surgery and the subperiosteal hematoma was evacuated during superior orbitotomy. A 42-year-old man had a headache and proptosis.…

Streptococcal necrotizing fasciitis of the eyelids and orbit.

Streptococcal necrotizing fasciitis in previously healthy persons can develop quickly and may be difficult to diagnose. In a number of cases, this potentially fatal condition has been seen originally in the eyelid and orbit. Intensive antibiotic therapy and prompt debridement are the usual course of treatment.

[Computed tomography in acute inflammation of the orbit].

Acute orbital infection, which is usually secondary to sinusitis, requires prompt diagnosis and timely treatment to prevent the infection from spreading and to avoid severe complications, such as osteomyelitis, orbital and cerebral abscesses, meningitis and cavernous sinus thrombophlebitis. Fifteen young patients with orbital cellulitis were examined: they were 13 men and 2 women, whose mean age was 23 years, with orbital cellulitis secondary to fronto-ethmoidal sinusitis (10 cases), to maxillary sinusitis (4 cases) and to craniofacial trauma (1 case). On the basis of CT findings the patients …

Clinical spectrum of histiocytic tumors of the orbit.

Histiocytic tumors of the orbit comprise an unusual group of lesions characterized by the infiltration of the orbital tissues by xanthomatous cells. Recently, there have been a number of new observations regarding the various histiocytic tumors that can affect the orbit. The condition previously referred to as histiocytosis X is believed to represent a proliferation of Langerhans cells and the term Langerhans cell histiocytosis is often used instead of histiocytosis X. Juvenile xanthogranuloma has been demonstrated to affect the orbit without involving the skin or the iris. The Erdheim-Chester…

Tumors metastatic to the orbit: a changing picture.

Clinical characteristics of tumors metastatic to the orbit are related to primary tumor biology, and vary substantially among the various primary types. Common known primary sites include breast, lung, prostate, and melanoma. Tumor presentations can be classified into four generalized syndromes of mass, infiltrative, inflammatory, and functional effects. We found the infiltrative syndrome of presentation to be more common than for other types of orbital neoplasm. Accurate diagnosis often depends on recognition of the types of clinical syndromes and on the use of diagnostic modalities such as c…
