Progression and projection for shoulder surgery in France, 2012-2070: Epidemiologic study with trend and projection analysis.

INTRODUCTION: Shoulder surgery has been rapidly expanding over the past 20 years and now makes up a large share of orthopedic surgery practice. Data on how this activity has changed is not available in France due to a lack of registries. The study objectives were to: (1) quantify the number of shoulder surgeries in France, (2) predict how this activity will change over the next 50 years based on extreme scenarios. METHODS: This study involved an analysis of shoulder surgery data taken from the French hospital discharge database (PMSI). Two mathematical scenarios were applied to define the cha

Optimizing the Use of Operating Rooms by Transitioning Common Hand Surgeries Into the Office Setting.

PURPOSE: The purpose of this study was to examine the true monetary implications, at the health system level, of moving simple hand procedures, performed with wide-awake local anesthesia no tourniquet surgery, from the ambulatory surgery center (ASC) to office setting. METHODS: We analyzed the costs, revenues, case times, and patient demographics for 2 cohorts of patients who underwent hand and non-hand surgical procedures over a 2-year period. We calculated the mean margin per minute for the top 5 procedures in non-hand orthopedic surgery subgroups, complex plastics hand, and non-hand plasti

Role of epiphysiodesis in the management of deformities of the foot and ankle.

Dynamic correction of orthopedic deformities of the foot and ankle may be performed by epiphyseal growth retardation, or arrest. These procedures lend themselves to the management of juvenile hallux abducto valgus with metatarsus primus adductus, and ankle valgus in particular. Historical review, indications, timing, and technique are addressed in this manuscript.

Antibacterial infection and immune-evasive coating for orthopedic implants.

Bacterial infection and infection-induced immune response have been a life-threatening risk for patients having orthopedic implant surgeries. Conventional biomaterials are vulnerable to biocontamination, which causes bacterial invasion in wounded areas, leading to postoperative infection. Therefore, development of anti-infection and immune-evasive coating for orthopedic implants is urgently needed. Here, we developed an advanced surface modification technique for orthopedic implants termed lubricated orthopedic implant surface (LOIS), which was inspired by slippery surface of Nepenthes pitcher

Medical Malpractice Litigation in Spine Surgery: A Systematic Review.

BACKGROUND: Spine surgery is a high-risk specialty for litigation, with most practicing surgeons encountering at least 1 malpractice claim during their careers. This systematic review aimed to evaluate the trends and characteristics of medical malpractice claims after spine surgery. METHODS: A systematic search was conducted on PubMed, Scopus, Embase, and Web of Science to find studies reporting on patients who underwent spine surgery and sued their surgeons. Studies that included original extractable data on litigation after spine surgery were included without date or geographical restrictio

Introducing surgical workflow recognition in orthopaedic surgery with timestamp supervision.

Surgical workflow recognition (SWR) is associated with numerous potential applications to improve patient safety and surgeon performance. So far, SWR studies have mainly focused on endoscopic procedures due to the scarcity of publicly available open surgery video datasets. In this article, we propose for the first time to work on an open orthopaedic surgery called minimally invasive plate osteosynthesis (MIPO) for distal radius fractures (DRFs). For this purpose, we introduce a new dataset with 50 videos of the DRF MIPO procedure, representing almost 30 h of surgery. As far as we know, this cu

Minor Hand Surgery Procedures.

While hand surgery has developed as its own unique subspecialty since the mid-twentieth century, other providers, especially those practicing in emergency or more rural settings, may be called upon to perform minor surgical procedures of the hand. With understanding of the relevant anatomy and basic surgical principles, minor hand procedures can be done safely and effectively. This article will discuss common hand injuries and pathologies and their respective management.

The Modular Concept in Skull Base Surgery: Anatomical Basis of the Median, Paramedian and Lateral Corridors.

Introduction A thorough understanding of skull base anatomy is imperative to perform safely and effectively any skull base approach. In this article, we examine the microsurgical anatomy of the skull base by proposing a modular topographic organization in the median, paramedian, and lateral surgical corridors in relation to transcranial and endoscopic approaches. Methods Five dry skulls were studied focusing on the intracranial and exocranial skull base. Two lines were drawn parallel to the lateral border of the cribriform plate of the ethmoid bone and foramen lacerum, respectively. Lines 1 an

Pectus excavatum and scoliosis: a review about the patient's surgical management.

Although Nuss procedure is performed with satisfactory results, there has been a controversy in the literature regarding the effect of a Nuss procedure on the spine. This review article aims to perform an updated overview of the literature about the effect of pectus excavatum correction at the spine and the management of patients with both pectus excavatum and scoliosis. Although acquired scoliosis has been rarely reported after a Nuss procedure, studies show that the Nuss procedure can have a beneficial effect in mild coexisting scoliosis especially when it is performed during the adolescence

Development of a cadaveric training model for pollicization of the index finger.

There are currently no surgical models for learning index finger pollicization. This led us to develop and evaluate a cadaveric model for index finger pollicization. Ten hands from fresh cadavers were used. In each case, we evaluated all the surgical steps, the appearance of the web space, the new thumb's position, and the model's advantages and disadvantages. Flap coverage was insufficient due to poor skin condition in 3 cases and the commissure was too short in 3 cases. The dissection and tendon transfer steps were performed correctly in 9 cases. In one case, the interosseous transfer was to

Hollow Notched K-Wires for Bone Drilling With Through-Tool Cooling.

Kirschner wire (K-wire) is a common tool in clinical orthopedic surgery for bone fracture fixation. A significant amount of heat is generated in bone drilling using K-wires, causing bone thermal necrosis and osteonecrosis. To minimize the temperature rise, a hollow notched K-wire in a modified surgical hand drill with through-tool cooling was developed to study the bone temperature, debris evacuation, and material removal rate. The hollow notched K-wire was fabricated by grinding and micro-milling on a stainless steel tube. Bone drilling tests were conducted to evaluate its performance against
