Familial spastic paraplegia: evidence for a fourth locus.

Autosomal dominant familial spastic paraparesis (AD-FSP) is a genetically heterogeneous disorder of the central nervous system characterized by a progressive spasticity of the legs. One gene causing AD-FSP (FSP1) has recently been mapped to chromosome 14q, another gene (FSP2) to chromosome 2p, and a third gene (FSP3) to chromosome 15q. We now report a large Dutch family with AD-FSP without linkage to any of these chromosomes, providing evidence for a fourth locus (FSP4).

Hereditary distal dominant amyotrophy followed by spastic paraplegia.

Clinical, neurophysiological and neuropathological investigations were performed on five patients from two families with autosomal dominant distal amyotrophy followed by spastic paraplegia and with a positive history in two generations of these two families. All cases in the two families had a benign clinical course, although two mothers could not walk without support at around 60 years old. Neurophysiological studies revealed normal maximum conduction velocities of peripheral sensory and motor nerves, and the central spinal sensory pathway. Distribution of motor nerve conduction velocities in…

Flaccid paraplegia as complication of umbilical artery catheterization.

Two patients are described who acquired neonatal flaccid paraplegia associated with umbilical artery catheterization. Neurophysiologic studies demonstrated spinal cord injury at the level of T7 and below in Patient 1 and at the level of L1 and below in Patient 2. Deferred spinal cord magnetic resonance imaging confirmed regional spinal cord atrophy. In Patient 1, the syndrome was presumed to be triggered by a spasm or embolism of the Adamkiewicz artery due to movement of the umbilical artery catheter. In Patient 2, the spinal cord ischemia was probably caused by an embolic mechanism.

Acute paraplegia: a presenting manifestation of aortic dissection.

Two patients who presented with acute paralysis of the lower extremities as an initial manifestation of aortic dissection are described. The first patient had transient chest pain followed by flaccid paralysis of her lower extremities and severe back pain. In the second patient, sudden paralysis of both legs developed without pain of any sort. The paraplegia completely resolved in a few minutes; however, chest and back pain later ensued. Both patients had a proximal (type I or A) aortic dissection. The first patient's entrance tear in the aortic intima was just above the aortic valve with ante…

Dominantly inherited spastic paraplegia and multifocal palmoplantar hyperkeratosis.

We have extended the study of a previously published kindred with spastic paraplegia and palmoplantar hyperkeratosis from 2 to 18 persons. Present evidence suggests that both neural and cutaneous manifestations are due to a single mutant gene, inherited as an autosomal dominant. Nerve conduction and EMG examination provide evidence for mild lower motor and primary sensory neuron (axon) involvement but these techniques are not useful for early detection. The disorder is a unique genetic disease which stands apart from other mostly recessively inherited varieties of neuroichthyosis.

Evaluation of acute surgical intervention in traumatic paraplegia.

A retrospective study was conducted over a two-year period of 95 consecutive admissions for traumatic paraplegia. The average age was 32 years. Seventy-two (76%) of these patients had acute surgical intervention. Fifty had Harrington rod placement with posterior fusion, 10 had additional laminectomy and one had rodding and anterior fusion. Seven laminectomies (alone) were performed at other hospitals. For those with rodding and/or fusion, rehabilitation stay was 70 days; for those with no surgery, 81 days. This difference was not statistically significant. However, when days of acute medical/s…

Familial spastic paraplegia, mental retardation, and precocious puberty.

Two brothers had progressive spastic paraplegia and precocious puberty develop due to Leydig's cell hyperplasia when they were 2 years old. Both later had moderate mental retardation. Family members displayed brisk lower-extremity reflexes and dysarthria in a pedigree that suggested autosomal dominant inheritance with variable expression. Precocious puberty has been associated with other neurologic syndromes. Its occurrence in two brothers with spastic paraplegia has not, to our knowledge, been previously reported.

Dynorphin induces partially reversible paraplegia in the rat.

Endogenous opioids have previously been implicated in the pathophysiology of spinal cord injury, but the specific opioid(s) involved has not been determined. Here we report that dynorphin administered intrathecally causes dose-related, partially reversible, hindlimb paralysis in the unanesthetized rat. Other opioids, including mu-selective and delta-selective synthetic enkephalins or beta-endorphin, failed to induce motor dysfunction. These findings suggest that dynorphin or dynorphin-related peptides may play a role in spinal cord injury.

Spinal somatosensory evoked potentials in hereditary spastic paraplegia.

Cervical somatosensory evoked potentials elicited by median nerve stimulation were recorded from 18 cases of hereditary spastic paraplegia. Motor and sensory nerve conduction in the median nerve was normal in each. In one third of the patients no spinal evoked potential was detectable. In the remainder the amplitude of the evoked potential was reduced in comparison with a control series; the latency was not significantly different. These changes suggest that a selective degeneration of the centrally directed axons derived from the dorsal root ganglion cells occurs in this disorder. The finding…

[Paraplegia following acute infrarenal aortic occlusion (author's transl)].

The clinical syndrome of paraplegia following acute occlusion of the infrarenal aorta may be caused by either ischemic spinal cord damage or ischemia of the cauda equina and sacral nerve roots and ganglia. The neurologic manifestations are similar and therefore specific anatomic diagnosis is difficult. From October 1972 to February 1975 a total of 31 patients with infrarenal aortic occlusion were treated at the Medizinische Hochschule in Hannover. In nine cases the occlusion up to the renal arteries was acute. Three of these patients presented beside acute ischemic manifestations on both legs …
