INTRODUCTION: We studied the epidemiological trends of enteric fevers (typhoid and paratyphoid fever) in Singapore from 1990 to 2009 and carried out a review of the current prevention and control measures.
MATERIALS AND METHODS: Epidemiological records of all reported enteric fevers maintained by the Communicable Diseases Division, Ministry of Health from 1990 to 2009 were analysed.

An outbreak of typhoid fever in the largest refugee camp of the besieged capital of Phnom-Penh, Cambodia, and availability of a locally manufactured Anti-Tetanus-Typhoid-Paratyphoid vaccine prompted us to test the practicability of a mass immunization campaign by carrying out a trial in the smaller, well delineated refugee camp of Pochentong. In the 16-year period 1975-90, there were 267 cases of acute infection with typhoidal salmonellae reported in Scotland, in addition to which 32 chronic carriers were identified. The overall incidence of disease changed little over this period, but there was a fall in indigenously acquired paratyphoid B and typhoid, and a rise in imported paratyphoid A. A disease with main symptoms of fever, headache, malaise, profuse sweating and diarrhoea occurred as an outbreak in a few villages at suburb of Hengyang City from March to July 1987. It was diagnosed as Type B paratyphoid fever by laboratory examinations. The number of cases occurred in this outbreak was 59 in total. All of them recovered after isolation and proper-treatment.

In vitro studies carried out in this Department confirmed the high activity of mecillinam against Salmonella spp. As a result we set up a study to investigate the efficacy of mecillinam in patients suffering from enteric fever. To date 13 out of 15 patients with typhoid or paratyphoid fever have been successfully treated with this new amidinopenicillanic acid antibiotic.

Development of cellular immunoreactivity to Salmonella typhi and Salmonella paratyphi-A was studied by the leukocyte migration inhibition test in 9 patients with typhoid fever and in 2 patients with paratyphoid fever. Cellular reactivity could be demonstrated from the first days of the disease in all the subjects. The most pronounced migration inhibition was observed during the febrile period. It is suggested that specific cellular reactivity may play a pathogenetic role in typhoid fever.

A patient with paratyphoid fever relapsed bacteriologically during the treatment with chloramphenicol, and was treated with daily doses of 600 to 800 mg of ofloxacin for 14 days. Salmonella paratyphi A was eradicated. Ofloxacin appears to be an effective drug for the eradication of S. paratyphi A.
