Abnormal origin of the right subclavian artery from the right pulmonary artery in a patient with D-transposition of the great vessels and left juxtaposition of the right atrial appendage: an unusual anatomical variant.

Isolation of the left subclavian artery (LSCA) or its anomalous origin from the pulmonary artery (PA) has been documented in several cases, especially in association with a right-sided aortic arch. Similar anomalies involving the right subclavian artery (RSCA) are less frequent. Anomalous origin of the RSCA from the PA in association with D-transposition of the great arteries (D-TGA) is exceedingly rare and only two cases have been reported so far. We present here, a case of aberrant origin of the RSCA from the right PA in a patient with D-TGA, in whom the diagnosis was rendered difficult due

Within-patient comparison of His-bundle pacing, right ventricular pacing, and right ventricular pacing avoidance algorithms in patients with PR prolongation: Acute hemodynamic study.

AIMS: A prolonged PR interval may adversely affect ventricular filling and, therefore, cardiac function. AV delay can be corrected using right ventricular pacing (RVP), but this induces ventricular dyssynchrony, itself harmful. Therefore, in intermittent heart block, pacing avoidance algorithms are often implemented. We tested His-bundle pacing (HBP) as an alternative. METHODS: Outpatients with a long PR interval (>200 ms) and intermittent need for ventricular pacing were recruited. We measured within-patient differences in high-precision hemodynamics between AV-optimized RVP and HBP, as well

Two patients of left lung cancer with right aortic arch: review of eight patients.

We report two cases of left lung cancer in patients with variant right aortic arches. Preoperative heart examination ascertained that neither patient had congenital heart disease. Patient 1 exhibited a right aortic arch with mirror-image branching of the major arteries. The patient's clinical stage was T1aN0M0 stage IA. Patient 2 exhibited a right aortic arch with an aberrant left subclavian artery. The patient received induction chemotherapy for cT2aN2M0 stage IIIA adenocarcinoma of the lung. In patients with a right aortic arch undergoing surgery, especially mediastinal lymph node dissection

Assessment of the predictive value of right ventricular longitudinal strain and functional trends in right ventricular function of patients after LVAD implantation.

Right heart failure (RHF) remains a major determinant of morbidity and mortality following left ventricular assist device (LVAD) implantation. Right ventricular free wall strain (RVFWS) and global longitudinal strain (RVGLS) have emerged as promising echocardiographic markers, yet their long-term prognostic utility and suitability to assess changes in RV function remain poorly defined. In this single-center retrospective study, 57 of 95 consecutive HeartMate 3 LVAD recipients (mean age 57 ± 11 years; 79% male) with adequate echocardiographic quality were analyzed. Patients were stratified by RH

Quantitative Evaluation of Right Ventricular Workload Based on the Stroke Work Index in Patients after Right Ventricular Outflow Tract Reconstruction.

The evaluation of right ventricular workload is sometimes complicated in patients after right ventricular outflow tract reconstruction (RVOTR) because both stenotic and regurgitation lesions are involved. In this study, we modified the right ventricular stroke work index (RVSWI) and evaluated the relationship between the modified RVSWI (mRVSWI) and patient prognosis after RVOTR.We enrolled 69 patients who underwent RVOTR (the RVOTR group), including those who needed early reoperation (early reoperation subgroup) and those who did not (follow-up subgroup), and 13 age-matched control participant

Intermittent right upper extremity ischemia in a patient with simultaneous aberrant right subclavian artery and patent foramen ovale: A case report.

BACKGROUND: Upper extremity intermittent ischemia due to non-aneurysmal, not occluded aberrant right subclavian artery (ARSA) is rare. CASE PRESENTATION: We describe a 30-year-old male who suffered from PFO and non-aneurysmal, not occluded ARSA, and presented by intermittent right upper extremity ischemia. He was treated by right carotid subclavian transposition for ARSA and antiplatelet medication for PFO. CONCLUSION: Authors assume that intermittent limb ischemia can occur secondary to anatomical changes in a patient without aneurysmal degeneration or occluded ARSA and the existence of pur

Asymptomatic right atrial myxoma originating from the inferior vena cava and right atrium junction in a patient with breast ductal adenocarcinoma.

We present asymptomatic right atrial myxoma in a patient with breast ductal adenocarcinoma. A 58-year-old female was referred to our clinic for the evaluation of the potential cardiotoxic side effects of anthracycline-based chemotherapy. She had a 10-month history of modified radical mastectomy for ductal adenocarcinoma of the breast, followed by adjuvant chemotherapy. To evaluate potential side effects of anthracycline-based regimen, transthoracic echocardiography was performed, which showed a highly mobile, cystic, and heterogeneous mass, 20 x 25 mm in size, located in the right atrium. Right ventricular myocardial activation delay in adult patients with right bundle branch block late after repair of Tetralogy of Fallot.

Electromechanical interaction, with prolonged QRS duration due to right ventricular (RV) overload, has been described as a predictor of unfavorable outcome in patients late after correction of Tetralogy of Fallot (TOF). Aim of our study was to evaluate myocardial function and activation delay of both left and right ventricles in TOF patients. Doppler echo, treadmill test and pulsed Tissue Doppler (TD) were performed in 25 healthy subjects and in 30 adult patients who had undergone surgery for TOF, all with right bundle branch block on ECG. Exclusion criteria were evidence of residual pulmonary

Postinfarct refractory right ventricle: right ventricular exclusion. A possible option to mechanical cardiac support, in patients unsuitable for heart transplant.

BACKGROUND: Postbypass refractory right ventricle (RRV) may develop due to right ventricular (RV) ischemia or infarction. In cases with RV infarction, recovery is often prolonged and salvage rate is extremely poor. In this retrospective study, we have examined the role of right ventricular exclusion (RVE), as a possible option to conventional weaning or bridging to heart transplant (B-HTX), in patients who were unsuitable for heart transplant. METHODS: During last 5 years, cumulative incidence of postbypass refractory circulatory failure (RCF) in our adult patients was 0.39% (26/6542). This p

Incident Atrial Fibrillation and Flutter in Patients With Pulmonary Arterial Hypertension: Influence of Right Ventricular Dilatation and Reduced Right Atrial Function.

BACKGROUND: The relationship between right atrial (RA) structural and functional remodeling and risk of incident atrial fibrillation/flutter (AF/AFL) in pulmonary arterial hypertension remains poorly defined. METHODS: In a multicenter observational cohort study of 326 patients with pulmonary arterial hypertension, we evaluated associations between RA structure and function, as determined by echocardiography, and incident AF/AFL. Incidence rates were calculated using time-to-event analysis. Cox proportional hazards models identified factors associated with incident AF/AFL, and Harrell's C-stat

Implantation of a temporary microaxial left ventricular assist device via the right axillary artery in a patient with aberrant right subclavian artery.

A temporary microaxial left ventricular assist device is commonly implanted via the right axillary artery to treat decompensated heart failure or cardiogenic shock. However, limited literature describes implantation in a variant aortic arch anatomy setting. We present a case of implantation via the right axillary artery in a patient with an aberrant right subclavian artery (Arteria Lusoria) as a bridge to heart transplantation.

Rationale and design of the Nephrotic Syndrome Study Network (NEPTUNE) Match in glomerular diseases: designing the right trial for the right patient, today.

Glomerular diseases are classified using a descriptive taxonomy that is not reflective of the heterogeneous underlying molecular drivers. This limits not only diagnostic and therapeutic patient management, but also impacts clinical trials evaluating targeted interventions. The Nephrotic Syndrome Study Network (NEPTUNE) is poised to address these challenges. The study has enrolled >850 pediatric and adult patients with proteinuric glomerular diseases who have contributed to deep clinical, histologic, genetic, and molecular profiles linked to long-term outcomes. The NEPTUNE Knowledge Network, co

Right ventricle systolic function and right ventricle-pulmonary artery coupling in patients with severe aortic stenosis and the early impact of TAVI.

Few data are available on the prevalence of right ventricle (RV) systolic dysfunction, assessed including RV strain, and RV to pulmonary artery (PA) coupling in patients with aortic stenosis (AS) submitted to TAVI and the early effect of the procedure. We performed standard and speckle tracking echocardiography in 80 patients with severe AS the day before TAVI and within 48 h after TAVI. In all patients we measured TAPSE/PASP (cut-off for RV-PA uncoupling 0.31) and in 60/80 we were able to analyze RV global longitudinal strain (RV-GLS) and RV free wall strain (RV-FWS). RVFAC and TAPSE were imp
