Periodic Fracture of Active Tissues.

Fracture typically signifies mechanical failure in engineering materials, whereas controlled cracking may actively sculpt tissues through precise biological regulation. Here, we establish a multiscale nonlinear peridynamic theory that accounts for cellular mechanosensing to decipher the spontaneous fracture of active tissues. We show that tissues cultured in a ring-shaped domain can undergo periodic fracture to generate multicellular aggregates with regular spacing, recapitulating prior morphogenetic experiments on avian dermal cell collectives. It is found that the number of cracks varies non

Hypokalemic Periodic Paralysis Syndrome.

Hypokalemic periodic paralysis (HPP) is a rare neuromuscular disorder characterized by recurrent episodes of acute muscle weakness or paralysis secondary to hypokalemia. Attacks are often triggered by metabolic or physiologic stressors and may require critical care management. We present two cases of HPP, highlighting challenges in management. A 22-year-old male and a 42-year-old male presented with severe hypokalemia and acute flaccid paralysis requiring intensive care unit admission due to profoundly low potassium levels. Both patients required aggressive repletion with symptom resolution. D

Dietary Supplements in the Perinatal Period.

The physiologic changes of pregnancy, in addition to the nutritional needs of the growing fetus, require supplementation and dietary changes during pregnancy. Additional macronutrient needs include protein and polyunsaturated fatty acids. Micronutrients also have important roles in supporting maternal health and fetal development, including vitamins A, D, C, B 6 , B 12 , and K, in addition to iodine, iron, choline, zinc, calcium, and folate. The most common form of supplementation in pregnancy is oral prenatal vitamins, although these can vary in their composition and inclusion of certain micr

Listeria rhombencephalitis in the postpartum period.

We present a case of Listeria rhombencephalitis (brainstem encephalitis) with abscess formation in a healthy woman in her late 30s at 6 weeks postpartum, presenting with acute onset of fever, headache and neurologic deficits. The diagnosis was confirmed with a positive Listeria monocytogenes cerebral spinal fluid (CSF) PCR assay and contrast-enhanced brain MRI revealing multiple brainstem abscesses. The patient's course was complicated by aspiration pneumonia due to bulbar dysfunction and neuromuscular respiratory failure, requiring prolonged intubation. The patient was treated with an 8 week

Autoinflammatory Diseases/Periodic Fevers.

Children with intermittent fevers present to pediatricians and other primary care child health providers for evaluation. Most patients will have self-limited, benign infectious illnesses. However, the possibility of a periodic fever syndrome should be considered if febrile episodes become recurrent over an extended period and are associated with particular signs and symptoms during each attack. This review discusses the current conceptualization of autoinflammatory diseases with specific focus and detail on familial Mediterranean fever; tumor necrosis factor receptor-associated periodic syndro

[Loss of sight in the postpartum period].

BACKGROUND: Vision problems in case of pre-eclampsia or Hemolysis, Elevated Liver, Low Platelets syndrome (HELLP) occur in 25-40% of the cases. Ablatio retinae as a complication occurs in only 0,1-2%. CASE DESCRIPTION: This article describes the case of a healthy 31-year-old woman who gave birth to her first child. A few hours after delivery she experienced vision loss. HELLP was diagnosed. Because of persistent vision loss combined with headache, the ophthalmologist and neurologist were consulted. A bilateral ablatio retinae as a complication of HELLP was diagnosed. Headache was most likely

Use of diuretics in the neonatal period.

The use of diuretics is extremely frequent in sick neonates, the more so in very premature newborn infants. The use of diuretics in patients whose kidney function is immature necessitates a thorough knowledge of renal developmental physiology and pathophysiology. This review presents the basic aspects of body fluid homeostasis in the neonate, discusses the development of kidney function, and describes the mechanisms involved in electrolyte and water reabsorption along the nephron. Diuretics are then classified according to the site of their action on sodium reabsorption. The use of diuretics i

Anesthesia-Associated Periodic Discharges.

PURPOSE: Anesthetic agents have been widely used in the treatment of refractory status epilepticus and the medical management of increased intracranial pressure whenever the goal is therapeutic burst suppression. Periodic patterns typically consisting of generalized periodic discharges (GPDs) following emergence from anesthesia have been described in several case reports. However, their clinical significance and in particular whether these patterns are epileptiform remains unclear. METHODS: This is a single-center, retrospective, observational study examining EEG patterns following emergence

Reconsideration of Periodic Psychosis of Adolescence.

The condition of periodic psychosis of adolescence based on the clinical features of recurrent depressive symptoms, sub-stupor, and psychotic symptoms whose features return to a normal state within 2 weeks with no residual symptoms has been often seen during adolescence. However, international recognition of periodic psychosis of adolescence is low and the condition is not recognized as an independent disease in ICD-10 or DSM-5. We presented a case report of a depressive episode central to periodic psychosis of adolescence in a 16-year old female. The symptoms presented in the case correspond

Ulcerative colitis in the postpartum period.

We describe a scarcely reported case in which ulcerative colitis (UC) occurred in the postpartum period. The aims of this case report are to reinforce the recent assertion that a diet is a ubiquitous environmental factor in inflammatory bowel disease (IBD) and that a plant-based diet (PBD) is recommended for IBD. A 29-year-old woman normally delivered her first child. She first noticed bloody diarrhea 4.5 months after delivery. She was diagnosed with UC (left-sided colitis, moderate severity). Sulfasalazine induced remission. She then experienced and learned about PBD during an educational hos

[A case of periodic thyrotoxic paralysis].

Periodic thyrotoxic paralysis (TPP) is a disease that usually affects Asian men from 20 to 50 years. It is secondary to genetic mutations of ion channels for the transport of sodium and potassium. It is characterized by recurrent and brutal bouts of paresis / flaccid paralysis, without sensory involvement, in a context of hyperthyroidism. Seizures preferentially affect proximal muscles and are often associated with severe hypokalemia. However, serious repercussions are rare, except in cases of extreme weakness of the respiratory muscles or cardiac arrhythmia induced by hypokalemia. The treatme

Periodic focal epileptiform discharges.

OBJECTIVE: To report intraoperative periodic focal epileptiform discharges (PFEDs) during awake craniotomy using high-density electrocorticography (HD-ECoG). METHODS: We retrospectively analyzed 81 patients undergoing awake craniotomy between 9/29/2016 and 7/5/2018. Intraoperative HD-ECoG was performed with direct electrocortical stimulation (DECS) for functional brain mapping. Real-time interpretation was performed and compared to scalp EEG when performed. Perioperative seizures, surgical complications, and characteristics of PFEDs were assessed. RESULTS: 69/81 patients (mean age 48.5 years

Hyperkalaemic periodic paralysis in pregnancy.

Hyperkalaemic periodic paralysis is a rare skeletal muscle disorder which is characterised by episodic muscle paralysis associated with hyperkalaemia. Although it is an autosomal-dominant disease, cases of de novo mutations have been reported. We report the case of a 30-year-old woman, gravida 5 para 3+1, who was planned for an elective repeated caesarean section at 38 weeks and 3 days of pregnancy. She developed recurrent episodes of hyperkalaemic periodic paralysis after receiving corticosteroids. Intravenous calcium gluconate was administered to normalise potassium levels (from 6.3 mmol/L t
