Effects of implant geometry and surface treatment on osseointegration after functional loading: a dog study.

The purpose of this study was to evaluate the geometry and surface characteristics of osseointegration after functional loading by radiographic, periodontal and histomorphometric analyses. We analysed three groups of implants with different geometry and surface characteristics using experimental dogs. The control group received Brånemark implants (group 1). Group 2 and group 3 implants each had a 0.5-mm pitch height but differed in surface characteristics. Group 2 implants were machine surfaced and group 3 implants were thermally oxidized at 800 degrees C for 2 h in a pure oxygen atmosphere. F

Implant-surgical and prosthetic rehabilitation of patients with multiple dental aplasia: a clinical report.

The expanded experience with oral implants and supplementary augmentation techniques has opened new possibilities for treating patients with oligodontia or anodontia with fixed prostheses. A problem in treating such patients is the need to place implants in growing maxillae or mandibles, as many of these patients are children or adolescents. When implant treatment is postponed until the patient is full grown, dysfunctions become manifest, which necessitates extensive surgical measures to achieve a fixed prosthetic restoration. This report illustrates the problems associated with different conc

Microleakage at the abutment-implant interface of osseointegrated implants: a comparative study.

Microleakage can occur at the abutment-implant (A-I) interface in osseointegrated implants and may cause malodor and inflammation of peri-implant tissues. The degree of microleakage at the A-I interface of 5 implant systems was comparatively assessed at varying closing torques. Using colored tracing probes driven by a 2-atm pressure system, the interface microleakage of Brånemark, Sulzer Calcitek, 3i, ITI, and Steri-Oss implants was determined spectrophotometrically. Microleakage through the A-I interface occurred in all systems, with variability between systems, samples, and closing torques.

Comparison of three-dimensional printed resin crowns and preformed stainless steel crowns for primary molar restorations: a randomized controlled trial.

The importance of aesthetics in children has increased over time. Therefore, this multicenter randomized clinical trial aimed to analyze and compare three-dimensional (3D)-printed resin crowns (RCs) as a potential alternative to stainless-steel crowns (SSCs) for restoring primary molars with extensive carious lesions. According to the null hypothesis, no statistically significant difference was observed in restoration failure between RC and SSC groups. A total of 56 primary molars after pulp treatment at two dental hospitals were included. After pulp treatment, the teeth were randomly divided

Soft- and hard-tissue augmentation by orthodontic treatment in the esthetic zone.

A technique that may effectively create a greater volume of available hard and soft tissue in the vertical plane without surgical intervention is reported and explained. Limitations of the forced eruption are also discussed. Creating an esthetic implant-supported restoration is a challenge in patients who have alveolar resorption and/or attachment loss, especially when they present with a high smile line. Many methods to augment this loss of tissue have been proposed; most involve surgical procedures to add bone or bone substitutes to compensate for the loss of alveolar tissue. This case repor

Implant-assisted overdenture: a case report.

A patient's extreme susceptibility to periodontal disease, likely exacerbated by a history of smoking, resulted in severe periodontal breakdown. Employing a systematic approach that identified both patient risk and prognosis using data gathered during the patient examination, a treatment plan was developed that included extraction of structurally compromised teeth and the use of implant-assisted complete overdentures. This treatment approach accomplished a reduction of biomechanical, periodontal, and dentofacial risk, as well as an increase in functional harmony.

Implant failure associated with actinomycosis in a medically compromised patient.

Oral actinomycosis is not a common disease, but it can cause massive destruction. This article reports a case of implant failure associated with actinomycosis. A 55-year-old Caucasian male patient had tooth #20 extracted years ago and an implant placed 3 years ago. The #20 implant area developed an abscess about 1½ years after implant placement. Radiographic findings revealed a large radiolucency on the mesial aspect of the #20 implant. The implant was surgically removed and the lesion thoroughly debrided. The patient experienced severe pain when the apical soft tissue was curreted following i

Treating the oral sequelae of an acoustic neuroma.

This article describes the treatment of oral complications that arose after surgery for the removal of an acoustic brain tumor. The authors contend that a thorough oral examination can pinpoint the cranial nerves that have been affected by the tumor or the surgery, and that dental appliances can offer special protection after surgery.

A multidisciplinary approach for the rehabilitation of a patient with an excessively worn dentition: a clinical report.

This clinical report describes a multidisciplinary approach to the diagnosis and treatment of a patient with a severely worn dentition. The treatment included osteotomy and immediate implant placement and loading in the mandible. The definitive restorations were implant- and tooth-supported metal ceramic restorations. These restorations were fabricated with metal occlusal surfaces at an increased occlusal vertical dimension, which provided acceptable esthetics and function.

The Fifth German Oral Health Study (Fünfte Deutsche Mundgesundheitsstudie, DMS V) - rationale, design, and methods.

BACKGROUND: Oral diseases rank among the most prevalent non-communicable diseases in modern societies. In Germany, oral epidemiological data show that both dental caries and periodontal diseases are highly prevalent, though significant improvements in oral health has been taking in the population within the last decades, particularly in children. It is, therefore, the aim of the Fifth German Oral Health Study (DMS V) to actualize the data on current oral health status and to gather information on oral health behavior and risk factors. In addition to current oral health monitoring, the study wi

Implant-retained overdentures for young children with severe oligodontia: a series of four cases.

PURPOSE: The treatment need is high in children with severe oligodontia and anodontia, because they often have functional and esthetic problems owing to missing teeth. Because the intraforaminal region barely grows after eruption of the permanent mandibular incisors, dental implant treatment should be considered a treatment option for these children. The purpose of our study was to assess the treatment outcomes regarding satisfaction and the care and aftercare of implant-retained mandibular overdentures in a series of 4 young children without erupted mandibular teeth from either severe oligodo

Severe deep neck space infections and mediastinitis of odontogenic origin: clinical relevance and implications for diagnosis and treatment.

CONCLUSIONS: Early diagnosis and aggressive antimicrobial and surgical treatment are essential to successfully treat extensive cervico-mediastinal abscesses of odontogenic origin. Patient management should be performed by experienced clinicians well trained in managing possible complications. We recommend close clinical and radiological postoperative follow-up investigations with early surgical re-intervention if necessary. OBJECTIVES: While neck infections affecting the perimandibular space have a high prevalence and their clinical aspects have repeatedly been discussed, further spread of th

Implant failure and subsequent treatment: report of a complex case.

In a 40-year-old woman, a central incisor was to be replaced with an implant-supported crown. When the implant was uncovered, the soft tissue around the implant showed an unfavorable profile: The adjacent lateral incisor showed significant loss of periodontal attachment. Subsequent soft tissue and connective tissue grafts failed to correct the problem, and an alternative treatment plan was formulated. Errors in the initial treatment, including incorrect implant position and failure to analyze the cause of the periodontal attachment loss are discussed in detail; the options for further treatmen

Tissue preservation and maintenance of optimum esthetics: a clinical report.

Today, most tooth replacement in the esthetic zone is done using implants placed in a delayed surgical protocol. Unfortunately, this delay can result in loss of both hard and soft tissue during the healing period, necessitating guided tissue regeneration techniques at the time of implant placement. Recent developments with tapered implants have facilitated predictable immediate implant placement, preserving the osseous structure surrounding the socket. Further developments with custom healing abutments can preserve the crestal soft tissues, including the papillae. This article reviews techniqu

Effective periodontal treatment in a patient with type IIA von Willebrand's disease: report of a case.

von Willebrand's disease (vWD) is one of the most common hereditary hemorrhagic disorders. A mild to moderate deficiency of factor VIII and von Willebrand factor (vWf) often is associated with gingival bleeding. In this case report, the periodontal treatment of a patient with vWD is described. A 45-year-old woman with type IIA vWD was referred for periodontal therapy because of an episode of gingival hemorrhage and percussion pain of teeth #18 and #47. The periodontal findings included probing depths ranging from 2 to 6 mm, horizontal bone loss, and Class II furcation involvement of tooth #46.

Clinical report with up to 4 years of follow-up on a cervically modified stepped screw-type implant.

PURPOSE: In 1998, a modification of the macrostructure of the Frialit-2 implant in the cervical region was introduced to stabilize peri-implant bone. Limited data are available on the clinical effect of this modification. Therefore, the soft-tissue situation, marginal bone loss, and implant failure rate were analyzed after 4 years of clinical experience with the modified Frialit-2 Synchro implant. MATERIALS AND METHODS: From 1998 to 2001, 190 cervically modified implants were placed and documented prospectively in 58 patients. Of these implants, 147 were placed in original jaw bone, 22 in are
